The following terms and conditions (Terms) govern your enrollment and participation in a dental membership plan (Dental Membership Plan) being offered by Dr. Zak Dental Care (Dentist). These terms and your enrollment in a Dental Membership Plan constitute a binding agreement between you and Dr. Zak Dental Care (the Agreement). If you do not agree to these Terms, do not enroll in this Dental Membership Plan. THE DENTAL MEMBERSHIP PLAN IS NOT AN INSURANCE PLAN AND IS NOT INTENDED TO REPLACE DENTAL INSURANCE. THE DENTAL MEMBERSHIP PLAN IS NOT A QUALIFIED HEALTH PLAN UNDER THE AFFORDABLE CARE ACT OR A MEDICARE PRESCRIPTION DRUG PLAN. THE MEMBERSHIP FEE IS NOT AN INSURANCE PREMIUM. NEITHER THE DENTAL MEMBERSHIP PLAN, OR THE DENTIST OFFERING THE DENTAL MEMBERSHIP PLAN, ARE LICENSED INSURERS, HEALTH MAINTENANCE ORGANIZATIONS, OR OTHER UNDERWRITERS OF HEALTH CARE SERVICES.

  • You may not combine a Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion
  • It is possible that, during the course of treatment by a Dentist, he or she may discover a dental or health condition, such as periodontal disease, that may require further treatment the costs associated with any further treatment are your responsibility
  • 2. ACCOUNT SIGN-UP. To enroll in a Dental Membership Plan using the zakdental.com online platform, you must open an account (the Account). You represent that all information you provide during the Account sign-up process and at any time thereafter, for yourself and for all family members (collectively, the Account Information), will be true, accurate, complete, and current, and that you will promptly update your Account Information as necessary such that it is, at all times, true, accurate, complete, and current. The Plapfrom will use and maintain your Account Information subject to and consistent with the terms set forth in our Privacy Policy. The Agreement incorporates the terms and conditions set forth in our Privacy Policy. By accessing the zakdental.com online platform and/or participating in the Dental Membership Plan, you are consenting to have your personal data and the personal data of your family members used by us as set forth in the Privacy Policy. You may enroll your spouse under your Account. You also may enroll dependent minors in the Dental Membership Plan, provided that you are the legal parent or guardian for each individual enrolled under your Account. To create an Account, you must be 18 years or older, have a unique email address, and have a payment method registered with the zakdental.com online platform. You will be required to provide your name and email address and to create a password. To access the Account, the email address and password will be required. As the Account holder, you should not reveal the Account Information to anyone else. You are responsible for maintaining the confidentiality and security of your Account Information and for all activities that occur on or through the Account.

    3. PAYMENT TERMS AND PROCESSING. The Dentist offering the Dental Membership Plan that you choose will specify available payment plans, such as paying month-to-month or paying annually. In each case, Membership Fees will be due in advance, and your payment method on file will be billed accordingly. The zakdental.com online platform uses a third-party payment processor to collect and process all payments. In order to make payments you may be required to agree to the third-party terms and conditions and/or privacy policy. You are responsible for the timely payment of all fees. You agree that the zakdental.com online platform may process payment for all Membership Fees associated with the Dental Membership Plan that you choose using the payment method (e.g., credit card, debit card, etc.) that you have provided in your Account Information. It is your responsibility to notify your Dentist if your payment method has changed. If you do not provide a valid payment method, or if your designated payment method is determined to be inactive for any reason, your Dentist may immediately discontinue your enrollment in the Dental Membership Plan. You agree not to cause your credit or debit card company to reverse or chargeback any fees charged in accordance with this Agreement; and in the event you do so, we and/or your Dentist may terminate your enrollment in the Dental Membership Plan, and you agree to reimburse us and/or your Dentist for any costs incurred in responding to such chargeback, including, without limitation, our and your Dentists actual costs paid to the credit or debit card company, including chargeback fees, and the value of the time our and your Dentists employees spend on the matter as determined in our or your Dentists discretion in good faith. Your Dentist may increase the Membership Fee of the Dental Membership Plan from time to time. You will receive an email notification to the email address you registered on the zakdental.com online platform of any such increase in fees no less than thirty (30) days before the new rate becomes effective. If you do not agree to the increase(s), you may cancel your membership. By establishing an Account and enrolling in a Dental Membership Plan through the zakdental.com online platform, you authorize the Dentist in whose Dental Membership Plan you are enrolling to bill for the monthly Membership Fee or annual Membership Fee. If your payment method on file or billing information changes, it is your responsibility to provide updated information to us on a timely basis. Your zakdental.com online platform / Member Account dashboard will include real-time data with respect to your enrollment information, including the beginning of the current enrollment period and a history of your payments of Membership Fees.

