The following terms and conditions (Terms) govern your enrollment and participation in a dental membership plan (Dental Membership Plan) being offered by Dr. Zak Dental Care (Dentist). These terms and your enrollment in a Dental Membership Plan constitute a binding agreement between you and Dr. Zak Dental Care (the Agreement). If you do not agree to these Terms, do not enroll in this Dental Membership Plan. THE DENTAL MEMBERSHIP PLAN IS NOT AN INSURANCE PLAN AND IS NOT INTENDED TO REPLACE DENTAL INSURANCE. THE DENTAL MEMBERSHIP PLAN IS NOT A QUALIFIED HEALTH PLAN UNDER THE AFFORDABLE CARE ACT OR A MEDICARE PRESCRIPTION DRUG PLAN. THE MEMBERSHIP FEE IS NOT AN INSURANCE PREMIUM. NEITHER THE DENTAL MEMBERSHIP PLAN, OR THE DENTIST OFFERING THE DENTAL MEMBERSHIP PLAN, ARE LICENSED INSURERS, HEALTH MAINTENANCE ORGANIZATIONS, OR OTHER UNDERWRITERS OF HEALTH CARE SERVICES.

  • You may not combine a Dental Membership Plan with any other discounts, discount plans, dental insurance (including Medicare, Medicaid, other forms of government insurance or assistance, and/or any private insurance plan or policy), or in-office promotion
  • It is possible that, during the course of treatment by a Dentist, he or she may discover a dental or health condition, such as periodontal disease, that may require further treatment the costs associated with any further treatment are your responsibility
  • 2. ACCOUNT SIGN-UP. To enroll in a Dental Membership Plan using the zakdental.com online platform, you must open an account (the Account). You represent that all information you provide during the Account sign-up process and at any time thereafter, for yourself and for all family members (collectively, the Account Information), will be true, accurate, complete, and current, and that you will promptly update your Account Information as necessary such that it is, at all times, true, accurate, complete, and current. The Plapfrom will use and maintain your Account Information subject to and consistent with the terms set forth in our Privacy Policy. The Agreement incorporates the terms and conditions set forth in our Privacy Policy. By accessing the zakdental.com online platform and/or participating in the Dental Membership Plan, you are consenting to have your personal data and the personal data of your family members used by us as set forth in the Privacy Policy. You may enroll your spouse under your Account. You also may enroll dependent minors in the Dental Membership Plan, provided that you are the legal parent or guardian for each individual enrolled under your Account. To create an Account, you must be 18 years or older, have a unique email address, and have a payment method registered with the zakdental.com online platform. You will be required to provide your name and email address and to create a password. To access the Account, the email address and password will be required. As the Account holder, you should not reveal the Account Information to anyone else. You are responsible for maintaining the confidentiality and security of your Account Information and for all activities that occur on or through the Account.

    3. PAYMENT TERMS AND PROCESSING. The Dentist offering the Dental Membership Plan that you choose will specify available payment plans, such as paying month-to-month or paying annually. In each case, Membership Fees will be due in advance, and your payment method on file will be billed accordingly. The zakdental.com online platform uses a third-party payment processor to collect and process all payments. In order to make payments you may be required to agree to the third-party terms and conditions and/or privacy policy. You are responsible for the timely payment of all fees. You agree that the zakdental.com online platform may process payment for all Membership Fees associated with the Dental Membership Plan that you choose using the payment method (e.g., credit card, debit card, etc.) that you have provided in your Account Information. It is your responsibility to notify your Dentist if your payment method has changed. If you do not provide a valid payment method, or if your designated payment method is determined to be inactive for any reason, your Dentist may immediately discontinue your enrollment in the Dental Membership Plan. You agree not to cause your credit or debit card company to reverse or chargeback any fees charged in accordance with this Agreement; and in the event you do so, we and/or your Dentist may terminate your enrollment in the Dental Membership Plan, and you agree to reimburse us and/or your Dentist for any costs incurred in responding to such chargeback, including, without limitation, our and your Dentists actual costs paid to the credit or debit card company, including chargeback fees, and the value of the time our and your Dentists employees spend on the matter as determined in our or your Dentists discretion in good faith. Your Dentist may increase the Membership Fee of the Dental Membership Plan from time to time. You will receive an email notification to the email address you registered on the zakdental.com online platform of any such increase in fees no less than thirty (30) days before the new rate becomes effective. If you do not agree to the increase(s), you may cancel your membership. By establishing an Account and enrolling in a Dental Membership Plan through the zakdental.com online platform, you authorize the Dentist in whose Dental Membership Plan you are enrolling to bill for the monthly Membership Fee or annual Membership Fee. If your payment method on file or billing information changes, it is your responsibility to provide updated information to us on a timely basis. Your zakdental.com online platform / Member Account dashboard will include real-time data with respect to your enrollment information, including the beginning of the current enrollment period and a history of your payments of Membership Fees.

