The following terms and conditions (Terms) govern your enrollment and participation in a dental membership plan (Dental Membership Plan) being offered by Dr. Zak Dental Care (Dentist). These terms and your enrollment in a Dental Membership Plan constitute a binding agreement between you and Dr. Zak Dental Care (the Agreement). If you do not agree to these Terms, do not enroll in this Dental Membership Plan. THE DENTAL MEMBERSHIP PLAN IS NOT AN INSURANCE PLAN AND IS NOT INTENDED TO REPLACE DENTAL INSURANCE. THE DENTAL MEMBERSHIP PLAN IS NOT A QUALIFIED HEALTH PLAN UNDER THE AFFORDABLE CARE ACT OR A MEDICARE PRESCRIPTION DRUG PLAN. THE MEMBERSHIP FEE IS NOT AN INSURANCE PREMIUM. NEITHER THE DENTAL MEMBERSHIP PLAN, OR THE DENTIST OFFERING THE DENTAL MEMBERSHIP PLAN, ARE LICENSED INSURERS, HEALTH MAINTENANCE ORGANIZATIONS, OR OTHER UNDERWRITERS OF HEALTH CARE SERVICES.

  • You may not combine a Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion
  • It is possible that, during the course of treatment by a Dentist, he or she may discover a dental or health condition, such as periodontal disease, that may require further treatment the costs associated with any further treatment are your responsibility
  • 2. ACCOUNT SIGN-UP. To enroll in a Dental Membership Plan using the zakdental.com online platform, you must open an account (the Account). You represent that all information you provide during the Account sign-up process and at any time thereafter, for yourself and for all family members (collectively, the Account Information), will be true, accurate, complete, and current, and that you will promptly update your Account Information as necessary such that it is, at all times, true, accurate, complete, and current. The Plapfrom will use and maintain your Account Information subject to and consistent with the terms set forth in our Privacy Policy. The Agreement incorporates the terms and conditions set forth in our Privacy Policy. By accessing the zakdental.com online platform and/or participating in the Dental Membership Plan, you are consenting to have your personal data and the personal data of your family members used by us as set forth in the Privacy Policy. You may enroll your spouse under your Account. You also may enroll dependent minors in the Dental Membership Plan, provided that you are the legal parent or guardian for each individual enrolled under your Account. To create an Account, you must be 18 years or older, have a unique email address, and have a payment method registered with the zakdental.com online platform. You will be required to provide your name and email address and to create a password. To access the Account, the email address and password will be required. As the Account holder, you should not reveal the Account Information to anyone else. You are responsible for maintaining the confidentiality and security of your Account Information and for all activities that occur on or through the Account.

    3. PAYMENT TERMS AND PROCESSING. The Dentist offering the Dental Membership Plan that you choose will specify available payment plans, such as paying month-to-month or paying annually. In each case, Membership Fees will be due in advance, and your payment method on file will be billed accordingly. The zakdental.com online platform uses a third-party payment processor to collect and process all payments. In order to make payments you may be required to agree to the third-party terms and conditions and/or privacy policy. You are responsible for the timely payment of all fees. You agree that the zakdental.com online platform may process payment for all Membership Fees associated with the Dental Membership Plan that you choose using the payment method (e.g., credit card, debit card, etc.) that you have provided in your Account Information. It is your responsibility to notify your Dentist if your payment method has changed. If you do not provide a valid payment method, or if your designated payment method is determined to be inactive for any reason, your Dentist may immediately discontinue your enrollment in the Dental Membership Plan. You agree not to cause your credit or debit card company to reverse or chargeback any fees charged in accordance with this Agreement; and in the event you do so, we and/or your Dentist may terminate your enrollment in the Dental Membership Plan, and you agree to reimburse us and/or your Dentist for any costs incurred in responding to such chargeback, including, without limitation, our and your Dentists actual costs paid to the credit or debit card company, including chargeback fees, and the value of the time our and your Dentists employees spend on the matter as determined in our or your Dentists discretion in good faith. Your Dentist may increase the Membership Fee of the Dental Membership Plan from time to time. You will receive an email notification to the email address you registered on the zakdental.com online platform of any such increase in fees no less than thirty (30) days before the new rate becomes effective. If you do not agree to the increase(s), you may cancel your membership. By establishing an Account and enrolling in a Dental Membership Plan through the zakdental.com online platform, you authorize the Dentist in whose Dental Membership Plan you are enrolling to bill for the monthly Membership Fee or annual Membership Fee. If your payment method on file or billing information changes, it is your responsibility to provide updated information to us on a timely basis. Your zakdental.com online platform / Member Account dashboard will include real-time data with respect to your enrollment information, including the beginning of the current enrollment period and a history of your payments of Membership Fees.

