The following terms and conditions (Terms) govern your enrollment and participation in a dental membership plan (Dental Membership Plan) being offered by Dr. Zak Dental Care (Dentist). These terms and your enrollment in a Dental Membership Plan constitute a binding agreement between you and Dr. Zak Dental Care (the Agreement). If you do not agree to these Terms, do not enroll in this Dental Membership Plan. THE DENTAL MEMBERSHIP PLAN IS NOT AN INSURANCE PLAN AND IS NOT INTENDED TO REPLACE DENTAL INSURANCE. THE DENTAL MEMBERSHIP PLAN IS NOT A QUALIFIED HEALTH PLAN UNDER THE AFFORDABLE CARE ACT OR A MEDICARE PRESCRIPTION DRUG PLAN. THE MEMBERSHIP FEE IS NOT AN INSURANCE PREMIUM. NEITHER THE DENTAL MEMBERSHIP PLAN, OR THE DENTIST OFFERING THE DENTAL MEMBERSHIP PLAN, ARE LICENSED INSURERS, HEALTH MAINTENANCE ORGANIZATIONS, OR OTHER UNDERWRITERS OF HEALTH CARE SERVICES.

  • You may not combine a Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion
  • It is possible that, during the course of treatment by a Dentist, he or she may discover a dental or health condition, such as periodontal disease, that may require further treatment the costs associated with any further treatment are your responsibility
  • 2. ACCOUNT SIGN-UP. To enroll in a Dental Membership Plan using the zakdental.com online platform, you must open an account (the Account). You represent that all information you provide during the Account sign-up process and at any time thereafter, for yourself and for all family members (collectively, the Account Information), will be true, accurate, complete, and current, and that you will promptly update your Account Information as necessary such that it is, at all times, true, accurate, complete, and current. The Plapfrom will use and maintain your Account Information subject to and consistent with the terms set forth in our Privacy Policy. The Agreement incorporates the terms and conditions set forth in our Privacy Policy. By accessing the zakdental.com online platform and/or participating in the Dental Membership Plan, you are consenting to have your personal data and the personal data of your family members used by us as set forth in the Privacy Policy. You may enroll your spouse under your Account. You also may enroll dependent minors in the Dental Membership Plan, provided that you are the legal parent or guardian for each individual enrolled under your Account. To create an Account, you must be 18 years or older, have a unique email address, and have a payment method registered with the zakdental.com online platform. You will be required to provide your name and email address and to create a password. To access the Account, the email address and password will be required. As the Account holder, you should not reveal the Account Information to anyone else. You are responsible for maintaining the confidentiality and security of your Account Information and for all activities that occur on or through the Account.

    3. PAYMENT TERMS AND PROCESSING. The Dentist offering the Dental Membership Plan that you choose will specify available payment plans, such as paying month-to-month or paying annually. In each case, Membership Fees will be due in advance, and your payment method on file will be billed accordingly. The zakdental.com online platform uses a third-party payment processor to collect and process all payments. In order to make payments you may be required to agree to the third-party terms and conditions and/or privacy policy. You are responsible for the timely payment of all fees. You agree that the zakdental.com online platform may process payment for all Membership Fees associated with the Dental Membership Plan that you choose using the payment method (e.g., credit card, debit card, etc.) that you have provided in your Account Information. It is your responsibility to notify your Dentist if your payment method has changed. If you do not provide a valid payment method, or if your designated payment method is determined to be inactive for any reason, your Dentist may immediately discontinue your enrollment in the Dental Membership Plan. You agree not to cause your credit or debit card company to reverse or chargeback any fees charged in accordance with this Agreement; and in the event you do so, we and/or your Dentist may terminate your enrollment in the Dental Membership Plan, and you agree to reimburse us and/or your Dentist for any costs incurred in responding to such chargeback, including, without limitation, our and your Dentists actual costs paid to the credit or debit card company, including chargeback fees, and the value of the time our and your Dentists employees spend on the matter as determined in our or your Dentists discretion in good faith. Your Dentist may increase the Membership Fee of the Dental Membership Plan from time to time. You will receive an email notification to the email address you registered on the zakdental.com online platform of any such increase in fees no less than thirty (30) days before the new rate becomes effective. If you do not agree to the increase(s), you may cancel your membership. By establishing an Account and enrolling in a Dental Membership Plan through the zakdental.com online platform, you authorize the Dentist in whose Dental Membership Plan you are enrolling to bill for the monthly Membership Fee or annual Membership Fee. If your payment method on file or billing information changes, it is your responsibility to provide updated information to us on a timely basis. Your zakdental.com online platform / Member Account dashboard will include real-time data with respect to your enrollment information, including the beginning of the current enrollment period and a history of your payments of Membership Fees.