    4. LIMITATIONS. Not all dental services are Included Services under the Dental Membership Plan. Only those specifically listed by your Dentist as Included Services are provided by your Dentist at no additional charge to you. If you require any dental services that are not Included Services, you are responsible for paying your Dentist for those Non-Included Services at the rates established by your Dentist. You may also be subject to a Dentists late payment fee and other applicable office policies. Any disputes over the fees charged by dentists must be addressed directly with the Dentist. Use of the zakdental.com online platform requires compatible devices, internet access, a unique email address, and certain software; and may be affected by the performance of these factors. You agree that meeting these requirements, which may change from time to time, is your responsibility. We do not represent or guarantee you will be able to access the zakdental.com online platform at all times.

  • Member Cancellation during the initial ninety (90) day period. * If you cancel your membership in the Dental Membership Plan during the initial ninety (90) days* following your enrollment (whether such payment is a monthly payment or an annual payment) your Dentist will provide a full refund of your paid Membership Fees. NOTWITHSTANDING THE FOREGOING, IF YOU HAVE RECEIVED ANY DENTAL SERVICES UNDER YOUR DENTAL MEMBERSHIP PLAN WITHIN THE FIRST NINETY (90) DAYS* AND CANCEL YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN AFTER RECEIVING THOSE SERVICES, YOUR DENTIST WILL BILL YOU FOR THE SERVICES RENDERED TO YOU AT YOUR DENTISTS THEN-CURRENT, NONDISCOUNTED UCR (USUAL AND CUSTOMARY RATES). PAYMENT IN FULL FOR ANY SERVICES RENDERED BY YOUR DENTIST DURING THIS PERIOD WILL BE REQUIRED PRIOR TO CANCELLATION OF YOUR MEMBERSHIP IN THE DENTAL MEMBERSHIP PLAN, AND THE ISSUANCE OF A REFUND.
  • Billing Error. In order to be considered a billing error you must provide sufficient written documentation to demonstrate the existence of a billing error and adequate proof of the amount paid due to such error.
  • Dental Membership Plan Termination. If your Dentist terminates your membership in a Dental Membership Plan without your consent or permission, or ceases offering a Dental Membership Plan, your Dentist will provide a pro rata refund to the payer on your Account.
  • 7. COMPLAINTS. All complaints about the dental services provided by the Dentist, the fees you were charged, or any other dissatisfaction with a Dentist, must be addressed directly with the Dentist. You may submit your complaint via our online help request system or by contacting our team via email.

    8. GOVERNING LAW; ARBITRATION. The Agreement shall be governed by and construed in accordance with the laws of the State of California, without reference to conflicts of those laws provisions and, as to matters affecting copyrights, trademarks and patents, by U.S. federal law. Any dispute or claim arising out of, or in connection with, this Agreement shall be finally settled by binding arbitration in Los Angeles, California, in accordance with N.C. Gen. Stat. 1-569.1 et seq. (the Uniform Arbitration Act) and the then-current rules and procedures of the American Arbitration Association by one (1) arbitrator appointed by the American Arbitration Association. The arbitrator shall apply the law of the State of California, without reference to rules of conflict of law or statutory rules of arbitration, to the merits of any dispute or claim. Judgment on the award rendered by the arbitrator may be confirmed, reduced to judgment and entered in any court of competent jurisdiction. You agree that, any provision of applicable law notwithstanding, the arbitrator shall have the authority to award the prevailing party its costs and reasonable attorneys fees. In the event that the above arbitration provision is held invalid or unenforceable, then any dispute with respect to the Agreement shall be brought and heard either in the California state courts located in Los Angeles, California, or the federal district court located in Los Angeles, California. In such event, you consent to the in personam jurisdiction and venue of such courts. You agree that service of process upon you in any such action may be made if delivered in person, by courier service, by first class mail, and shall be deemed effectively given upon receipt.