    4. LIMITATIONS. Not all dental services are Included Services under the Dental Membership Plan. Only those specifically listed by your Dentist as Included Services are provided by your Dentist at no additional charge to you. If you require any dental services that are not Included Services, you are responsible for paying your Dentist for those Non-Included Services at the rates established by your Dentist. You may also be subject to a Dentists late payment fee and other applicable office policies. Any disputes over the fees charged by dentists must be addressed directly with the Dentist. Use of the zakdental.com online platform requires compatible devices, internet access, a unique email address, and certain software; and may be affected by the performance of these factors. You agree that meeting these requirements, which may change from time to time, is your responsibility. We do not represent or guarantee you will be able to access the zakdental.com online platform at all times.

  • Member Cancellation during the initial ninety (90) day period. * If you cancel your membership in the Dental Membership Plan during the initial ninety (90) days* following your enrollment (whether such payment is a monthly payment or an annual payment) your Dentist will provide a full refund of your paid Membership Fees. NOTWITHSTANDING THE FOREGOING, IF YOU HAVE RECEIVED ANY DENTAL SERVICES UNDER YOUR DENTAL MEMBERSHIP PLAN WITHIN THE FIRST NINETY (90) DAYS* AND CANCEL YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN AFTER RECEIVING THOSE SERVICES, YOUR DENTIST WILL BILL YOU FOR THE SERVICES RENDERED TO YOU AT YOUR DENTISTS THEN-CURRENT, NONDISCOUNTED UCR (USUAL AND CUSTOMARY RATES). PAYMENT IN FULL FOR ANY SERVICES RENDERED BY YOUR DENTIST DURING THIS PERIOD WILL BE REQUIRED PRIOR TO CANCELLATION OF YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN, AND THE ISSUANCE OF A REFUND.
  • Billing Error. In order to be considered a billing error you must provide sufficient written documentation to demonstrate the existence of a billing error and adequate proof of the amount paid due to such error.
  • Dental Membership Plan Termination. If your Dentist terminates your membership in a Dental Membership Plan without your consent or permission, or ceases offering a Dental Membership Plan, your Dentist will provide a pro rata refund to the payer on your Account.
  • 7. COMPLAINTS. All complaints about the dental services provided by the Dentist, the fees you were charged, or any other dissatisfaction with a Dentist, must be addressed directly with the Dentist. You may submit your complaint via our online help request system or by contacting our team via email.

    8. GOVERNING LAW; ARBITRATION. The Agreement shall be governed by and construed in accordance with the laws of the State of California, without reference to conflicts of those laws provisions and, as to matters affecting copyrights, trademarks and patents, by U.S. federal law. Any dispute or claim arising out of, or in connection with, this Agreement shall be finally settled by binding arbitration in Los Angeles, California, in accordance with N.C. Gen. Stat. 1-569.1 et seq. (the Uniform Arbitration Act) and the then-current rules and procedures of the American Arbitration Association by one (1) arbitrator appointed by the American Arbitration Association. The arbitrator shall apply the law of the State of California, without reference to rules of conflict of law or statutory rules of arbitration, to the merits of any dispute or claim. Judgment on the award rendered by the arbitrator may be confirmed, reduced to judgment and entered in any court of competent jurisdiction. You agree that, any provision of applicable law notwithstanding, the arbitrator shall have the authority to award the prevailing party its costs and reasonable attorneys fees. In the event that the above arbitration provision is held invalid or unenforceable, then any dispute with respect to the Agreement shall be brought and heard either in the California state courts located in Los Angeles, California, or the federal district court located in Los Angeles, California. In such event, you consent to the in personam jurisdiction and venue of such courts. You agree that service of process upon you in any such action may be made if delivered in person, by courier service, by first class mail, and shall be deemed effectively given upon receipt.

    9. CHANGES TO THESE TERMS AND CONDITIONS. Dr. Zak Dental Care reserves the sole right at any time to modify, discontinue, or delete portions of these Terms. We will notify you of any material changes to these Terms by email to the email address that you have registered on the zakdental.com online platform. It is your responsibility to check your email periodically for changes to the Terms. If you do not agree with the new Terms, you must cancel your membership and stop using the zakdental.com online platform Services. Your continued use of the zakdental.com online platform, the zakdental.com online platform Services and/or the Dental Membership Plan after any changes to these Terms constitutes your acceptance of the revised Terms and Conditions.

    I have been coming here for a number of years. The facility and people on site are nice. My issue is that trying to speak to anyone who can give you an answer is extremely frustrating. Voice calls are offshored. When I have spoken to numerous people they do t have the answers and have to get someone to call me back, which doesn't happen. I get stuck in an endless cycle of promises and no. Delivery. All I was originally asking for was Information around what was covered in a membership plan I signed up for. If you can't provide that or easily direct me to the information there is something seriously wrong. Owing to frustration I did request a refund on September 28th on a recorded call with a lady who promised that transaction was processsed while she was on the call with me and email confirmation sent. I k ow refunds can take 7-10 days but surprise surprise still no refund or email. Your customer service needs serious attention. I would also like my $300. Communicating with you should not be so painful and if the only recourse is through reviews and social media something is very wrong with your process.