    4. LIMITATIONS. Not all dental services are Included Services under the Dental Membership Plan. Only those specifically listed by your Dentist as Included Services are provided by your Dentist at no additional charge to you. If you require any dental services that are not Included Services, you are responsible for paying your Dentist for those Non-Included Services at the rates established by your Dentist. You may also be subject to a Dentists late payment fee and other applicable office policies. Any disputes over the fees charged by dentists must be addressed directly with the Dentist. Use of the zakdental.com online platform requires compatible devices, internet access, a unique email address, and certain software; and may be affected by the performance of these factors. You agree that meeting these requirements, which may change from time to time, is your responsibility. We do not represent or guarantee you will be able to access the zakdental.com online platform at all times.

  • Member Cancellation during the initial ninety (90) day period. * If you cancel your membership in the Dental Membership Plan during the initial ninety (90) days* following your enrollment (whether such payment is a monthly payment or an annual payment) your Dentist will provide a full refund of your paid Membership Fees. NOTWITHSTANDING THE FOREGOING, IF YOU HAVE RECEIVED ANY DENTAL SERVICES UNDER YOUR DENTAL MEMBERSHIP PLAN WITHIN THE FIRST NINETY (90) DAYS* AND CANCEL YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN AFTER RECEIVING THOSE SERVICES, YOUR DENTIST WILL BILL YOU FOR THE SERVICES RENDERED TO YOU AT YOUR DENTISTS THEN-CURRENT, NONDISCOUNTED UCR (USUAL AND CUSTOMARY RATES). PAYMENT IN FULL FOR ANY SERVICES RENDERED BY YOUR DENTIST DURING THIS PERIOD WILL BE REQUIRED PRIOR TO CANCELLATION OF YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN, AND THE ISSUANCE OF A REFUND.
  • Billing Error. In order to be considered a billing error you must provide sufficient written documentation to demonstrate the existence of a billing error and adequate proof of the amount paid due to such error.
  • Dental Membership Plan Termination. If your Dentist terminates your membership in a Dental Membership Plan without your consent or permission, or ceases offering a Dental Membership Plan, your Dentist will provide a pro rata refund to the payer on your Account.
  • 7. COMPLAINTS. All complaints about the dental services provided by the Dentist, the fees you were charged, or any other dissatisfaction with a Dentist, must be addressed directly with the Dentist. You may submit your complaint via our online help request system or by contacting our team via email.

    8. GOVERNING LAW; ARBITRATION. The Agreement shall be governed by and construed in accordance with the laws of the State of California, without reference to conflicts of those laws provisions and, as to matters affecting copyrights, trademarks and patents, by U.S. federal law. Any dispute or claim arising out of, or in connection with, this Agreement shall be finally settled by binding arbitration in Los Angeles, California, in accordance with N.C. Gen. Stat. 1-569.1 et seq. (the Uniform Arbitration Act) and the then-current rules and procedures of the American Arbitration Association by one (1) arbitrator appointed by the American Arbitration Association. The arbitrator shall apply the law of the State of California, without reference to rules of conflict of law or statutory rules of arbitration, to the merits of any dispute or claim. Judgment on the award rendered by the arbitrator may be confirmed, reduced to judgment and entered in any court of competent jurisdiction. You agree that, any provision of applicable law notwithstanding, the arbitrator shall have the authority to award the prevailing party its costs and reasonable attorneys fees. In the event that the above arbitration provision is held invalid or unenforceable, then any dispute with respect to the Agreement shall be brought and heard either in the California state courts located in Los Angeles, California, or the federal district court located in Los Angeles, California. In such event, you consent to the in personam jurisdiction and venue of such courts. You agree that service of process upon you in any such action may be made if delivered in person, by courier service, by first class mail, and shall be deemed effectively given upon receipt.

