Chair Patricia Perlee isthe retired Executive Director of Joint Neighborhood Project.Patty is the President of the Board of the Chautauqua Home Rehabilitation and Improvement Corporation.She also is active with the Zonta Club of Jamestown and First Presbyterian Church. She became affiliated with TRC a number of years ago through her position with Joint Neighborhood Project, as she worked with many people who received medical care through The Resource Centers Primary Care Clinic.Patty has served on TRCs Board since 2013.

Vice Chair Richard Erickson, CPA,is the Director of Accounting for Chautauqua Institution and the Athenaeum Hotel. Professionally, Dick is a member of the American Institute of Certified Public Accountants and the New York State Society of CPAs. He has served on the advisory committee of the Southwest Chapter of the American Red Cross. He has served on TRCs Board since 2015.

Secretary Dawn Columbare, DNP, is a nursing education consultant who retired from Jamestown Community College after a career in which she served as the Director of Nursing and as a Nursing Professor. Dawn is a member of Sigma Theta Tau National Honor Society of Nursing and the National League for Nursing, and she serves as a peer reviewer for the Accreditation Commission for Education in Nursing Peer Reviewer. Active in the community, she serves as the Board Vice President and Finance Committee Chair for Creche and is a longtime member of Zion Covenant Church, where she serves on the Leadership Team and the Missions Committee. Dawn serves on the Community Service and Program committees of Zonta International, and she sits on the Grants and Scholarship committees for the Chautauqua Region Community Foundation. She was elected to TRCs Board in 2017.

Treasurer Todd Jacobson, M.D., is the Medical Director for Occupational Health at UPMC Chautauqua. Todd is a member of the American College of Occupational and Environmental Medicine and the New York College of Occupational and Environmental Medicine. Locally, he is a past Board member of the Boys and Girls Club of Jamestown and the Jamestown Safety Council, and he is a member of the Jamestown Municipal Band. Todd has been a member of TRCs Board since 2003, serving as Chair from 2012 to 2015. He has a daughter who receives supports from The Resource Center.

Timothy Adam has worked at SKF for 30 years. He is the President of Affinity One Federal Credit Union and a Trustee at Kidder United Methodist Church. Tim coaches Special Olympics golf, swimming and floor hockey, and he is a founding member of the Lakewood Area Buddy Walk to support people with Down syndrome. He serves on the planning committee for TRCs monthly dances at the Dunham Avenue building. He has a daughter who receives supports from The Resource Center. Tim was elected to TRCs Board in 2020.

Douglas Anderson has been involved with The Resource Center for more than 40 years. He started out working at our Jamestown manufacturing center, and over the years he also worked on our Environmental Services cleaning crew. Even though hes now retired, Doug has taken a role working with the team that interviews people who have applied for positions as Direct Support Professionals at The Resource Center. Additionally, Doug has lived in TRC residences for about four decades.He serves on our Human Rights Committee and is an active member of the Aktion Club, a community service group for people with developmental disabilities. Doug was elected to TRCs Board in 2020.

Marie Carrubba, Esq., is the Executive Director of the Southwestern Independent Living Center. Long an active community volunteer, Marie currently serves on the Jamestown City Council. She first served on TRCs Board from 1989 to 2000 and was Chair from 1997 to 2000. She was reelected to the Board in 2010. She has been involved with TRC since 1983.

R. Michael Goldman, Esq., is an attorney practicing in Jamestown, NY. He is a member of the American Bar Association, the New York State Bar Association and the Jamestown Bar Association. Active in the community, Mike is involved with Lutheran, Meals on Wheels and the Chautauqua Region Community Foundations Promise Fund. Mike has been a member of TRCs Board of Directors since 2003, serving as Chair from 2016 to 2019. He also has served as Board Secretary and Vice Chair.

Lisa Goodell is the former Executive Director of the Chautauqua Blind Association. She is Co-Chair of the Public Service Division of the United Way of Southern Chautauqua County, Secretary of the Jamestown Noon Rotary and Secretary of the Rotary Club of Jamestowns Community Service Fund. She also serves on the Finance Committee of Court Appointed Special Advocates in Chautauqua County. She was elected to TRCs Board in 2019.

Donna Haenggi is a retired secretary and receptionist, and for many years she volunteered with Special Olympics. She has been volunteering with The Resource Center since the early 1990s. Her late husband, Bill, was also a longtime TRC volunteer and Board member, and they both served as TRC delegates to The Arc New Yorks annual convention several times. We presented them with our Lifetime Achievement Award in 2018. They have a son who receives supports from TRC, and all three have helped to organize our monthly dances. Donna served on TRCs Board from 2013 to 2019, and after a one-year absence she was elected to the Board again in 2020.

Rebecca Hamlet Kapple is a former TRC employee who is a Senior Account Talent Manager with Kelly Services in Dunkirk.Rebecca is an owner/grower for the National Grape Association; is a member of the Concord Spellbinders Toastmasters Club; and serves on the Workforce Council of the Cassadaga Job Corps Center. She is the Vice President of the United Way of Northern Chautauqua Countys Board of Trustees and chairs its Nominating Committee, and she serves on the Grape Associations Nominating Committee.In addition. she serves on the planning committee for the Laurel Memorial Run & Walk. She was elected to TRCs Board in 2018.