    4. LIMITATIONS. Not all dental services are Included Services under the Dental Membership Plan. Only those specifically listed by your Dentist as Included Services are provided by your Dentist at no additional charge to you. If you require any dental services that are not Included Services, you are responsible for paying your Dentist for those Non-Included Services at the rates established by your Dentist. You may also be subject to a Dentists late payment fee and other applicable office policies. Any disputes over the fees charged by dentists must be addressed directly with the Dentist. Use of the zakdental.com online platform requires compatible devices, internet access, a unique email address, and certain software; and may be affected by the performance of these factors. You agree that meeting these requirements, which may change from time to time, is your responsibility. We do not represent or guarantee you will be able to access the zakdental.com online platform at all times.

  • Member Cancellation during the initial ninety (90) day period. * If you cancel your membership in the Dental Membership Plan during the initial ninety (90) days* following your enrollment (whether such payment is a monthly payment or an annual payment) your Dentist will provide a full refund of your paid Membership Fees. NOTWITHSTANDING THE FOREGOING, IF YOU HAVE RECEIVED ANY DENTAL SERVICES UNDER YOUR DENTAL MEMBERSHIP PLAN WITHIN THE FIRST NINETY (90) DAYS* AND CANCEL YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN AFTER RECEIVING THOSE SERVICES, YOUR DENTIST WILL BILL YOU FOR THE SERVICES RENDERED TO YOU AT YOUR DENTISTS THEN-CURRENT, NONDISCOUNTED UCR (USUAL AND CUSTOMARY RATES). PAYMENT IN FULL FOR ANY SERVICES RENDERED BY YOUR DENTIST DURING THIS PERIOD WILL BE REQUIRED PRIOR TO CANCELLATION OF YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN, AND THE ISSUANCE OF A REFUND.
  • Billing Error. In order to be considered a billing error you must provide sufficient written documentation to demonstrate the existence of a billing error and adequate proof of the amount paid due to such error.
  • Dental Membership Plan Termination. If your Dentist terminates your membership in a Dental Membership Plan without your consent or permission, or ceases offering a Dental Membership Plan, your Dentist will provide a pro rata refund to the payer on your Account.
  • 7. COMPLAINTS. All complaints about the dental services provided by the Dentist, the fees you were charged, or any other dissatisfaction with a Dentist, must be addressed directly with the Dentist. You may submit your complaint via our online help request system or by contacting our team via email.

    8. GOVERNING LAW; ARBITRATION. The Agreement shall be governed by and construed in accordance with the laws of the State of California, without reference to conflicts of those laws provisions and, as to matters affecting copyrights, trademarks and patents, by U.S. federal law. Any dispute or claim arising out of, or in connection with, this Agreement shall be finally settled by binding arbitration in Los Angeles, California, in accordance with N.C. Gen. Stat. 1-569.1 et seq. (the Uniform Arbitration Act) and the then-current rules and procedures of the American Arbitration Association by one (1) arbitrator appointed by the American Arbitration Association. The arbitrator shall apply the law of the State of California, without reference to rules of conflict of law or statutory rules of arbitration, to the merits of any dispute or claim. Judgment on the award rendered by the arbitrator may be confirmed, reduced to judgment and entered in any court of competent jurisdiction. You agree that, any provision of applicable law notwithstanding, the arbitrator shall have the authority to award the prevailing party its costs and reasonable attorneys fees. In the event that the above arbitration provision is held invalid or unenforceable, then any dispute with respect to the Agreement shall be brought and heard either in the California state courts located in Los Angeles, California, or the federal district court located in Los Angeles, California. In such event, you consent to the in personam jurisdiction and venue of such courts. You agree that service of process upon you in any such action may be made if delivered in person, by courier service, by first class mail, and shall be deemed effectively given upon receipt.

