The following terms and conditions (Terms) govern your enrollment and participation in a dental membership plan (Dental Membership Plan) being offered by Dr. Zak Dental Care (Dentist). These terms and your enrollment in a Dental Membership Plan constitute a binding agreement between you and Dr. Zak Dental Care (the Agreement). If you do not agree to these Terms, do not enroll in this Dental Membership Plan. THE DENTAL MEMBERSHIP PLAN IS NOT AN INSURANCE PLAN AND IS NOT INTENDED TO REPLACE DENTAL INSURANCE. THE DENTAL MEMBERSHIP PLAN IS NOT A QUALIFIED HEALTH PLAN UNDER THE AFFORDABLE CARE ACT OR A MEDICARE PRESCRIPTION DRUG PLAN. THE MEMBERSHIP FEE IS NOT AN INSURANCE PREMIUM. NEITHER THE DENTAL MEMBERSHIP PLAN, OR THE DENTIST OFFERING THE DENTAL MEMBERSHIP PLAN, ARE LICENSED INSURERS, HEALTH MAINTENANCE ORGANIZATIONS, OR OTHER UNDERWRITERS OF HEALTH CARE SERVICES.

  • You may not combine a Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion
  • It is possible that, during the course of treatment by a Dentist, he or she may discover a dental or health condition, such as periodontal disease, that may require further treatment the costs associated with any further treatment are your responsibility
  • 2. ACCOUNT SIGN-UP. To enroll in a Dental Membership Plan using the zakdental.com online platform, you must open an account (the Account). You represent that all information you provide during the Account sign-up process and at any time thereafter, for yourself and for all family members (collectively, the Account Information), will be true, accurate, complete, and current, and that you will promptly update your Account Information as necessary such that it is, at all times, true, accurate, complete, and current. The Plapfrom will use and maintain your Account Information subject to and consistent with the terms set forth in our Privacy Policy. The Agreement incorporates the terms and conditions set forth in our Privacy Policy. By accessing the zakdental.com online platform and/or participating in the Dental Membership Plan, you are consenting to have your personal data and the personal data of your family members used by us as set forth in the Privacy Policy. You may enroll your spouse under your Account. You also may enroll dependent minors in the Dental Membership Plan, provided that you are the legal parent or guardian for each individual enrolled under your Account. To create an Account, you must be 18 years or older, have a unique email address, and have a payment method registered with the zakdental.com online platform. You will be required to provide your name and email address and to create a password. To access the Account, the email address and password will be required. As the Account holder, you should not reveal the Account Information to anyone else. You are responsible for maintaining the confidentiality and security of your Account Information and for all activities that occur on or through the Account.

    3. PAYMENT TERMS AND PROCESSING. The Dentist offering the Dental Membership Plan that you choose will specify available payment plans, such as paying month-to-month or paying annually. In each case, Membership Fees will be due in advance, and your payment method on file will be billed accordingly. The zakdental.com online platform uses a third-party payment processor to collect and process all payments. In order to make payments you may be required to agree to the third-party terms and conditions and/or privacy policy. You are responsible for the timely payment of all fees. You agree that the zakdental.com online platform may process payment for all Membership Fees associated with the Dental Membership Plan that you choose using the payment method (e.g., credit card, debit card, etc.) that you have provided in your Account Information. It is your responsibility to notify your Dentist if your payment method has changed. If you do not provide a valid payment method, or if your designated payment method is determined to be inactive for any reason, your Dentist may immediately discontinue your enrollment in the Dental Membership Plan. You agree not to cause your credit or debit card company to reverse or chargeback any fees charged in accordance with this Agreement; and in the event you do so, we and/or your Dentist may terminate your enrollment in the Dental Membership Plan, and you agree to reimburse us and/or your Dentist for any costs incurred in responding to such chargeback, including, without limitation, our and your Dentists actual costs paid to the credit or debit card company, including chargeback fees, and the value of the time our and your Dentists employees spend on the matter as determined in our or your Dentists discretion in good faith. Your Dentist may increase the Membership Fee of the Dental Membership Plan from time to time. You will receive an email notification to the email address you registered on the zakdental.com online platform of any such increase in fees no less than thirty (30) days before the new rate becomes effective. If you do not agree to the increase(s), you may cancel your membership. By establishing an Account and enrolling in a Dental Membership Plan through the zakdental.com online platform, you authorize the Dentist in whose Dental Membership Plan you are enrolling to bill for the monthly Membership Fee or annual Membership Fee. If your payment method on file or billing information changes, it is your responsibility to provide updated information to us on a timely basis. Your zakdental.com online platform / Member Account dashboard will include real-time data with respect to your enrollment information, including the beginning of the current enrollment period and a history of your payments of Membership Fees.