    9. CHANGES TO THESE TERMS AND CONDITIONS. Dr. Zak Dental Care reserves the sole right at any time to modify, discontinue, or delete portions of these Terms. We will notify you of any material changes to these Terms by email to the email address that you have registered on the zakdental.com online platform. It is your responsibility to check your email periodically for changes to the Terms. If you do not agree with the new Terms, you must cancel your membership and stop using the zakdental.com online platform Services. Your continued use of the zakdental.com online platform, the zakdental.com online platform Services and/or the Dental Membership Plan after any changes to these Terms constitutes your acceptance of the revised Terms and Conditions.

    Scammers! Run! Take your time - read some of the negative reviews - not their obviously hired bot/reviewers. Zak Dental office is clean in appearance but as dirty as they can be presenting themselves to be caring for their patients. I started my journey with Zak Dental March 2022. Dentist advised I needed over $11,000 of dental work over a Zoom call. I never had a "hands on" meeting with the dentist. I was required to pay in advance before I could get on the schedule for the work. I'm in pain, I'm positive I have an issue than needs to be resolved quickly. I accepted the policy of prepayment. They have an unorganized texting system to schedule appointments and you have to wait for them to contact you. My one appointment that ZD scheduled - ZD canceled on the day of the appointment. They promptly charged me $50 for "missed appointment" on their accounting ledger. I have text from them proving ZD cancelled. Their rep would get back to me once the specialist schedule was available. (There is also sign posted at the office states they charge $25 to make a future appointment. ) Months go buy, zero scheduling from ZD. Covid issues, specialist schedule unknown... I'm just waiting to get relief. Receptionist at the office cannot make an appointment, refuses to give contact information for a manager and will not do anything. Receptionist stated they were short staffed... My response for for 7 months?? I have made 4 physical trips to the office, numerous emails, texts and phone calls to resolve these issues. I have promises but zero results that ZD will get back to me. I have an email stating my records are ready for pickup. Receptionist informed me it is not there. NONE of the emails have their names, signature line or phone number to confirm who I was communicating with. I have been in pain for 7 MONTHS. My issue has now escalated. They still have my money so I have been unable to go elsewhere. How many other people have they placed in this position? How much can this office make holding clients money? What kind of Dentist would leave a patient in severe pain for 7 months, prepaid - ?? I also see evidence from the insurance company ZD is incompetent on their billing practices. ZD did not give the insurance company the specific information required. Is this why my case is delayed?? It is now September 2022, and NO appointments, NO results and NO refund. ZD has pretty makeup, expensive equipment, social media dollars and salespeople - but the results clearly showcase they are scam artists preying on people in pain. I have informed Zak Dental I am filing a formal complaint with the Dental Board of CA.

    As a new patient, I had to go through three appointments before I could get a simple cleaning. I met with Dr. Zak virtually. He tried to sell me all kinds of dental work I didn't need or want. When I asked questions, he cut me off and said he had to go to his next appointment. To add insult to injury, they billed an insurance plan I no longer use. I'm unsure how they got that information because I never provided it. They are unprofessional and unethical. They make it very difficult to contact them regarding billing issues. Update: After writing this review, they finally contacted me. However, I'm not getting anywhere. They claim they billed the correct insurance because their database can pull up all insurances. As I explained to them on multiple occasions, I don't use that insurance because I stopped paying for it. The insurance company called them to retract their claims, but they couldn't get through because their phone system threw them in a loop. It's been a nightmare trying to deal with these people. I will stay away from here.

    Syeda, Chris, Homa, Dianela and Dr. Robin were all amazing. The office is super clean. The check in process was super easy and fast. Dr. Robin made me feel so comfortable and explained to me everything that would be done today. I can not express how grateful I'm to have found this office. Plus I got a free goodie bag

    This place is a assembly line. I went in to get my teeth cleaned and instead I had 15 minutes of x-rays. Then two days later I'm supposed to have an appointment virtual to discuss what to do. When did Dental become so difficult. It did when I became dentist for profit. My regular dentist goes in with a pic and tells me what I need. Then I get my teeth cleaned and if necessary I get x-rays. There was a woman at the front counter trying to get her tooth put back in before her vacation. She really did not understand what was going on because of all the difficulty with individual appointments and sales people in other locations. I will find a new dentist that does it the old fashion way in one visit. Two visits to get your teeth clean come on.Everybody was very nice and it was very clean but this is not how dentistry is done. I won't be coming back. Read some of the other reviews they seem very fake.