    9. CHANGES TO THESE TERMS AND CONDITIONS. Dr. Zak Dental Care reserves the sole right at any time to modify, discontinue, or delete portions of these Terms. We will notify you of any material changes to these Terms by email to the email address that you have registered on the zakdental.com online platform. It is your responsibility to check your email periodically for changes to the Terms. If you do not agree with the new Terms, you must cancel your membership and stop using the zakdental.com online platform Services. Your continued use of the zakdental.com online platform, the zakdental.com online platform Services and/or the Dental Membership Plan after any changes to these Terms constitutes your acceptance of the revised Terms and Conditions.

    I had a prophylaxis cleaning done by Dr Asatani recently. Based on my results he is the ultimate dentist in his field. He provided quality care and dental work.

    I've been a client of Temecula Dental long BEFORE Dr. Zak took it over. I never had any real issues with Dr. Zak until recently. I was in severe pain from a cracked wisdom tooth, and the office couldn't see for another 3 weeks. Even though they knew it was an emergency situation. On the day I was suppose to get the tooth removed, the office scheduled me with a general dentist and not an oral surgeon. So They wanted me to wait an additional 2 weeks for the oral surgeon. Even after I paid in advance! I was literally in pain for over an a month and they did nothing to help me. They just left me hanging in the wind! I have since changed my dentist. Watch for their fees too. I was over charged twice! They had to refund a $1,600 and a $350 charge. That's almost a $2,000 mistake on their part because they don't see you in person, they use Zoom so they do a lot of missed diagnosis.

    Good luck trying to schedule an appointment! I tried for over a month to schedule with them...you don't get to talk to them, instead they send emails and texts back and forth. After trying to schedule just a consultation appointment for a month I gave up. Update: terrible response! well they said it themselves...there is no reason a remote team member shouldn't be able to handle scheduling. Get it together guys!!

    insurance had just changed and this office was assigned to me. i called for tooth pain and instead i was told about their wellness plan for 5 minutes and when i asked "what about my tooth pain?" i get an "i actually don't know" if you have a dental emergency try somewhere else

    New to the place, my friend dropped me and my son at this office along Winchester Rd. We were attended right away, took all the info related to health and dental history, and then took the impressions of my son's teeth. As a doting mom, I appreciated the friendliness and efficiency of the staff and technician we got chance to work with that day, that was Wednesday last week, 3pm. Sure to come back for the continuation. Thanks so much, it was a kid-friendly dental place.

    Three (3) months ago, my wife insisted that we should go and visit the nearest which is Zak Dental along Winchester Rd. Since we had a great time during the first encounter, that's with consultations and comprehensive x-rays, with staff and doctors who are truly diligent in attending to our immediate dental concerns, we keep coming to maximize our benefits. They are equipped with the latest technology, the house has complete line up of professionals, more importantly, the costing that we discussed is quite competitive. So what more to look for in a dental clinic, this office can accommodate all our needs, from basic to advance treatment.

    Admittedly, recent pandemic made me worry about going to dental facility, so I waited 'til situation calm down a little and then I went to Dr Zak along Winchester Rd. I wanted a smile makeover, this facility helped make it happen. They bleached my teeth and replaced the four front teeth with porcelain veneers. I was thrilled to see the amazing transformation! From that day on, I have referred several family members and coworkers !

