BOTOX Cosmetic (onabotulinumtoxinA) is indicated in adult patients for the temporary improvement in the appearance of: - moderate to severe glabellar lines associated with corrugator and/or procerus muscle activity - moderate to severe lateral canthal lines associated with orbicularis oculi activity - moderate to severe forehead lines associated with frontalis activity

Postmarketing reports indicate that the effects of BOTOX Cosmetic and all botulinum toxin products may spread from the area of injection to produce symptoms consistent with botulinum toxin effects. These may include asthenia, generalized muscle weakness, diplopia, ptosis, dysphagia, dysphonia, dysarthria, urinary incontinence and breathing difficulties. These symptoms have been reported hours to weeks after injection. Swallowing and breathing difficulties can be life threatening and there have been reports of death. The risk of symptoms is probably greatest in children treated for spasticity but symptoms can also occur in adults treated for spasticity and other conditions, particularly in those patients who have an underlying condition that would predispose them to these symptoms. In unapproved uses, including spasticity in children, and in approved indications, cases of spread of effect have been reported at doses comparable to those used to treat cervical dystonia and spasticity and at lower doses.

CONTRAINDICATIONS BOTOX Cosmetic is contraindicated in the presence of infection at the proposed injection site(s) and in individuals with known hypersensitivity to any botulinum toxin preparation or to any of the components in the formulation.

The potency Units of BOTOX Cosmetic are specific to the preparation and assay method utilized. They are not interchangeable with other preparations of botulinum toxin products and, therefore, units of biological activity of BOTOX Cosmetic cannot be compared to nor converted into units of any other botulinum toxin products assessed with any other specific assay method.

Spread of Toxin Effect Please refer to Boxed Warning for Distant Spread of Toxin Effect. No definitive serious adverse event reports of distant spread of toxin effect associated with dermatologic use of BOTOX Cosmetic at the labeled dose of 20 Units (for glabellar lines), 24 Units (for lateral canthal lines), 40 Units (for forehead lines with glabellar lines), 44 Units (for simultaneous treatment of lateral canthal lines and glabellar lines), and 64 Units (for simultaneous treatment of lateral canthal lines, glabellar lines, and forehead lines) have beenreported.

Serious Adverse Reactions With Unapproved Use Serious adverse reactions, including excessive weakness, dysphagia, and aspiration pneumonia, with some adverse reactions associated with fatal outcomes, have been reported in patients who received BOTOX injections for unapproved uses. In these cases, the adverse reactions were not necessarily related to distant spread of toxin, but may have resulted from the administration of BOTOX to the site of injection and/or adjacent structures. In several of the cases, patients had pre-existing dysphagia or other significant disabilities. There is insufficient information to identify factors associated with an increased risk for adverse reactions associated with the unapproved uses of BOTOX. The safety and effectiveness of BOTOX for unapproved uses have not been established.

Hypersensitivity Reactions Serious and/or immediate hypersensitivity reactions have been reported. These reactions include anaphylaxis, serum sickness, urticaria, soft-tissue edema, and dyspnea. If such reactions occur, further injection of BOTOX Cosmetic should be discontinued and appropriate medical therapy immediately instituted. One fatal case of anaphylaxis has been reported in which lidocaine was used as the diluent and, consequently, the causal agent cannot be reliably determined.

Cardiovascular System There have been reports following administration of BOTOX of adverse events involving the cardiovascular system, including arrhythmia and myocardial infarction, some with fatal outcomes. Some of these patients had risk factors including pre-existing cardiovascular disease. Use caution when administering to patients with pre-existing cardiovascular disease.

Increased Risk of Clinically Significant Effects with Pre-existing Neuromuscular Disorders Individuals with peripheral motor neuropathic diseases, amyotrophic lateral sclerosis, or neuromuscular junction disorders (eg, myasthenia gravis or Lambert-Eaton syndrome) should be monitored when given botulinum toxin. Patients with neuromuscular disorders may be at increased risk of clinically significant effects including generalized muscle weakness, diplopia, ptosis, dysphonia, dysarthria, severe dysphagia, and respiratory compromise from onabotulinumtoxinA (see Warnings and Precautions).

