Our goal is to provide a medical home that focuses on high quality primary and preventive care for the whole family and to ensure that all Patients, including those who are medically underserved, have continuous access to high quality community-based health care services including a primary care home.

In 1973, Siskiyou Community Health Center (SCHC) began as the Takilma Peoples Clinic in the Illinois Valley. After a medical crisis in the community, volunteer residents of the Illinois Valley turned an old farmhouse into a community health center. Dr. Jim Shames came from Multnomah County Health Department to train local volunteers. His plan was to only stay for a few months, but he ended up moving to the valley and served as SCHC’s Medical Director until 2003

In 1987, the Longwood Fire forced them to evacuate and move to a new building in Cave Junction. They began expanding and a few years later the Takilma Peoples Clinic became known as Siskiyou Community Health Center. In the 1990’s, the clinic became a Federally Qualified Health u2028 Center (FQHC), offering services to anyone, regardless of their financial condition

In 1997, SCHC merged with Our Valley Clinic and Gateway Dental Clinic in Grants Pass building up these services to FQHC level. Then, in 2001, community volunteers and the board of directors embarked on a building program at the north end of Cave Junction to replace its outgrown medical facility. In early 2007, this new 10,000 square foot medical facility opened its doors to patients. It included a state-of-the-art lab and radiology facility operated by Asante Health System. In 2014, this facility doubled in size, expanding the medical clinic and behavioral health department, bringing the Outreach department onsite, and adding a pharmacy and dental clinic

In 2010, SCHC added a pharmacy and a laboratory inside the Grants Pass medical clinic. However, the demand for their services increased and it was time to expand again in order to accommodate the growing needs of the community. In January of 2012, SCHC moved to the first LEED-certified green building in Grants Pass. Located at 1701 NW Hawthorne Avenue, SCHC offers primary medical care for the entire family, a pharmacy, behavioral health services, laboratory services, Outreach services, and a Walk-In Clinic. To increase the service area for families in need, an Outreach location was established in Medford in November of 2015. In the spring of 2016, SCHC moved their Grants Pass dental clinic on-site with their primary care facility and added radiology services

Siskiyou Community Health Center is committed to the health and well-being of the community and will continue to care for our patients during the COVID-19 outbreak. It is our intention to avoid sending patients to the Emergency Room whenever possible so they can care for the most critical patients. In order to maintain the highest level of safety for our patients, staff, and health care providers, weve made some changes in protocol and have implemented the use of telemedicine visits when appropriate. For all patients seen in the office be assured we have a rigorous cleaning process of the entire clinic to prevent the spread of infection.

  • We are working hard to keep communication open with all patients whenever possible please answer your phone if you see the call is fromSiskiyou Community HealthCenter so we may speak with you directly and not have to leave a message.
  • Our pharmacy is open. Prescriptions can be delivered to your car if you are not feeling well. To request this service, please call ahead at (541) 472-4747 if you are in Grants Pass and (541) 592-2010 if you are in Cave Junction.
  • We have moved quickly to implement the use of telemedicine. Patients will receive the same quality medical care they come to expect from the comfort of their home through a live video chat on their tablet, smartphone or computer.

    After July 17, 2020, Siskiyou Community Health Center will no longer serve as the Josephine County COVID-19 testing site. Limited COVID-19 testing is available to SCHC patients only. Tests cannot be given without a lab order from a medical professional.

    About 3 years ago I had a cleaning scheduled. I always get nervous as I have sensitive teeth and gums. Kathy was my hygienist. She made me so comfortable and put me at ease. She's gentle and does a wonderful job! I see her twice a year and she never changes. Very sweet and kind. I hope she never leaves!!

    Came in with extreme tooth pain after I got a filling done, but I'm unable to take ibuprofen anymore as it makes me nauseous from overuse as I've been dealing with tooth pain for a long time now.. Siskiyou dental told me they don't prescribe pain meds, but did give me amoxicillin. Unfortunately, it did nothing, and I had to go to urgent care, I got much better care there for my pain. I'll go back to Siskiyou, as I do really like the dental hygienist, she's wonderful.

    I have an extreme difficulty going to dentists. They refered me to an excellent dentist in Medford that puts you to sleep. Thank you as I wasn't able to go through the cleaning process.

    In the years I've been with this dental group I've been shuffled around from one dentist to the next, one hygienist to the next, as apparently they have a large turnover of service providers, had a crown broken by one of their dentists, and now have been charged and forced to pay for a service I did not authorize nor receive. Generally poor customer service.

    Dr. Swanson is the worst dr around. She's rude and has zero respect for her patients. She hates her job. DO NOT GO HERE YOU WILL REGRET IT!

    What is the point of taking patients when you cannot take care of them? Booked months and months out, never getting call backs, dropping appointments, miscommunication with staff. Absolutely DONE with this clinic. Zero ACTUAL care given here. They will shuffle you in like cattle for those evaluations but then book you 6 months out for treatment, refer you out and never follow up. Joke of a facility and all you get told is "there is nothing we can do about it" when THEY make a mistake. Switching dental group ASAP. Good job getting those healthcare dollars ya'll!

    The Associate who takes appointments was Very Rude, Condescending & Crass. When she called to confirm my appointment she informed me because my insurance refused to pay for another Xray and that I needed to go out of my way just a few days shy of my appointment and aquire copies of the xrays or I wouldn't have an appointment. I work full time I don't know how they expect me to obtain such copies in two days with my busy schedule so my appointment was canceled. Who knew they were so Crass. They would deny future clients service because of xrays. You don't need an xray to clean & only clean teeth. Their just money hungry and want that insurance money. Very Rudy & unprofessional.

    I have lived in southern OREGON for 22 years and sadly, I've needed the services of many doctors, specialists, therapists, imaging and yes, dentists. By and large there have been very few I can say anything good about, but Siskiyou Dental is definitely the exception!! A tooth abscess, swelling and pain severe enough to bring me to my knees, Siskiyou Dental made a time slot to get me in to see a dentist the same day I called and every step there were courteous, compassionate, qualified people working with me. Dr Anderson is the best dentist I've had in more than 22 years, thank you, Sir! The staff from reception to assistants to scheduling... perfection as far as I'm concerned, thank you all so much!!

    The staff are nice and courteous. The wait time for your appointment is long and you can expect to wait longer past your appointment time for them to even take you back. It's more waiting then too. It seems they are always running behind. I feel the practice was better when they were smaller business and in the old building. I feel the business has expanded so much that they can't keep up.

    Booked out 6 months in advance for a cleaning. Broke a tooth squeezed me in for consultation no guarantee how long to schedule to fix it. I will be looking for another clinic.