    ZAK TEMECULA DENTAL GROUP TAKES ADVANTAGE OF SENIOR CITIZENS. I am OUTRAGED by their lack of dental skills, damage done to patients without any regard, and their lack of customer service to fix their mistakes. I am writing on behalf of my grandfather, the (former) patient, of this office. My grandfather has been a patient of this dentist for OVER THIRTY (3 - 0) YEARS. He has been mistreated by this office and I will not let it slide. About a year and a half ago he went in to have a bottom partial done for his dentures. EVER since that day, he has had serious issues with the partial. He has been unable to properly chew food which has resulted in serious, long lasting digestive issues, making daily life for him almost unbearable!!! Over the following 3 months after getting the partial put in, he had to return to their office at least 6 times!!!!!!!!!! My grandfather was unable to properly bite down and chew food after getting the partial. Each time, he had issues with the new partial and the dentist thought it just needed MORE adjusting. Dr A would grind on the brand new bottom partial in an attempt to adjust it and stop the issues my grandfather was having. He would come in, get it adjusted, and try it again. After 6 unsuccessful visits, my grandfather decided to see a new dentist for the first time in 30 years and get another opinion. The other dentist IMMEDIATELY saw the issue and said Dr A was completely missing the fact that one of his TOP teeth was causing the issue, NOT the partial. Dr. A should have ground down the top tooth that was too long and hitting the partial but he NEVER SAW IT!!! OVER 6 visits and he NEVER SAW IT so my grandfather was stuck with teeth that can't bite down and chew. COMPLETELY incompetent dentist. My grandfather decided to go to yet ANOTHER dentist just in case the second dentist was wrong. He goes to a THIRD dentist (who knew nothing about the second dentist) and he also IMMEDIATELY identified the issue.....the TOP tooth that was too long!!!! My grandfather went back to Temecula Dental and told them he wants his money back from the procedure they did because they completely missed this, caused him immense problems, cost him money to get 2nd and 3rd opinions, and now he needs to pay for a new TOP and BOTTOM to get the whole issue corrected and fitting properly!! Temecula Dental has REFUSED to give him his money back, offering only a partial refund. They are demanding a letter from one of the OTHER dentists confirming that what they did was wrong. ARE YOU KIDDING ME?!?! Why would another dentist need to prove to you that you did wrong??? How do you not know this yourself!? Of course, these ethical dentists will not throw another dentist under the bus that way, even if they deserve it, so alas, no letter will be written. THIS IS COMPLETELY UNACCEPTABLE AND UNETHICAL to withhold a full refund and hold his money hostage that way. How can you put a senior citizen patient through HELL and incompetence then refuse to refund for your AWFUL work and HORRENDOUS service?!?!?! This is completely and utterly BAFFLING!! My grandfather will continue to picket with a sign in front of their building every day until they agree to a FULL and complete refund of his money. NEVER EVER EVER give these quacks your business!!!!

    What dentistry doesn t have a dentist? Dr Zak My crown fell out as I was brushing my teeth and using waterpic last eve. I was in pain all night. 8 am I called Dr Zak I was promised to get my broken crown fixed by Dr Zak which was an emergency call. When I got there Dr Zak or any other dentist was not there and the office staff laughed it away "oh sorry I guess we don 't communicate that well with the people taking the call for the patients" I just think if you call in an emergency call it seems reasonable that the dentist shows up? I left in tears as my pain was throbbing and hurting so much. Thankfully Dr Lee from Temecula dental came to my rescue and I came home from her with a new crown. There are sooo many dentists in our areas so please be aware of Dr Zak s office

    As a new patient I was really impressed the staff was pleasant and professional and the x-ray technician did a wonderful job was very thorough looking forward to my next appointment so we can get a treatment plan in place and get some work done.

    I have been to this office twice in the past 2 months, all during COVID-19. First for a cleaning, impressions, and fillings then second time for night guard fitting. Both times were very professional and they had me in and out in no time. They are taking every precaution necessary to minimize the risk of transmission during these times and staying very professional as always! Highly recommend this office. I also didn't have any dental insurance when I first went in for tooth pain and their in house dental plan is pretty comparable to any other plan. It's 2 cleanings a year (every 6 months) and doctors visit plus they cover around half the cost of procedures.