    4. LIMITATIONS. Not all dental services are Included Services under the Dental Membership Plan. Only those specifically listed by your Dentist as Included Services are provided by your Dentist at no additional charge to you. If you require any dental services that are not Included Services, you are responsible for paying your Dentist for those Non-Included Services at the rates established by your Dentist. You may also be subject to a Dentists late payment fee and other applicable office policies. Any disputes over the fees charged by dentists must be addressed directly with the Dentist. Use of the zakdental.com online platform requires compatible devices, internet access, a unique email address, and certain software; and may be affected by the performance of these factors. You agree that meeting these requirements, which may change from time to time, is your responsibility. We do not represent or guarantee you will be able to access the zakdental.com online platform at all times.

  • Member Cancellation during the initial ninety (90) day period. * If you cancel your membership in the Dental Membership Plan during the initial ninety (90) days* following your enrollment (whether such payment is a monthly payment or an annual payment) your Dentist will provide a full refund of your paid Membership Fees. NOTWITHSTANDING THE FOREGOING, IF YOU HAVE RECEIVED ANY DENTAL SERVICES UNDER YOUR DENTAL MEMBERSHIP PLAN WITHIN THE FIRST NINETY (90) DAYS* AND CANCEL YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN AFTER RECEIVING THOSE SERVICES, YOUR DENTIST WILL BILL YOU FOR THE SERVICES RENDERED TO YOU AT YOUR DENTISTS THEN-CURRENT, NONDISCOUNTED UCR (USUAL AND CUSTOMARY RATES). PAYMENT IN FULL FOR ANY SERVICES RENDERED BY YOUR DENTIST DURING THIS PERIOD WILL BE REQUIRED PRIOR TO CANCELLATION OF YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN, AND THE ISSUANCE OF A REFUND.
  • Billing Error. In order to be considered a billing error you must provide sufficient written documentation to demonstrate the existence of a billing error and adequate proof of the amount paid due to such error.
  • Dental Membership Plan Termination. If your Dentist terminates your membership in a Dental Membership Plan without your consent or permission, or ceases offering a Dental Membership Plan, your Dentist will provide a pro rata refund to the payer on your Account.
  • 7. COMPLAINTS. All complaints about the dental services provided by the Dentist, the fees you were charged, or any other dissatisfaction with a Dentist, must be addressed directly with the Dentist. You may submit your complaint via our online help request system or by contacting our team via email.

    8. GOVERNING LAW; ARBITRATION. The Agreement shall be governed by and construed in accordance with the laws of the State of California, without reference to conflicts of those laws provisions and, as to matters affecting copyrights, trademarks and patents, by U.S. federal law. Any dispute or claim arising out of, or in connection with, this Agreement shall be finally settled by binding arbitration in Los Angeles, California, in accordance with N.C. Gen. Stat. 1-569.1 et seq. (the Uniform Arbitration Act) and the then-current rules and procedures of the American Arbitration Association by one (1) arbitrator appointed by the American Arbitration Association. The arbitrator shall apply the law of the State of California, without reference to rules of conflict of law or statutory rules of arbitration, to the merits of any dispute or claim. Judgment on the award rendered by the arbitrator may be confirmed, reduced to judgment and entered in any court of competent jurisdiction. You agree that, any provision of applicable law notwithstanding, the arbitrator shall have the authority to award the prevailing party its costs and reasonable attorneys fees. In the event that the above arbitration provision is held invalid or unenforceable, then any dispute with respect to the Agreement shall be brought and heard either in the California state courts located in Los Angeles, California, or the federal district court located in Los Angeles, California. In such event, you consent to the in personam jurisdiction and venue of such courts. You agree that service of process upon you in any such action may be made if delivered in person, by courier service, by first class mail, and shall be deemed effectively given upon receipt.

