The following terms and conditions (Terms) govern your enrollment and participation in a dental membership plan (Dental Membership Plan) being offered by Dr. Zak Dental Care (Dentist). These terms and your enrollment in a Dental Membership Plan constitute a binding agreement between you and Dr. Zak Dental Care (the Agreement). If you do not agree to these Terms, do not enroll in this Dental Membership Plan. THE DENTAL MEMBERSHIP PLAN IS NOT AN INSURANCE PLAN AND IS NOT INTENDED TO REPLACE DENTAL INSURANCE. THE DENTAL MEMBERSHIP PLAN IS NOT A QUALIFIED HEALTH PLAN UNDER THE AFFORDABLE CARE ACT OR A MEDICARE PRESCRIPTION DRUG PLAN. THE MEMBERSHIP FEE IS NOT AN INSURANCE PREMIUM. NEITHER THE DENTAL MEMBERSHIP PLAN, OR THE DENTIST OFFERING THE DENTAL MEMBERSHIP PLAN, ARE LICENSED INSURERS, HEALTH MAINTENANCE ORGANIZATIONS, OR OTHER UNDERWRITERS OF HEALTH CARE SERVICES.

  • You may not combine a Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion
  • It is possible that, during the course of treatment by a Dentist, he or she may discover a dental or health condition, such as periodontal disease, that may require further treatment the costs associated with any further treatment are your responsibility
  • 2. ACCOUNT SIGN-UP. To enroll in a Dental Membership Plan using the zakdental.com online platform, you must open an account (the Account). You represent that all information you provide during the Account sign-up process and at any time thereafter, for yourself and for all family members (collectively, the Account Information), will be true, accurate, complete, and current, and that you will promptly update your Account Information as necessary such that it is, at all times, true, accurate, complete, and current. The Plapfrom will use and maintain your Account Information subject to and consistent with the terms set forth in our Privacy Policy. The Agreement incorporates the terms and conditions set forth in our Privacy Policy. By accessing the zakdental.com online platform and/or participating in the Dental Membership Plan, you are consenting to have your personal data and the personal data of your family members used by us as set forth in the Privacy Policy. You may enroll your spouse under your Account. You also may enroll dependent minors in the Dental Membership Plan, provided that you are the legal parent or guardian for each individual enrolled under your Account. To create an Account, you must be 18 years or older, have a unique email address, and have a payment method registered with the zakdental.com online platform. You will be required to provide your name and email address and to create a password. To access the Account, the email address and password will be required. As the Account holder, you should not reveal the Account Information to anyone else. You are responsible for maintaining the confidentiality and security of your Account Information and for all activities that occur on or through the Account.

    3. PAYMENT TERMS AND PROCESSING. The Dentist offering the Dental Membership Plan that you choose will specify available payment plans, such as paying month-to-month or paying annually. In each case, Membership Fees will be due in advance, and your payment method on file will be billed accordingly. The zakdental.com online platform uses a third-party payment processor to collect and process all payments. In order to make payments you may be required to agree to the third-party terms and conditions and/or privacy policy. You are responsible for the timely payment of all fees. You agree that the zakdental.com online platform may process payment for all Membership Fees associated with the Dental Membership Plan that you choose using the payment method (e.g., credit card, debit card, etc.) that you have provided in your Account Information. It is your responsibility to notify your Dentist if your payment method has changed. If you do not provide a valid payment method, or if your designated payment method is determined to be inactive for any reason, your Dentist may immediately discontinue your enrollment in the Dental Membership Plan. You agree not to cause your credit or debit card company to reverse or chargeback any fees charged in accordance with this Agreement; and in the event you do so, we and/or your Dentist may terminate your enrollment in the Dental Membership Plan, and you agree to reimburse us and/or your Dentist for any costs incurred in responding to such chargeback, including, without limitation, our and your Dentists actual costs paid to the credit or debit card company, including chargeback fees, and the value of the time our and your Dentists employees spend on the matter as determined in our or your Dentists discretion in good faith. Your Dentist may increase the Membership Fee of the Dental Membership Plan from time to time. You will receive an email notification to the email address you registered on the zakdental.com online platform of any such increase in fees no less than thirty (30) days before the new rate becomes effective. If you do not agree to the increase(s), you may cancel your membership. By establishing an Account and enrolling in a Dental Membership Plan through the zakdental.com online platform, you authorize the Dentist in whose Dental Membership Plan you are enrolling to bill for the monthly Membership Fee or annual Membership Fee. If your payment method on file or billing information changes, it is your responsibility to provide updated information to us on a timely basis. Your zakdental.com online platform / Member Account dashboard will include real-time data with respect to your enrollment information, including the beginning of the current enrollment period and a history of your payments of Membership Fees.