Dysphagia and Breathing Difficulties Treatment with BOTOX and other botulinum toxin products can result in swallowing or breathing difficulties. Patients with pre-existing swallowing or breathing difficulties may be more susceptible to these complications. In most cases, this is a consequence of weakening of muscles in the area of injection that are involved in breathing or oropharyngeal muscles that control swallowing or breathing (see Boxed Warning).

Pre-existing Conditions at the Injection Site Caution should be used when BOTOX Cosmetic treatment is used in the presence of inflammation at the proposed injection site(s) or when excessive weakness or atrophy is present in the target muscle(s).

Human Albumin and Transmission of Viral Diseases This product contains albumin, a derivative of human blood. Based on effective donor screening and product manufacturing processes, it carries an extremely remote risk for transmission of viral diseases and variant Creutzfeldt-Jakob disease (vCJD). There is a theoretical risk for transmission of Creutzfeldt-Jakob disease (CJD), but if that risk actually exists, the risk of transmission would also be considered extremely remote. No cases of transmission of viral diseases, CJD or vCJD have ever been identified for licensed albumin or albumin contained in other licensed products.

DRUG INTERACTIONS Co-administration of BOTOX Cosmetic and aminoglycosides or other agents interfering with neuromuscular transmission (eg, curare-like compounds) should only be performed with caution as the effect of the toxin may be potentiated. Use of anticholinergic drugs after administration of BOTOX Cosmetic may potentiate systemic anticholinergic effects.

The effect of administering different botulinum neurotoxin products at the same time or within several months of each other is unknown. Excessive neuromuscular weakness may be exacerbated by administration of another botulinum toxin prior to the resolution of the effects of a previously administered botulinum toxin.

USE IN SPECIFIC POPULATIONS There are no studies or adequate data from postmarketing surveillance on the developmental risk associated with use of BOTOX Cosmetic in pregnant women. There are no data on the presence of BOTOX Cosmetic in human or animal milk, the effects on the breastfed child, or the effects on milkproduction.

Highly recommend this dental office. I hadn't been to the dentist in much longer than I should have and was a bit nervous, my experiences have been nothing but great. The staff is very friendly and Dr. Schara is amazing.

I'll start off with 4 stars,, have never been here but my son has an appointment this coming friday,, will update as soon as we leave!! They got him in no hassle,, so far so good.. UPDATE::: THIS IS THEE BEST DENTIST OFFICE I've been to.. my son was such a big boy and the staff was amazing THANKS GUYS!! KEEP UP THE WONDERFUL WORK...

I had not been to the dentist is YEARS! I was really nervous prior to my first appt. I heard great things about Dr Schara. Kellie is wonderful along with the rest of the staff. I drive about 40 minutes just to see them:)

I can only say positive things about Dr Schara. I have had everything done at his office: root canals, crowns, fillings and cleanings. He has always taken special care to make sure I am at ease during the process. Not to mention he is kind, personable, professional, and has a great sense of humor. I have also been fit in for emergency appointments without hassle. I have never felt pressured into having unnecessary work done, and have always felt heard. I have had no issues with the front desk staff. They have always made sure to check my insurance prior to work being done and communicated the results prior to work being done. Contrary to other reviews, they have always been pleasant and personable to deal with.

Best dentist office I have found in years. Dr Schara is really good at what he does and takes time with his patients. . The rest of the staff is just as friendly and treat me with compassion and care. Lastly, the prices are much more affordable then some of those fancy pants places in Woodbury. I've been to most of them and would take this place over them any day of the week. .

Thank you Schara Dentistry for taking so many precautions to be sure my safety and the safety of your staff were number 1. I was a little "nervous" to be there with my mouth open to anything in the air. After sitting in the chair and seeing all the precautions, I was put at ease instantly. Thank you again for always making me feel like number 1.