    9. CHANGES TO THESE TERMS AND CONDITIONS. Dr. Zak Dental Care reserves the sole right at any time to modify, discontinue, or delete portions of these Terms. We will notify you of any material changes to these Terms by email to the email address that you have registered on the zakdental.com online platform. It is your responsibility to check your email periodically for changes to the Terms. If you do not agree with the new Terms, you must cancel your membership and stop using the zakdental.com online platform Services. Your continued use of the zakdental.com online platform, the zakdental.com online platform Services and/or the Dental Membership Plan after any changes to these Terms constitutes your acceptance of the revised Terms and Conditions.

    Their service is so complicated and annoying! Every time I need an appointment, even something as simple as a regular preventive check up and cleaning, i can't just book it and do it. No. First, you book an appointment in the office when they do pictures and x rays. Then, you have to have a virtual appointment with the actual doctor where they tell you what needs to be done. And only then you can book the actual appointment and get things done. How annoying! Why can't the doctor just LOOK into my mouth when I'm there and do the cleaning?? I'm not going to take time off from work 3 times for one stupid cleaning!!!

    A nightmare. Avoid like a plague. Cleaning was painful (it isn't at other dentists). Had fillings that didn't fit right, have been trying to schedule a follow-up to correct. They do not respond to requests on their website. You cannot call the local office directly, and the call center will refuse to connect you, so you have to play a telephone game (it's been a month). I should have seen the red flag when they charged me for full year in advance.

    I feel bad for the nice receptionists that have to deal with the changing policies and ridiculousness that is zak dental. I was originally with the dentist in this office who retired and company was taken over by zak. Now I get Nickle and dimed, upsold, and have spent 3 years of unhappiness at zak dental hoping it will improve. Their work is decent but they are super corporate and money hungry. Since covid I now have to come in for x-rays, go home for a virtual appt with doctor and then come back for cleaning. 3 events! When it was just a 2hr experience before. They also charged me extra money for an extra office visit. So unhappy and over it. Time to find a smaller dentist that cares for their patients, not just collecting their fees and making money.     Worst dental experience I have ever had in 50 years of going to dentists. I have been fortunate to have dental insurance my entire life. At the age of 50 I have only had 3 cavities. Before going to Zac Dental I had never had a bad dental experience in my life. During 2020 I experienced tooth pain for the first time in my life, that summer I discovered my excellent dentist had retired and that Zac Dental had taken over their practice. After the first exam Zac Dental told me I needed 4 crowns. This was extremely surprising to me as I had been told my teeth looked great less than a year before. I should have found a second opinion right then. BUT I was in pain, I agreed to one crown on the tooth that was hurting me, because it was 2020 lock down and I really didn't want to look around for another dentist at that point. The dentist who put the crown in was the most rude and unprofessional health care practitioner I have ever encountered (and I am over 50!). He yelled at me when tears were rolling down my face because he was so rough. I went to a different dentist a month later and he told me he could see no reason for 3 more crowns. I had one small cavity easily filled, no need for a crown, other two teeth were totally fine. The new dentist also told me that the crown Zac Dental had put in was one of the worst he has ever seen. The crown Zac Dental put in fell out a little over a year later when I was flossing my teeth. My new dentist showed me the fallen out crown and explained to me how poorly the crown had been made by Zac Dental and why it had not fit properly. It makes me so sad that this kind of poor quality work can go on. I am resistant to writing bad reviews, but after the crown fell out and I learned how bad a job they had done, and shared my story with a friend she told me I really should post a review so that people know what is going on there. It makes me really sad that other people have had to endure this kind of physical and emotional dental abuse.

    This place is really shady. They tried to overcharge me and when I spoke to my insurance and found out that I was being overcharged, they refused to speak to my insurance. The staff was nice but I would not recommend this place to anybody!

    This has been a nightmare!! They took x-rays and then high jacked my insurance. I was never seen by a dentist or even spoken to one. Now I have to go through waiting a month because of how they operate. I returned to my old dentist. She is an hour away. I was looking for someone near me. I am a senior citizen and have felt abused and taken advantage of. I wish that I had read these reviews first. I would never have gone to this sketchy place. This was 4 months ago. I would advice anyone that needs dental care to STAY AWAY from this place!! If you go there you will regret it. I would hope that they would change seeing so many disgruntled patients. They don't seem to care unless it involves MONEY. You can not resolve anything because their central call center is busy and you have to spend hours on hold. ZAK does not value our time. STAY AWAY or you will regret it. They do not deserve even one star.