    4. LIMITATIONS. Not all dental services are Included Services under the Dental Membership Plan. Only those specifically listed by your Dentist as Included Services are provided by your Dentist at no additional charge to you. If you require any dental services that are not Included Services, you are responsible for paying your Dentist for those Non-Included Services at the rates established by your Dentist. You may also be subject to a Dentists late payment fee and other applicable office policies. Any disputes over the fees charged by dentists must be addressed directly with the Dentist. Use of the zakdental.com online platform requires compatible devices, internet access, a unique email address, and certain software; and may be affected by the performance of these factors. You agree that meeting these requirements, which may change from time to time, is your responsibility. We do not represent or guarantee you will be able to access the zakdental.com online platform at all times.

  • Member Cancellation during the initial ninety (90) day period. * If you cancel your membership in the Dental Membership Plan during the initial ninety (90) days* following your enrollment (whether such payment is a monthly payment or an annual payment) your Dentist will provide a full refund of your paid Membership Fees. NOTWITHSTANDING THE FOREGOING, IF YOU HAVE RECEIVED ANY DENTAL SERVICES UNDER YOUR DENTAL MEMBERSHIP PLAN WITHIN THE FIRST NINETY (90) DAYS* AND CANCEL YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN AFTER RECEIVING THOSE SERVICES, YOUR DENTIST WILL BILL YOU FOR THE SERVICES RENDERED TO YOU AT YOUR DENTISTS THEN-CURRENT, NONDISCOUNTED UCR (USUAL AND CUSTOMARY RATES). PAYMENT IN FULL FOR ANY SERVICES RENDERED BY YOUR DENTIST DURING THIS PERIOD WILL BE REQUIRED PRIOR TO CANCELLATION OF YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN, AND THE ISSUANCE OF A REFUND.
  • Billing Error. In order to be considered a billing error you must provide sufficient written documentation to demonstrate the existence of a billing error and adequate proof of the amount paid due to such error.
  • Dental Membership Plan Termination. If your Dentist terminates your membership in a Dental Membership Plan without your consent or permission, or ceases offering a Dental Membership Plan, your Dentist will provide a pro rata refund to the payer on your Account.
  • 7. COMPLAINTS. All complaints about the dental services provided by the Dentist, the fees you were charged, or any other dissatisfaction with a Dentist, must be addressed directly with the Dentist. You may submit your complaint via our online help request system or by contacting our team via email.

    8. GOVERNING LAW; ARBITRATION. The Agreement shall be governed by and construed in accordance with the laws of the State of California, without reference to conflicts of those laws provisions and, as to matters affecting copyrights, trademarks and patents, by U.S. federal law. Any dispute or claim arising out of, or in connection with, this Agreement shall be finally settled by binding arbitration in Los Angeles, California, in accordance with N.C. Gen. Stat. 1-569.1 et seq. (the Uniform Arbitration Act) and the then-current rules and procedures of the American Arbitration Association by one (1) arbitrator appointed by the American Arbitration Association. The arbitrator shall apply the law of the State of California, without reference to rules of conflict of law or statutory rules of arbitration, to the merits of any dispute or claim. Judgment on the award rendered by the arbitrator may be confirmed, reduced to judgment and entered in any court of competent jurisdiction. You agree that, any provision of applicable law notwithstanding, the arbitrator shall have the authority to award the prevailing party its costs and reasonable attorneys fees. In the event that the above arbitration provision is held invalid or unenforceable, then any dispute with respect to the Agreement shall be brought and heard either in the California state courts located in Los Angeles, California, or the federal district court located in Los Angeles, California. In such event, you consent to the in personam jurisdiction and venue of such courts. You agree that service of process upon you in any such action may be made if delivered in person, by courier service, by first class mail, and shall be deemed effectively given upon receipt.