I want to preface this by stating that Dr. Schara is fantastic. He is very prompt and does a great job. His hygienists are also great and very detail oriented. His front desk staff is abysmal. I first started going there in early February of 2020 when one of my front teeth broke. They got me in, did x-rays and created a treatment plan. Now this is where things started getting weird for me. They told me they needed to do a deep clean to see everything that was going on. I agreed and they started the cleanings. Every time I went back, more and more fillings were added to the list. Originally I needed 5 fillings, two one one side and 3 on the other. Last week, I got the right side done and fillings put in. Now again it was supposed to be the whole right side and an extraction. They told me we ran out of time to do the extraction and I would need to reschedule it, so I did. While they were doing the fillings, the Dr. kept bumping the tooth that is broken and I would wince in pain. He said we should probably take care of that before we get close to maxing out my insurance. He said this in front of his assistant so I assumed she would catch that and add it to the notes ( remember this was my chief complaint) Fast forward to yesterday and I come in and they do the deep clean on the left side and fillings and an extraction. All is well. I pay for it and ask what next weeks out of pocket total was going to be. They tell me $500 because my insurance was maxed out. I was in complete disbelief. They maxed out my insurance but didn't address the main issue, the whole reason I even went into the office. So after a bit of arguing back and forth, me becoming more and more agitated, I cancelled all future appointments and left( before I said or did anything I would regret.) I get a call this morning that went to voicemail. They wanted to discuss future treatments. I called back, expecting to at least hear an apology for not addressing the main issue, but alas didn't receive one. I informed them that I would no longer be their patient and went about my day. I get a call about 20 minutes ago from Janelle. She starts talking and I apologize for getting so agitated yesterday. She is talking about doing a partial or something that is less expensive then the crowns they wanted to do on the broken tooth. I then go back to how disappointed I am that they would max out my yearly insurance and not address the main issue. She start going into how I said I didn't care, that it needed to be done. Now I did say that it needed to be done, but the main issue should have been addressed first. I told her that Dr. Schara had even said that it should be addressed. Now here is where things get weird. She starts yelling into her end of the phone and calling me a liar. She says that Dr. Schara has no idea what insurance maxes at and that we can start over if I stop lying. Couple of issues here. 1. I am sure a Dr. who has done this as long as Dr. Schara has, can figure out when insurance will max out. 2. As someone who is in customer service, why would you yell at the patient/ customer and call them a liar. So I interrupt her and tell her that if she wants to calm down and stop yelling at me, she can call me back. I hang up the phone and start going back to my day. Little history on me here so you all know context. I have not been to a dentist in 5 years. My teeth need alot of work, which I am fully aware of. That being said, I am in sales. I am missing a front tooth and have another one broken at the gum. Both of which prevent me from eating anything I can not put on a spoon and avoid that area. Both areas cause immense pain if I am not super careful. Sorry for being long winded here, but I think people should know what they are getting themselves into before they choose this office. Again, Dr. Schara is great, the couple hygienists I worked with were good. The front staff and whom I assume is the office manager are completely unprofessional and do not have the patients best interest at the forefront. I will never go back nor would I recommend them

I've thought that the staff and hygienists were rude, as others have commented in their reviews. During my first appointment with them, they told me that I need braces, despite my previous dentist always commenting on how lucky I was to have straight teeth. Another time, they told me I had 2 cavities. I was at home on break from college out of state at the time, so I asked them to send the x-rays to a dentist near the university where I would have it filled. That dentist said that I didn't need a filling, and just recommended that I use a fluoride mouthwash to remineralize the tooth. I do not trust this practice and I do not recommend them.

I've had Dr. Schara as my dentist for over 10 years. Both he and his staff are always prompt, courteous and professional. They make routine teeth cleanings easy and fast. They were even able to fit me in to repair a cracked tooth on the same day I called, which was a relief because I was set to leave the next day for a week away. Love this office and team!