    I've gone to this dentist office for years and it is honestly the best. The reception staff are always so helpful, nice and professional. Dr. A. is one of the best dentists that I've ever encountered. He is always professional, informative, and has a great attitude. Would recommend this office to anyone!

    I came into Dr.Zak Temecula Office for a second opinion after a horrible experience I had at another local dentist office. I had a molar that had been bothering me for months a month before Covid hit. My last dentist misdiagnosed me and would initially tell me my tooth was broken from the root and had to be pulled, I ended up with an abscess and when I went for my follow up, he completely forgot what he told me on my first visit and I had to pay $300 to have my gums cauterized because he said it was a gum infection. Abscess was back a week later. I continued with an abscess and pain for 3 more months. I kept insisting That my tooth was broken and He kept saying I needed to see a specialist for my gums. That's how I ended up in Dr.Zak 's office in Temecula! As I walked in I was greeted by their amazing Front office personal Jennifer Villarroel. The office it's self is a bit older not as updated as other dental offices I've been too, but they have everything. I want to congratulate every staff member I have encountered in this particular office. They have gone above and beyond to follow Covid protocol and keep us protected. They took care of my abscess and right away they let me know it was a broken tooth that had become infected! Dr.A was the one who took care of me and was the nicest and was very caring!! I loved the fact that he always made sure I was ok! I had to get a bridge and had a temporary. I went on vacation and it got loose, I email Jennifer V to let her know what had happen and she responded very quickly and send me screen shots of things I can get over the counter to hold it together while I was out of town. She also set up an appointment first thing in the morning for the day I was coming back into town! I have had such an amazing response with this office!!! I highly recommend this office. Never judge it's book by its cover! This office is awesome!!!! The staff will go above and beyond for you and they never make you feel uncomfortable. They are very understanding and helpful!! HIGHLY RECOMMENDED!!

    I am so happy I found this dental office!! During these crazy times I was a bit nervous to visit a dental office but they made me feel safe! I called to check in for my appointment for X-rays and I was about 30 mins early and they were able to take me back and get started right away. Also the office felt safe, clean and sanitized and everyone was wearing masks and proper gear. The X-ray tech was amazing, super friendly and she made the whole process quick and easy! I also scheduled an appointment with the dentist on a video call and he was AMAZING! I felt I was really taken care of and listened to and haven't had this type of experience with a dentist since I was a child. They really made me feel comfortable and I felt they were very honest about what I need done. I look forward to making this my new dental home. Thank you for your amazing customer service and making me feel comfortable and taken care of! Best dental experience I have ever had!

    I took my daughter in on Friday Sep. 24th to get 4 fillings. The check in was by texting when we got there. We waited maybe 2 minutes before we were told to come inside. When we got inside there was hand sanitizer present at the counter and we got our temperature checked right away the staff made us feel safe as they were wearing they're protective mask and face shield. They took my daughter back right away where they then had her wash her hands and rinse her mouth before they worked on her fillings. It was an amazing experience and I would recommend them to everyone!

    This place has been my savior the past months. I've had some extensive dental care done to my mouth and during that time it's been awesome considering your dealing with the mouth. The staff and the dentists themselves have been so amazing and helpful. There's been a few times I've had to just walk-in because of excruciating pain and they have always managed to get me in. They take great care in there patients and there's no other dentist I'd rather go too or recommend to someone. I will continue to stay loyal to this dental group and anyone else that is experiencing pain, swelling or any other symptoms that might be accumulated with the mouth, I highly suggest this dentist office. They won't let you down.

    I went to Dr. Zak Dental Group in Temecula to see Dr. Asatani to have permanent crowns put in. His care and expertise were both great. The bite matched perfectly the first time. I would highly recommend this office for all dental work. They are very kind, professional and upfront about what they are needing to do and how they will do it.
