The merger of Lancaster Health Center & Welsh Mountain Health Centers was built on the foundation of unity. Our two Federally Qualified Health Centers believed that together, we would stand stronger for our communities by uniting our missions, roots, and services so that we can provide accessible, affordable, and equitable healthcare.

on Strawberry Street as a part-time, volunteer-run medical center, seeing those who needed care and support. As they grew, they were able to add paid staff to their team. By 1980, they began providing dental care in addition to medical care. Also in 1980, they moved to an office on Rockland Street, merged with Lancaster Neighborhood Health Center and SACA Health Center and renamed the newly fashioned organization 'SouthEast Lancaster Health Services.' In 2018, the organization transitioned to the name 'Lancaster Health Center' to better reflect their growing services and locations in the community.

through the leadership of then-mayor Janice Stork, the City of Lancaster gifted SouthEast Lancaster Health Services with land at 625 South Duke Street. By 1996, they had raised enough money to build a brand new health center on the donated land. The building originally included 14 medical exam rooms and 6 dental operatories and a new partnership with HACCs Nursing Program to enhance patient education. In 2011, a major renovation of the Duke Street health center took place expanding capacity from 6 to 13 dental operatories, including a Pediatric Dental area tailored to providing comfortable and pleasant dental care for children.

Lancaster Health Center opened a health center in the Bright Side Opportunities Center on the Southwestern side of Lancaster City in November, 2006. The health center includes 6 medical exam rooms. There was a time when their Bright Side center was a hub for families who were newcomers to our community.

Lancaster Health Center purchased one of Thomas Edisons original Power Plants in downtown Lancaster. Over the next two years, Lancaster Health Center undertook a successful Capital Campaign, Project Health Equality and Access for Lancaster (HEAL) to renovate the historic site into a progressive health center with 24 medical exam rooms. The Water Street center was opened to the public in the summer of 2010.

In 2016, Lancaster Health Center partnered with Reynolds Middle School to open a school-based health center inside of the middle school providing primary care for Reynolds students, their families, and school faculty.

Lancaster Health Center expanded their services to meet the growing healthcare needs of our community by adding a 5th health center on New Holland Avenue. As their second largest building, this site added 18 exam rooms, totaling 64 medical exam rooms throughout 5 sites.

Welsh Mountain Medical Center was established as a project of CAP, the Christian Medical Society of Lancaster County, the Visiting Nurse Association & the Office of Economic Opportunity in the former Mt. Airy one-room schoolhouse.

Welsh Mountain Medical Center moved to a new building, incorporated as a non-profit organization, introduced their first board of directors, brought their first full-time physician on board, and expanded services to include dental care.

The health centers name changed to 'Welsh Mountain Medical & Dental Center' to better represent their recent addition of dental care. In addition, they introduced their Horse & Buggy Program, which provided immunizations, well-child screenings, lead poisoning prevention, and health and parenting education to Plain Clothes communities each year.

By the 1990s, Welsh Mountain Medical & Dental Center had grown from an old schoolhouse to a modern, 4,400 square foot health center serving over 4,000 medical and dental patients a year. Along with an expanded facility, they also expanded their mission to provide comprehensive, quality healthcare for everyone.

Though Welsh Mountain Medical & Dental Center had changed drastically from their humble beginnings, they remained focused on their mission of providing quality, comprehensive healthcare to all patients throughout the 2000s.

In early 2016, under the guidance of then CEO Jackie Concepcion, the health center changed their name to 'Welsh Mountain Health Centers' to unify the brand and bring clarity to their mission and vision, which had projected growth on the horizon.

In the summer months of 2018, Welsh Mountain Health Centers began construction on a new, state-of-the-art facility on Church Street in Lebanon. The new location replaced an existing health center already in Lebanon. The new location was more accessible to their patients and community.