    My kids are patients of Zak Dental in Valencia but we had to drive here for my son's ortho. There's been some different understanding between the x-ray technicians & the reception about some simple stuff like where my son can do mouthwash, can we wait in the lobby or not, that made us being ping-ponged. They also still required to wear mask while at this moment even we're not required to wear mask in a confined space like plane. Without a notification about mask that is not a state mandatory anymore, at least provide us mask in your lobby because at this moment we don't carry mask anymore unless being notified before the visit. This review was originally 3 stars, but i have to change to one after what happened next. My daughter and I waited outside until one lady who works there saw us waiting in the hall while going to restroom. She said "I'll get you mask" and come back inside. After a few minutes, she came empty handed and said that they didn't let her give us mask. Two pieces of masks. Ridiculous!

    I have been going to Zak Dental since February of this year, The staff and receptionists are Amazing and friendly. I started with Dr. Moya he explained everything I needed to get done In a way that I could understand. Dr.Zak Ward has been my Dr. Who has done all my cavities and took out a few teeth that were bothering not once did I feel pain. I had a worse experience with WD in simi valley they took out my front tooth and left me without one I have been so sad and not confident about my smile, but Dr. Ward and his team have been so great and I should be getting my front tooth done soon I can't wait to get my smile back Thank you to all theStaff, Maria Elena and all the helper's TEAM ZAK DENTAL I appreciate you all Thank You, Warm Regards Sarah Aguilar

    Worst dentist ever. I would give -5 stars if possible. Went to get a cavity filled, and the lady went to fill the WRONG TOOTH. I had to point out to her, that's the wrong tooth amd at first at she didn't believe me, and went to ask someone which tooth needs to be filled. Had braces, they never fully took off the cement glue and now have yellow spots on teeth, luckily another dentist scrapped it off for me. This place just makes you feel bad about yourself. And you have to wait an hour prior to your appointment time. WORST DENTIST EVER DO NOT GO NEAR THISS PLACE

    Working on the wrong tooth, they are butchers with an assembly line of patients where it often turns out like I Love Lucy and too many chocolates go by so fast, Lucy screws up and has to start hiding them. It's impossible to get past the call center to the real staff. First they tried to get us to sign a "treatment plan" so we'd be liable for whatever expensive ad ons they could run by us. Then they worked on THE WRONG TOOTH grinding the enamel down exposing nerves for no good reason. Thus turning an otherwise healthy tooth into a painful 24 hr nightmare so now I had unbearable pain in 2 teeth. while trying to tell me I wasn't in pain, it was "phantom pain". Pain in 2 teeth wouldn't be so bad, if it didn't add to the chronic back pain and pain I have from other health issues. They increased the overall pain burden of my life 10 fold. There is a special place in hell for these people.

    Don't sign anything! This is a scam operation designed to bilk the patient's insurance companies including MediCal as much as possible with unnecessary procedures, double sessions, seldom addressing the conditions that compelled you to see the dentist in the 1st place. My family have been directly lied to by the office staff which consists of a bureaucratic mess that never communicates with the call center. They won't ever give you procedures or specialties outside the scope of what the local hired dentist their little office can charge you for. You will never actually be treated by the real Dr Zak or have your issues be reviewed by the overseer or main "chair-dentist" as one office manager claimed when she lured us in for a second set of x-rays 3 weeks after they already x-rayed us. To rack up charges, they make you sign a concocted " Dental Plan" filled with pricey dream procedures that you can rarely see. They have an electronic signature device detached from any screen so you can't see what you are signing. Don't ever expect to get a referral for orthodontics, periodontics, dentures or implants. Their "referral department cannot be reached or doesn't exist. No phone number and a false email address was given us along with a series of excuses that don't add up. I'm pulling my family out of here.

    I usually don't ever write reviews. This place is hands-down the worst place in the entire world Dr. Chong (Asain man doctor) is the worst person to ever happen they are a scam and they are not good dentist please take your business elsewhere. My entire family that has been coming to them for 15 years has been scammed and ripped off including myself I came to understand them today as I got older and less naive.

    I've been coming to Dr. Zak's office for the past couple of years after having gone at least a few years neglecting my dental hygiene (so I needed to get A LOT done). Everyone has been very friendly during every one of my many visits, and that mattered to me since I was very nervous every time. If you are looking for a friendly staff who will ensure your teeth and gums look and feel their very best, give Dr. ZAK and the Simi Valley team a chance. By the way, they also offer patients affordable rates and payment options, something that I'm sure will come in handy during these tough financial times that so many people are struggling with. Everyone has been great here, especially Tony, Carmen, and Riham. Thank you all so much!