    Worse worse worse dental office. Do not waste your time or money. These people are a mess and don't know what they are doing both dental and office related. If there we zero stars that is what I would put down. They cancel appointments the day of dont notify of payments outstanding if any and will not provide service yet they make you miss work or school to go waste gas

    Dont go here. I got my wisdom teeth pulled out here and the process was rushed and painful. As they were extracting my teeth i tried to make convo because i was nervous and all they could talk about was how busy they were that day. They took out my teeth so roughly i didnt stop profusely bleeding for another 6 hours. Lastly they didnt even make sure i had my prescription for my pain medicine. I even asked about it and they said it would be emailed but i never received it. I dont think ill ever return here again horrible service and it left me suffering for hours with no pain meds. Had to take advils just to try and stop the pain.

    Trusted for 7years by the whole family. As our kids grow, so as our confidence in their dental care. We are well cared every time we visit. Always assisted with high quality of service. Have recommended this dental office to all friends that I knew in and our of the area.

    Do not use this dentist. On our first visit as a new client, they mixed me up with another patient! I have never seen anything like this. A new patient looking for an evaluation and teeth cleaning was mixed up for a completely different existing client who was apparently changing their Invisalign retainer. How do you confuse a new patient who is looking for a basic cleaning with an existing patient who is in for retainers?!? We were at 2 different appointment times! And this wasn't some quick "oops, we mixed you up for someone else". It was a ridiculous affair where the staff tried to tell me I didn't know who I was or why I was at their offices... Even after being checked in (albeit 30+ min after my appointment time when I'd been waiting in their waiting room) and after showing them my drivers license with my name and picture on it, they still tried to tell me I was someone else and I was there for another reason than I stated. Imagine that! One staff member even walked into the appointment room with the other persons retainer and shook them in my face insultingly calling out that "this is why you're here". I had to deal with 5 different staff workers, none of whom were the actual dentist I signed up for an appointment with. After another 15 minutes of trying to convince them that I was who I said I was, I was fed up and said that I wanted to leave and told them to not charge my insurance. That was another gut-wrenching process of speaking with even more employees who kept trying to coerce me back to the X-Ray room. All in all, this office was a completely disorganized, unprofessional (to put it politely), and frankly scary place to be in. My recommendation is to stay far far away.

    Tried and tested for 5 consecutive years for me and my kids, 11 and 9yrs of age. Both of my kiddos have nice set of teeth after the braces, while I got my root canal therapy and some fillings, minor and mid. Family friendly.

    This place is highly disorganized and very impersonal.They seem to be focused solely on making money. There is no one dentist you deal with. Every appointment is done with you through video chat. Every phone call seems to come from a call center overseas. Every inquiry you make has to be done through their online portal and they make you pay upfront for procedures and cancel your appointments last minute and reschedule only to cancel again. and not to mention all of the 5 star reviews on yelp, google, facebook are so vaguely positive they are definitely fake reviews by bots.

    For a great experience at Zak Dental located along Kanan Road, it is just right to share a 5star feedback on our visit 2weeks ago. Dad and I were treated with respect and kindness. Our dentists, an Ortho and an Endodontist are both sensitive to our needs, very patient and explained in details the necessary treatment, without pushing anything. The cost that we agreed upon was competitive. Two days after, the manager called us up for the upcoming appointments so we won't miss, and check on us how we are doing after the first visit. They are highly recommended especially for the elderly, like my dad. Thanks.

    My Ortho visit was a blast last Tuesday 1pm! I'm close to perfect teeth. Will be counting few more visits (3max) and I'm ready to take away my braces. Thanks to my doctor's exceptional skills and for being meticulous,. He is indeed has a great sense of professionalism. He wants the best for my dental health, my TMJ has gone.

    They utilize a team based approach to provide dental care and they stand behind the quality of the care they give to our family, whose been with them for many years, from my parents down to our children. Dr. Koppel and his team never let us down especially during the hard times. The office is blessed with good, skilled and caring professionals/staff. Busy all the time, this office was able to adhere with the government/health rules and so able to cater us these trying times. The wait is understandable for us, as long as the place gives us the safest, high quality of service, we will remain loyal patients.

    I am a mother seeking dental care for my entire family and the multi-specialty practice that Dr Zak offers for more comprehensive care is my reason of staying with them for almost a decade now. Having oral surgeon, periodontist, endo, and general practitioner in one place, saves our substantial time and effort, mind also the record management for each member of my family. I'm truly thankful how this dental care cope with us all through the many years, accommodating our changing needs.