    I want to share my experience in the dental office of lebanon it is my first time and I have to say it was pleasant, from the receptionist Isara was very kind and attentive at the window, then inside was Jenna she is great she is very passionate about her work very respectful and friendly very professional and at all times made me feel cared for excellent service 100/10, to finish off Dr. Nixon the best dentist who has cared for me in my entire life, from her professionalism and at the same time his sympathy made me feel as if She has been my dentist all my life!!! Thank you for being so different, my new dental team are excellent!

    If there was a negative (-1) star option, I would've started my scale there and see how many negative stars I could rate Union Community Care. I delayed my day to go in because I was required to be seen for a prescription refill - so I did just that, urine screen included. Fast forward 30 days - I call for prescription refill & I am told the exact same thing. Yet, when I was in there physically previously, I was told every 90 days I would need a physical appt. So I ask if I can speak to my practitioner - he's on vacation for 2 wks. So I ask if I can come in right now - nothing available. I was advised to call at 8am the following day and ask for a same day appointment as if I don't have to be somewhere tomorrow (Wednesday) at 8am? When I ask why I wasn't informed of this when I was in there physically, at the front desk w practitioner, all of a sudden nobody can answer that question and I simply receive the "this is our protocol" verbage. By the way - this is only my 2nd time trying to get a prescription refill. Simply sharing my experience. Update: I replied to Union Community Care's reply at 9:12am informing that I had a 2 hr window to discuss today. As of 1pm I have heard nothing.

    This place should get No Stars You Call in and nobody answers and unfortunately this is the only place that accepts my Daughters Insurance I rather not take my Daughter here after an incident that occurred when I have taken my Daughter there before

    It was a nice experience at Union Community care. The front desk receptionist Kaylani accommodated me for an appointment based on my urgent need. Dr. Sandeep & Dr. Amal provided a well treatment plan. Overall, it is a great team and charged me an affordable price.

    I SERIOUSLY HAD ENOUGH OF THIS PLACE. MAKES ME WASTE MY DANG TIME WITH DUMB RECEPTIONIST THAT ARE NOT CONSIDERATE AT ALL. VERY QUICK TO FOLLOW RULES WHENEVER THEY CHOOSE TO AND WHEN THEY DON'T; IS BECAUSE THEY WANT TO MAKE LIFE IMPOSSIBLE.

    This place is a joke. Every scheduled appointment gets cancelled, and the kids have to wait another two months just to be cancelled while we are on our way there. DON'T WASTE YOUR TIME!!!!

    You really need to open your scheduling more than a month in advance. You can't schedule a patient's next appointment at the end of a current appointment, you tell them to call back in the beginning of the month to schedule, and when you do call back you're all booked for that month and are told to call back again in a month. No thanks. You can't just keep pushing off dental issues until things get worse. And I like my dentist here, but the scheduling service sucks and it's a shame.

    Got my dentures made here, Dr. Nixon is amazing! Fit and finish was better than I had experienced prior. Friendly office staff. You can tell they really care about their patients. Never had an issue getting appointments in a timely manner.

    Called in to get my daughter seen set up and appointment told them I was at work and my mom would be taking my daughter who is 14 in for a bad tooth the receptionist said that this was fine only for my mom to drive 30 minutes one way to be told they can't see my daughter because they don't have constant. I spoke with the receptionist once my daughter was trying to be seen told them that she may be seen and they still wouldn't see her even though they make children go back by themselves anyway

    This dental office only provides poor service, I called for an appointment because I have an infected tooth and they denied me because I missed a single appointment, they do not care about my pain because they never gave me the antibiotic prescription and if it were not for my primary doctor I would still have my infected tooth, I do not recommend this place, they have no professional ethics or humanity.

    Ever since covid getting an appt here is next to impossible. I have a better chance of getting Into my PCP and I have to stalk call them for 3 hours just to get through. They used to be ok now I might as just let my teeth fall out.

    Theres a reason why they have only 2 stars. I'd say that's too many stars for them. Absolutely horrible place. A butcher, as my husband calls it. All they do is tear out teeth. Their work is no good at all. And to top it all off, they make you put on suffocation devices on. NO THANKS! I like my oxygen! Do yourself a favor and find a better dentist!