    9. CHANGES TO THESE TERMS AND CONDITIONS. Dr. Zak Dental Care reserves the sole right at any time to modify, discontinue, or delete portions of these Terms. We will notify you of any material changes to these Terms by email to the email address that you have registered on the zakdental.com online platform. It is your responsibility to check your email periodically for changes to the Terms. If you do not agree with the new Terms, you must cancel your membership and stop using the zakdental.com online platform Services. Your continued use of the zakdental.com online platform, the zakdental.com online platform Services and/or the Dental Membership Plan after any changes to these Terms constitutes your acceptance of the revised Terms and Conditions.

    Today had a really bad experience in Zak Dental for the X-ray exam. I think the guy helped me the exam is new employee, he was nice but had to take more than 8 times for each side teeth x-ray pictures. Then in another step to take some photos, the mirror is too large for my mouth, it is difficult to put it in. The guy was gentle, tried to put it in slowly, but his mentor(a female) was so rude, didn't care if it was painful even though I told her too hurt, her attitude was so bad which made me feel I am not a patient but just a tool? I finally decided to not to take the pics for the down teeth. It should be my choice, right? But she said she had to take...? I have been in Zak dental for several years, I like the lady for cleaning, she is so nice! But today I really had a bad experience. If the mirror size is too large, it is not to ask patient deep breath, please buy some different size mirrors

    Impossible to get a response from this dental group. They do not answer the phone and ignore email requests. It's easy to make an appointment, but you have no idea who the dentist is. I will change as soon as I can. I prefer a doctor or dentist that communicates.

    Warning... Dr Zak's dental care is the worst place you can go in my opinion. Dr Zak's built a bridge for me that actually fell apart in only two weeks. He would not repair or replace it. It never fit right. The would not refund my money. I lost over 4,000.00. Dr. Zak's sucks. Find anyone else. They should check their records since they change dentists often. Beware

    Dr. Olsen, his Lisa and Silvia were exceptional throughout the process of my new bridge. From the scheduling, x-rays, cleanings, proper fitting, shading of the teeth, scans and impressions. On down to the alignment and composition of both temporary & new composition. Literally everyone from start to finish was a pleasure & delight to work with! Thank you, thank you, thank you ALL...Dr. Olsen, Lisa and Silvia for your kindness, expertise, patience, knowledge and detailed eyes in the art of dentistry!

    Went to the office. Was told to take a seat. Suddenly hear who was first. There was no sign in sheet, take a guess who was first. Finally was called and was told they will take xrays but visit is virtual . Also that my husband will have to come back for his virtual appnt. This place is a big joke They never answer their phone and never return calls.

    Went there for cleaning and at spot found out that there is going to be copay. They should mention it over the phone. On few occasions they tried to charge me copays that other dental offices didn't with the same insurance. Even with the price that they are trying to enforce is not worth the service you are getting at the end. It looks like the business is more focused on marketing then service that they are providing to their 'dear' patients. Ask for the cost before you walk in .

    Their scheduling capabilities are subpar. Too many non-human systems that do not communicate with each other. Try rescheduling or cancelling your appointment. You will discover takes many steps and may not have the outcome you wish. **** I'll have to add to my comment from the response received below; I was to get a call yesterday from Zak office to schedule an oral surgeon. No called, no message left. I called the local office to see about scheduling, got a voice mail that mentions going on Zak website to handle inquiries. And if you have to leave a voice message to "press one" I pressed one and the phone disconnected me. There's nothing wrong with using non-human systems, but if they don't work properly or too many patients are frustrated by them, the owner should take note and fix it! At the very lease, the owner should "call in" as if is patient an try to schedule an appointment - vet your systems out on behalf of your patients.

    Super unprofessional experience. I went to get a treatment for cavities on which I wasn't informed the procedure and what to expect. Dentist ended up cutting my lips and even dropped whatever liquid on my tongue which burned; no mention of what they were doing or using on me. Until the end I ended up walking out with stained, swollen black lips because she rubbed the floss on my lips; not once did she mention how I was doing. Definitely will not be returning. Super disgusting and unprofessional service.