    0/0 would recommend. Not only was I not seen by a dentist but was told I needed multiple root canals, and a $300 special cleaning that wasn't covered by insurance. My new dentist verified this was a lie

    Went there for cleaning and at spot found out that there is going to be copay. They should mention it over the phone. On few occasions they tried to charge me copays that other dental offices didn't with the same insurance. Even with the price that they are trying to enforce is not worth the service you are getting at the end. It looks like the business is more focused on marketing then service that they are providing to their 'dear' patients. Ask for the cost before you walk in .

    Signs of a practice that have gotten too big, too fast. Nothing but paperwork, getting shuffled between people, having to do a video visit with a dentist after only having x-rays and photos taken by a tech, being upsold on dental work no other provider has ever recommended. 0/0 recommend. Just switched to a new provider after yet another terrible experience trying to get a simple answer.

    Dentists there will over diagnose and treat and drive up your bill. Had to get a root canal and paid 2000 upfront. Endodontist DR GAVIN cancelled the morning of with no reason. I got it done with a different office and Zak dental has not given me back my money and it's been 3 weeks. Very unresponsive, unreliable, and untrustworthy.

    If you like to get your time wasted sign right up. Called the office only to be told to fill out the online intake form. Then I received a text and was promoted to call to make an appointment. Was given an appointment with the wrong location. At every step of the way, I made it clear what kind of appointment I needed- a simple cleaning. When I went to the appointment, I was told that I would be able to get a cleaning on the 3rd appointment due to COVID protocol. This is insanity and stupidity in full force. If you want to protect patients and staff, minimize their office visits, every time someone steps back into the office, the risk level increases. A 30-45 min cleaning with this dental office will cost you approximately 4-6 hours of your time when you take in the new patient protocol, travel time and of course taking time off of work/ obligations to make the appointment. I don't know about their dental services but their protocol is inefficient and doesn't prioritize client care.

    The legacy about caring for dental health is what I could pass to my kids and that is more highlighted with the effectiveness of dentists and their full round of service in Zak Dental in Ohio St. They make the right partner in the dental care of each family member that my husband and I truly appreciate!

    Miss treating & bad handling with payments. Quick to take your money so make sure invoices are correct. Dont pay over the phone they are greedy. pay at the location. one lady told me 100$ deposit then another told me half down just to set an appointment. they forgot to send my claim to insurance which caused me to reschedule my fillings. & to think ive been going here since the 80s. Dont go to this location sucks if you have to.

    Our family trusted Dr. Zak for 3generations, all services - restorations, extractions, cosmetics, all ages for years. Have known good people who worked here that moved to their other office, and promoted. I'm happy for these staff who truly love their profession and respect patients as well. Anyway, as per my own experience, the 4piece of high grade porcelain that Dr Zak's dentist put on me, was pretty awesome, flawless! It made me respectable in front of my audience. The cost is reasonable also compared to other offices. Cheers!!

    Two things for me - clean, white even teeth and no harm in my pocket. and they are both accommodated here at Zak Dental. I'm a student and no big money to spend, so I thank the management, for affordable treatment! Of course, I'm happy with my new lovable SMILE!

    Their procedure completely changed. Now you cannot book a simple cleaning unless you come 1) full mouth X Ray 2) zoom call another day (!) with a dentist 3) you can finally get the cleaning. It's easier to reach them recently. I appreciate that they are HONEST and not pushing for unnecessary services thanks to the opinion from 2 different dentists. One pushed for a crown, fillings on slightly chipped teeth. The other dentist said that none of that is needed. I strongly recommend their female dentist. My only problem is that they overcharge. Dr Zak too over Dr Riedler. I needed a crown because they said my tooth had too many filings and was too weak. I was quoted $747 when I paid $181 for the same kind of crown with Riedler. When I confronted them, they mumbled that my insurance will not cover as it is for esthetic. They had already shaved my tooth. If they had said it was esthetic I would have never agreed to a crown. Dr Riedler also messed up one of my crown. 2 dentists at Dr Zak said that as it was recent they would redo it for free. However on billing day I got an email saying that they will charge me as it is an "account receivable" which is not true as I had paid the service in full to Dr Riedler. When confronted, they changed the story and allegedly found XRay from the time the crown was done showing it was done correctly. It is not true as Riedler never did XRay after installing a crown. Overall beside billing, they are clean, polite, have brand new material and renovated after taking over Riedler.