    9. CHANGES TO THESE TERMS AND CONDITIONS. Dr. Zak Dental Care reserves the sole right at any time to modify, discontinue, or delete portions of these Terms. We will notify you of any material changes to these Terms by email to the email address that you have registered on the zakdental.com online platform. It is your responsibility to check your email periodically for changes to the Terms. If you do not agree with the new Terms, you must cancel your membership and stop using the zakdental.com online platform Services. Your continued use of the zakdental.com online platform, the zakdental.com online platform Services and/or the Dental Membership Plan after any changes to these Terms constitutes your acceptance of the revised Terms and Conditions.

    As per your dentist I need more than just dental cleaning done and it has been a week since I started requesting a call from my care coordinator as all the text messages I am receiving are for dental cleaning only on dates that do not work for me. In addition, when I reply to the text messages your team never text back. I have called your support team 9 times in the past 7 days and I even stopped by your location in Ventura to schedule the appointment but no one has been able to help. I am very frustrated as I have been ignored for 7 days already and although your support team continues to tell me that the issue has been escalated, I continue to receive the same text offering a time that does not work for me and is for dental cleaning only. Zak Dental seems to be nothing but a dentistry group without the ability to efficiently schedule patients for treatment. However, when it comes to make a patient feel ignored and frustrated, your group do actually do a great job.

    I was treated by the gentle Dr Shaeffer & his kind assistant for crown & it was THE EASIEST, MOST COMFORTABLE & QUICK dental procedure I have experienced! Injections were a breeze, no pain during procedure & they had it synced it went so fast! Relaxing music & their calm personalities + a blanket when I got cold= 5 stars

    Such an inefficient and unprofessional operation. If you would like to schedule an appointment, they will suggest a day and time via text message. If such time does not work for you, they will continue to suggest one date at a time until it works. If you call them to make the process more efficient, they will tell you that someone will call you within 24 to 48 hours. 3 days later, I have not received such call and continue to get text messages. Awful customer service.

    This place is made of nightmares, I had an infected tooth and was supposed to get a root canal and got cancelled and they forgot to call me, I then reschule and AGAIN the ONLY dentist is out. Don't even get me started on having to set up 3 different appointments BEFORE getting any actual work done. Like don't need a consultation I need a root canal you thieves. I called because I had a fever AND PAIN due to this infected tooth and their incompetent baboons of employees told me to wait another 2 weeks to get seen. These people quoted me 5k worth of work, I flew to Mexico and got it all done for $600 including medication. $1000 in airfare, tourism, dental anesthesia and medication. It's now 2022 and my teeth from Mexico are still strong. These people run a scam with shady people. Probably the bottom of the barrel employees

    Not a real dentist. I called to make an normal cleaning/check up appointment. I was told I would first do a consultation before having a treatment done. I declined and told him I just wanted a regular cleaning and check up. He said ok and scheduled me. I then got bombarded with 4 emails, 2 phone calls and 2 text messages reminding me of my appointment after I confirmed the first time. I show up 15 min early for my appointment and the girl checking me in takes 15 min to do so because she has no clue what she's doing. Then after sitting there for 20 min she has me start signing forms. She tells me that I need to sign a form for tele health so that my X-rays can be read. I proceeded to ask her what I'm having done today and she tells me today I get X-rays which decide what treatment I need. I told her that's not what I agreed to, to which she replied that that's what was happening today. Absolute waste of my time and hoax of a dentist office.

    The dental care is fine, the office is clean. But getting a hold of anyone at the Ventura office is so frustrating. This phone number just goes to a general call centre, where you either get a "call back" or they say they "are experiencing an unprecedented amount of calls". Then you don't get a phone call back for at least a day or at all. It's so hard when you need to find out information, or ask questions, etc. DR Zak- please make it possible to call the office directly and speak with someone. It's not asking too much at all, most every other dentists/orthodontist has a direct line with a receptionist- I do not understand why they don't have this? Update- I was recently there for an x-ray and waited 30 mins, they told me there was tech difficulties so they sent me home with no X-ray! Now I am researching new dentists, I should've done this sooner.