    Not your neighborhood dentist! The Ruffin Zak Dental office is the worst I've seen in my entire life. WOW! This office does not accept direct calls. A centralized center, somewhere, handles all appointments and communication. Below are the steps the patients need to go through. 1/ First Meeting (duration 20 minutes): X-ray, no one introduced themselves, no dentist was insight. 2/ First Dentist Meeting (2 days late): A virtual meeting with an ONLY-ONE-TIME online dentist. * This online dentist will discuss your dental plan(s) and procedures. ** A financial agent will take over the dentist consultation to discuss cost options (Yes! 4 times higher than the standard market value). The meeting ended with an assurance of a callback after she checked my HMO eligibility. 3/ Crickets Chirp (three weeks later): It appears that HMO insurance doesn't pay enough .

    Called the website phone number didn't work, dialed in through phone icon got through. The call took over 40 minutes to schedule an initial app for myself and 2 kids. Only to get email that it was booked at a different location over 2 hour drive!!! Called back and staff was so unprofessional in handling their mistake. I need a dental cleaning and they said it takes 3 appointment to even get that far for everyone. 1st visit exam, 2nd visit result and 3 visit you could start cleaning. Sounds to me that they just want an excuse to charge the insurance! Go elsewhere

    They forgot about me during the pandemic for six months stating that they lost me in the system they finally gave me a flimsy retainer that broke within a few weeks of having it I've been trying to get this resolved and no one well take responsibility for their lack keeping their appointments when I finally get in touch with someone and fixing the issue in a timely manner. Updating: Two appointments later I was told I would be called on Monday with a resolution on how this will be resolved since they will now need six months of braces to correct how badly my teeth shifted because of lack of their response to any of my emails calls or attempts to get a new retainer within a timely manner. Sue said she would get back to me, never did, after multiple texts, she's clearly pushing me off, and will not respond to any questions about how they will fix this issue

    I have a fear about dental appts due to past experiences during my childhood years. I've been seeing Dr.Olson at Zak dental and my visits with him have been the best experience ever. He cares about his patients and makes sure his patients are happy with the work he does.

    UPDATE Reduced by one star. ORIGINAL I've been going there now for a couple years. I have to say they do give a good level of care. I've gone to both the Ruffin Road and the North Park offices. The woman who has clean my teeth for the past two visits has been very genuine and informative. They do have a level of cleaning and disinfecting everything regarding covid and they haven't had any problems. I do think this is a place to be recommended and I generally think they do a good job.

    This place is an a ABSOLUTE SCAM! I had had previous treatments over the years (3-5 minute 'cleanings', $300. Crowns miraculously inflated to $1100 PLUS crowns and 'treatment', etc.) and suffered the claims of "we diagnosed this tooth less than a year ago and had you taken the treatment, this wouldn't have happened..." . True. As far as it goes: I challenged the inflated costs and what is actually covered by my insurance, and did not have it done then. Never received a satisfactory explanation as to why things weren't covered in my insurance AND why the overly inflated prices. Less than 9 months later, Now YOU MUST PREPAY -- in full! Before treatment! So as others have pointed out, if the patient receives inferior treatments THEY have to get the work re-done -- out of pocket. Any over billing (which an elderly couple were trying to get a refund from Dr. Zak's overcharging on their account while I was in the office this morning as a live example of Dr. Zak's practices) is on the patient. As others have stated from their personal experiences trying to get through for an appointment is an absolute nightmare: foreign 'call centers' with a limited script: "Oh we must schedule for X-rays and then a LATER appointment for a virtual appointment with a doctor to REVIEW your X-rays -- before we will schedule an appointment, then we can set up a treatment plan and set future appointments and coverage and payment." I explained I have a broken tooth! I need treatment now. This is a dental emergency! Foreign call center: "We have you scheduled for X-rays..." I went in this morning and the X-ray tech repeated, "...we're going to take a full set of X-rays this morning and then we'll set a future virtual appointment with a Doctor..." I asked to see the Office Manager. 10 minutes later someone showed up. I explained (and showed) the remaining 1/3 of my tooth remaining. "We'll take X-rays this morning." I asked what is Dr. Zak's office's policy for dental emergencies? "We take X-rays and prescribe pain medications and set up an appointment for later..." So much for patient well being, comfort, competent dental work and care. If you have other realistic options? Go somewhere else and get real dental care that's not inflated in prices, work that's necessary and resolves your issues, and not a scam!