Rich Kimball is a former TRC employee who is a retired School-to-Work Transition Specialist with Jamestown Public Schools. Rich is a member of Wounded Warriors, the American Bible Society and Special Olympics. His community service activities include supporting Zion Community Church and UCAN City Mission. Rich has a relative who receives supports from The Resource Center. He was appointed to TRCs Board in 2019.

Wayne Ormsby is a retired official with the New York State Office for People With Developmental Disabilities. In that role, over the years Wayne provided The Resource Center with technical assistanceand capital funding to develop residences and day programming. He also is a New York State municipal building code enforcement official. Waynes community activities include serving on the SUNY Fredonia Friends of Rockefeller Arts Center. He is a patron of the 1891 Fredonia Opera House and has volunteered with the United Way of Northern Chautauqua County, the Dunkirk Lighthouse and the billy mead Run for ALS Research. He has served onTRCs Boardsince 2016.

Marlene Sirianno, Esq., is Court Attorney Referee for Chautauqua and Erie County Supreme Courts. Marlenes other community service efforts have included serving on the Chautauqua County Youth Hockey Association Board for several years in the past. She became involved with The Resource Center once her son began receiving respite supports from TRC. Marlene served on our Board from 2012 to 2019, and after a two-year absence she was elected to the Board again in 2021.

Shoemakers remarks came at the end of a four-day review of The Resource Center conducted by representatives of The Council on Quality and Leadership (CQL), a Maryland-based organization dedicated to defining, measuring and improving quality of life for people receiving supports from human-service organizations. CQL works with organizations across the United States and in other countries.

CQL accreditation is coveted among human-service organizations, as it is difficult to achieve. The accreditation process involves a months-long self assessment that organizations must undergo in preparation for a visit by CQL officials, and many agencies bow out during the self-assessment process.

CQL offers three levels of accreditation, and The Resource Center had sought accreditation in the area of quality assurance. For this initial level of accreditation, CQL examined and assessed TRC in three areas: Basic Assurances, Shared Values and Personal Outcome Measures.

Those 10 factors are composed of 46 indicators on which organizations are scrutinized by CQL, which looks to determine whether there are systems and practices in place to ensure that the indicators are being met. We never expect that all 46 will be there, said Shoemaker, explaining that with organizations seeking to achieve CQL certification for the first time, its not unusual to see 12 to 20 indicators that are not fully in place. He said that TRC only had five indicators that are not yet in place.

The second component in the quality assurance certification process is in the area of Shared Values. For this, Shoemaker said he and his colleagues want to ensure that you share the values that CQL has, and see that these values are accepted by everyone in the organization. He said that having just one employee possessing a different set of values in the way service recipients should be treated can be extremely disruptive.

Taken together, Basic Assurances and Shared Values support the final piece of the quality assurance puzzle Personal Outcome Measures. Personal Outcome Measures help to ensure that services and supports provided by an organization are truly person-centered and are having results that are desired by the person. This information is gained through an interview with individuals receiving supports to determine the importance of various outcomes in each persons life, and how successful each person has been in achieving those outcomes. Knowledge gained during a Personal Outcome Measures interview can be used to develop or revise each individuals person-centered plan; information learned through many interviews can influence an organizations strategic plan.

If you lose a filling, forget it! It takes months to get in! They cancel people out of appointments because of lack of staff. If you do book an appointment, good luck getting in because there are only elderly retired dentists that work there and currently no hygienist. They need more staff to ensure that patients get correct timely treatment.

He is rude and at times seems careless.I used to respect this place until my fillings were WAYY TO LARGE 3 times caused unnecessary pain and suffering! Has said unwelcoming comments to me! Like I over stayed my welcome" ! Just wrong

I went in today to have an extraction. Everyone was very nice. I can't complain about that at all. My issue is all the damage done to my gums around my tooth and my check. It looks as if when he grabbed the tooth he just grabbed gums and all and pulled im missing chunk of my cheek and am having 100xs more pain from my cheek than the actual tooth. I've had many teeth pulled before and never had this much damage afterwards. I had my tooth pulled at 11am and here it is 7 hours later and still can't get the bleeding to stop. I understand it's not easy pulling teeth but I've literally never had as much blood or pain come with it. I'm definitely not happy with all the damage and the 1/4 inch cut on both sides of my gums. I'll pay out of pocket next time if this is the type of work fidelis/Medicaid pays for.

Out of the 33 years that I've been in and out of doctors offices this one gave me my first warm fuzzies! Right to the point told me with no B.S. that this is what needs done. Made sure I understood. Business settled I had a wonderful conversation with the staff and doctor. He informed me that he absolutely loves his job and I could see how genuine he was. Wonderful experience. My wife, 4 kids and myself have found our new lifelong dental facility. Thank you!
