Dr. Urbanek was born and raised in Cleveland, Ohio. He obtained an undergraduate degree (BS), dental school training (DDS), and a four-year residency in oral and maxillofacial surgery (MS) from The Ohio State University located in Columbus, Ohio. During the freshmen year of his undergraduate degree, he received the freshman scholar award, and in dental school won the senior award for outstanding achievement in anesthesiology. Dr. Urbanek trained under Dr. Larry Peterson during his surgery residency, who was one of the great leaders in the specialty at the time.
During a career in the Air Force, Dr. Urbanek was Chief of Oral and Maxillofacial Surgery at the United States Air Force Academy and Sheppard Air Force Base. He placed the first titanium dental implant at the Air Force Academy in 1994. While still in the Air Force, in 1999 he began moonlighting in Wichita Falls. His services and skills grew immensely. In 2005, he opened his own private practice with emphasis on dental implants.
In the last 25+ years, Dr. Urbanek has placed over 6,000 dental implants. Technology has improved dramatically over this time. Todays implant procedures are much more successful, more predictable, require fewer surgical steps, and are less traumatic.
Dr. Urbanek and his wife and college sweetheart, Terri, live part of the time in Wichita Falls and part of the time in Southlake with their two dogs. They have one son, Russ, who is married to Chevon, and two grandchildren, Savannah and Skylar.
When hes not practicing dentistry, Dr. Urbanek enjoys travel and outdoor activities. Hes involved with City Hope Church, Womans Faith Refuge, Faith Mission, the Pregnancy Help Center, Wichita Falls Area Food Bank, and Hospice of Wichita Falls.

Wildly professional on all levels. From the receptionist to the clinic was seamless. The office is exquisite and represents the type of dental care Dr. Green and Dr. Urbanek provide. I have been getting TMJ treatments here for almost almost 2 months now and feel significantly better. The staff here is incredibly friendly and always does there best to make you feel comfortable. If you trying to find a place to get treatments this is place for you!

The staff was friendly and welcoming. I was taken to a procedure room that was very comfortable. I was seen promptly and was through very quickly. Dr. Urbanek was very encouraging.

I got my wisdom teeth removed by Dr. Urbanek a week ago & have nothing but great things to say about the experience and staff. My recovery was not bad at all, and I went on vacation the next day and barely had to take any pain medication. I highly recommend going to Dr. Urbanek for wisdom teeth removal!

Excellent dental care and overall health services. Dr. Green and his team work with all systems to insure your smile is in alignment with all of your other bodily structures. VERY IMPORTANT! Our mouths are the window of our being and where disease starts. For that reason attention to detail of all oral structures are the most important place to start. A 10 star review for this outstanding office with state of the art equipment and conscientious caring Doctors!

My son had a tooth extraction completed by Star Image recently and I can't say enough positive things about the staff and the experience. Although it was daunting to think of my seven year old going under for the procedure, they did everything to address our concerns and ensure it went as smoothly as possible. I highly recommend them for any dental procedures you or your family may need.
