Dr. Mike as his patients call him, received a D.M.D. degree from the University of Lewisville, Kentucky in 1976. He then served as a dental officer in the U.S Navy Dental Corp for two years in San Diego California at the Navy Dental Center where he revived further training.
In 1978 he moved to Utah an opened a private practice in Cottonwood Heights. He taught as a clinical instructor at the University of Utah Medical Center in the graduate dental residency program for over a decade and ran a clinic for the department of health in Salt Lake City.
He has been practicing dentistry in Cottonwood Heights and South Jordan with an Emphasis on interceptive orthodontics, treatment of obstructive sleep apnea, sleep related breathing disorders in children, dental implantology and cosmetic dentistry. He is an accredited provider for The Healthy Start program to treat sleep disordered breathing in children and develop healthy normal dental arches.
He is a member of the American Academy of Dental Sleep Medicine, The American Academy of Implant Dentistry, The Utah Dental Association and is an instructor for the Academy of Minimally Invasive Implantology. Dr Van Leeuwen enjoys fishing for all types of fish, amateur radio (ham radio) and playing his musical keyboards.

I have extremely sensitive teeth, going to the dentist in the past has been traumatizing and i'd rather go gnaw on an electric fence then go to the dentist! DR Adam and his whole staff changed that for me. Like truly cared about my comfort and anxiety around getting my teeth worked on, and were extremely gentle with me. They went to extra measures to accommodate my crazy schedule, they treated my case like it was their most important one, and always recommend what is in my best interests. Dr Adam designed my porcelain veneers like an engineer designing my dream McLaren P1 supercar , putting the steering and camber dialed in to where I want it. Dr Adam will fine tune your grill and make it look like a million dollars! And if you are a sweet little old lady he will pull your car around! If you want a dentist that is not satisfied until you are, especially when it comes to cosmetic dentistry, or if you just want to see if he will be the valet that day, go see DR Adam!

Van Leeuwen Dental provides everything you could ask for in dental care. I came to them because my previous dentist, whom I had seen for almost 15 years, told me that I would have to have all of my bottom teeth removed but that I would have to pay a couple of thousand dollars upfront to have that done. I had cavities underneath teeth on each end of a bridge. Dr Van Leeuwen offered to try to save me from losing my bottom teeth and needing dentures to replace them. Without going into detail, he did a procedure which probably few would try. I felt absolutely no pain during or after the procedure. My teeth are saved and I didn't realize how much the cavities were causing the pain until that pain went away. The service is especially remarkable because Van Leeuwen Dental accepts Medicare patients. The entire staff are kind, professional caring people. As I said, there is everything you could hope for in dental care.

Both Dr. Adam and Dr. Michael Van Leeuwen are great! They treat their patients with professionalism and with an added bit of office humor that put you at ease. I had my very first fillings done and Dr Adam was maticulous in his work and even asked me to be picky, because he cared what I the patient's needs we're! Who can ask for better Dentists? Not to mention that the office manager, the office staff, and Dental Hygienist, and Dental Assistants we're friendly and wonderful as well! The final cost was a bit more than expected, but I definitely recommend them to anyone. Top notch!!

Today was my first visit to my new dentist. Dr. Adam and his staff were wonderful, friendly and kind. They were very thorough in their exam and xrays, and explained everything very clearly. They are very up front about their pricing and treatment plans for any work they are recommending. Fantastic experience, highly recommend them!

Van Leeueen Dental staff, all if them, were friendly and helpful. They explained all procedures and fees very well, and were careful to give options. The care was timely and professional. They made me feel comfortable and relaxed by their demeanor. I've used lots of dentists, but I'll never leave Van Lleeuwen Dental.

Professional and passionate about his work! He could have given me the veneers as is, and it would have looked fine. But he insisted on perfection, called his technician who drove over right away, and in minutes they were corrected. He could have given me temporaries, and had me come back for a second visit but he took the extra time because he's proud of his work and cares deeply about his patient's satisfaction. Went in today to get them checked and he was very thorough and attentive to my needs/opinions. Finally, a dentist I can stay with for good! I'll be sending everyone here.

I have been a patient with Adam VanLeween and his father for several years. I have never had such knowledgeable and wise care, with an outstanding support team. They have helped with everything from tooth removal, crowns, implant. They are caring & sensitive. Their INTERNAL DISCOUNT INSURANCE PLAN IS WONDERFUL. They created it for us patients who want to take part in our own tooth wellness program. Their cleaning team is very talented and give me good advice on how to stay out of their office. Adam & Mike make a good team and they share advice and help one another. They are both VERY BRIGHT and helpful. I referred my friend Scott, who had a very difficult bridge problem with a mixture of other medical problems. Scott gives Adam two thumbs up. Russ Marriott.