    Hands-down the worst experience I've ever had with any dentist across the country. From trouble scheduling appointments, to having the dentist not know what procedures they're performing on me, to failing to provide service, and then failing to provide a refund, I cannot express enough that you should find another dentist. I literally had to tell the dentist what procedure I was there for while he kept telling me I was his last appointment of the day and he couldn't wait to get drunk. I'm not kidding. Is that the type of dentist experience you want to have?

    UPDATE- I am still waiting to be contacted by this office. This provider has prolonged my treatment by several months! What's more, is they ONLY put my top braces on and never serviced them but want to be paid $1500 and are refusing to give me back my $4,000. I cannot reach anyone by phone and have put in numerous requests for someone to contact me. Still, I have not heard anything from this office. I had to begin a new treatment plan with a new provider and have new braces put on. This office has costed me so much in money, time, and stress. They have replied to my google review and not attempted to contact me!! Give me back my money!!!!! ****Informative review for those considering this office**** Please do not waste your time. My appointments were canceled for no reason in the beginning. Communication with this office is non-existant. Calls are routed to a third party office where you are told you will receive a call back and you do not. I received orthodontic treatment from this office and braces were put on my top teeth only. A second appointment was made to put braces on my bottom teeth and was canceled by the office because the orthodontist quit the practice. My treatment has been prolonged and noone has bothered to call me to reschedule or discuss treatment plans. I cannot stress how unprofessional this office is. Upon checking in, I watched the dentist and receptionist trash talk a client at the check-in window and were very well aware I was standing there waiting to check-in. Clients are encouraged to fill out a request form on the website to receive a call back but don't hold your breath. If you do receive a call back, do not expect them to give you accurate information or be able to answer any questions. Again, calls are routed to a third party customer service who do not work in the office. The third party claims to escalate issues to resolves them but is merely another message to the office saying the customer called. The more recent person I spoke to could not pronounce the word 'Orthodontics'..... huge red flag! My appointment was 9/21 and I have been told at least 5 times that I will recieve a call back. I have been told 3 times the issue is being escalated. The office has been paid upfront for my braces. I have expressed my concerns numerous times and finally requested a refund after it was evident noone was prioritizing returning my call and proceeding my treatment. I have never witnessed such unprofessional business practice in a dental office or any healthcare office. Truthfully, I would not have this office service my 13 year old dog and she has 3 teeth. I strongly recommend steering far and clear from this place if you expect to be treated like a paying customer and your concerns be heard. I do not feel comfortable continuing treatment with them.

    Disappointing at last visit. I scheduled a routine cleaning. Showed up and the tech spent 20-30 min doing X-rays instead. She also did NOT give me a thyroid shield exposing me to radiation. She also did not ask about pregnancy which is standard practice. When I inquired about my scheduled cleaning she replied - "oh no cleaning today just X-rays. You have to go on zoom to talk to a dentist and then you come back for a cleaning. A lot of staff are not vaccinated here." Why did they not tell me they canceled my cleaning? I called and scheduled for a cleaning specifically. They also never asked for my consent for the X-rays. And not giving me a thyroid shield is just illegal it seems. On top of this at my last cleaning, the hygienist cut my gums and it has not healed well. Now food gets stuck in my gums all the time. I went on this zoom dentist meeting and the dentist said "oh that's funny. It healed weird". Weird is how he described my body part. Pretty unprofessional. I was kind of shocked and laughed it off but it was not appropriate. Then he went on to suggest they give the gum a laser treatment to make the gum heal again for $70. I have asked for an appeal. Why do I have to pay for something the dental practice did wrong? Of course, they haven't gotten back to me yet still. Really disappointed in what has become of this practice. Would not recommend them if you can help it.

    This has been one of the worst experiences with a dentist... I got a filling that was poorly done so they redid it.. and a few short months later it had cracked and I had to go to another dentist to get it redone and I had to pay out of pocket because my insurance didn't cover it due to "frequency" like if it was my fault that they did a poor job.

    Thought all I need was cleaning but they are experts, they see thru x-rays what the real score is inside my mouth, so I let them do what's rightful. So got 2 fillings, and one root canal. Happy now with my clean and healthy mouth!