    Alize and Yuliana were super sweet and very helpful explaining the process in the office. I had my virtual with Dr. Park which was even more amazing he showed me every x-ray and explained why I need the treatment. Plus Dr. Zak has membership plan that really help people that don't have dental insurance and it comes with an electric toothbrush even more amazing!!!!

    VERY DISAPPOINTED My last experience was really bad. I got two cavities filled. I had to go back two different times because my bite didn't match up. Then I had to go back because those teeth were painful. It ends up the dentist told she got a nerve in each tooth and said I'd have to get a root canal. It's very difficult to get through on the phone. The office staff and the people who clean your teeth are nice but the Dentist did not have good bedside manner,especially for someone who get anxiety with going to the dentist I don't have any negative reviews like this.

    The drill-free, no anaesthesia dentistry that Dr. Zak offer, works best for me and for my 15yr old daughter. We had dental anxiety before the visit, but the new treatment and the whole staff made our experience totally different, it was actually awesome. Our dentist is incredibly smart and didn't waste any of our valuable time, he did what's suitable for us who are afraid of injections and pains. Amazing experience, sure to come back after 6mos.     Had multiple negative experiences with Zak Dental Group. First of all, good luck trying to get a hold of any office as they only take calls to their call center.. your root canal got infected? well tough luck, you won't be able to reach your dentist. If I had known this before I would have never sought treatment with them. *Note* Why don't the offices take calls directly? I was in multiple offices and the receptionists did NOT seem busy at all(on their phone most of the time) Second, they use predatory businesses practices to get you in treatment and hit you with a bill. I had a metal filling removed and a crown added. Why? Because they just wanted to front me the bill as it wasn't covered by my insurance. During my initial consultation I was given the impression that my treatment would be a necessity, it wasn't until after months after that I realized what happened as none of my authorizations were approved and the only treatment that was given to me was one that I paid out of pocket for. On top of that they charged me an extra 170 dollars more for a treatment that wasn't even performed.. took them more than a year to refund my money, even after multiple calls(now you see why they use a call center) Third. I had an orthodontic consultation with Dr Kristen Miller and wanted to find out more information about reconstructive orthognathic surgery.. I took time off work and traveled to their office only to find out that I wasted my time. She basically shot down the idea of my insurance EVER covering this kind of surgery, therefore making orthodontics rather useless. A BIG WASTE OF TIME. Well guess what Dr Miller, i'm happy to say that last month, my insurance approved my double jaw surgery. Just goes to show you how little their practitioners know. *edit* Just read the reply from Zak Dental.. laughable to say the least.. my dental insurance is the one that approved my jaw surgery.. just goes to show you how uninformed this Dental group is. Avoid them at all costs! *Second edit* it appears they deleted their original reply LMAO. Pathetic dental group. Do yourself a favor and seek treatment elsewhere. You've been warned.

    worst experience ever, i went there to get a couple of teeth replaced on my upper denture plus a small set for a couple of missing back teeth, my priority was the front missing teeth, its been over 4 months now and they just barely gave me the small back teeth set, nothing has been done about my front teeth, their still broken and embarrassing , they give me the same excuse every time , they dont know how long its going to take, they will know on my next visit, now my next visit is in Jan. that would make it almost 5 month

    Wow that was brutal. Worst medical experience of any type of my life. I think Zak is a franchise of some sort. You don't even see a dentist on your first consultation. There is a zoom call that you need to do afterwards. The dentist on the zoom call was great but the secretary was super rude. I don't think she actively didn't like me I just think she hated her job deeply and you could feel it. At a coffee shop thats fine but with your health it doesn't feel good or safe when there is complete disregard for you as a person. a kind of "set up the appointment and get out of my face" vibe. I will continue a couple more days in pain in order to use a different dentist.

    As a mom you need to find an office that could help you about your kids over all dental health, especially during challenging times of their lives. That's why I'm lucky coz I have Dr. Zak Dental for my children. Have tested their services for years already and more years to look forward to. Great facility!
