Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Love Dr Kuo and his team. So nice and caring. He explained my treatment and each treatment options in great detail. I immediately felt that I am in good hand with my dental work. I give dr kuo 5 starts and two thumbs up.

I have been Dr Kuo's patient for years. I started with having some sensitivities on my tooth. I went for an exam as new patient. The receptionist was so warm and welcoming. The staff are so professional and caring. Dr Kuo is so thorough and conservative. He explained all his findings with me and make me feel comfortable and in good hand. I highly recommend dr kuo to everyone. I'm so happy that he is my dentist.

Went 10 years ago and pulled the wrong tooth. I should have sued but was young and stupid.
