Alabama Medicaid is a program that pays for routine dental care for the children of Alabama under the age of 21 years, as long as the child is eligible for full Medicaid. Most children are no longer eligible after their 19th birthday unless they qualify for another category. Adults age 21 and older are not eligible for dental coverage through Medicaid.

More than a million Alabama residents each year qualify for full or partial Medicaid coverage. To qualify for Alabama Medicaid, applicants must meet and provide proof of income, meet age requirements, fill out forms correctly and turn in a completed application to the correct office or worker.

Alabama Medicaid dental services must be provided by or under the supervision of licensed dentists enrolled as Medicaid dental providers. Routine procedures involving the teeth covered* by Medicaid include:

  • Limited outpatient and inpatient hospitalization coverage for adults with dental problems exacerbating medical conditions (the dental procedures themselves are not covered, only facility and anesthesia services)
  • Orthodontic services considered medically necessary for children with cleft lip/palate and/or other severe facial and cranial abnormalities, and who are served through Childrens Rehabilitation Services
  • ALL Kids is a low-cost health care coverage program for children under age of 19 years. Benefits include regular checkups and immunizations, sick child doctor visits, prescriptions, vision and dental care, hospitalization, mental health and substance abuse services, and much more.

ALL Kids uses Blue Cross Blue Shield of Alabama (BCBSAL) to provide services through their preferred provider network. If your child is approved, coverage will begin on the first day of the month after the application is received.

Income eligibility determination can only be made when a complete application is received by ALL Kids. Please note, even if your income falls outside of the below guidelines, you may still be eligible for coverage the only way to know for sure is to apply.

  • The PPO network is a group of doctors, dentists, pharmacies, hospitals and outpatient facilities enrolled as BCBSAL providers to offer services to ALL Kids enrollees at special, prearranged rates. There is no deductible when you use a PPO provider. Some services may require a small copay.
  • A copay is a small fee you may have to pay up front when your child sees the providers. Depending on which coverage bracket your child qualifies for, some services may require a copay. Your copays are determined by the ALL Kids fee group your child is in. There are no copays for preventive services like regular check-ups, immunizations, dental cleanings and vision exams.
  • You can choose any doctor who is enrolled as a BCBSAL PPO provider. To find a preferred provider in Alabama, you can look in your ALL Kids Preferred Provider Directory, call the Blue Cross and Blue Shield of Alabama dedicated customer service number, 1.800.760.6851, or visit Blue Cross and Blue Shield of Alabama.

The floors are absolutely disgusting! This should be a sanitary environment for our kids to have dental work done but it is so far from it. This trip they once again had no sanitizer in the dispensers, bathrooms were absolutely disgusting no toilet paper or paper towels and looked like they haven't been cleaned in months. The carpets are covered in stains and the chairs in the waiting room are disgusting. Clearly no cleaning gets done here but they want you to wear a mask?! My daughter had to have a filling and the entire inside of her cheek was swollen and bleeding with cuts all over the inside of her mouth. Just because we have Medicaid insurance we shouldn't have to go to a dentist that is so rough, and the environment is not kid friendly plus it's always disgusting. I've honestly never seen anything like it!

Prior to Covid, my children had an appointment set for a couple of months. When we arrived for the appointment, we were told that the dentist was not in so our appointment was cancelled. No explanation other than that. I understand unforeseen events come up, but a phone call would have been nice, even if it were the morning of. Post covid and now under a new name, we thought we would give it another shot. Upon arrival, I was given two packets per child to update info (I'm assuming - nothing was said) some signature areas were highlighted. So, I signed the highlighted parts and returned to the counter asking if this was all or did I need to fill out more? As there were about 10 pages per packet = 40 pages(!) The receptionist told me that I needed to fill out the bottom packet for each child - good to know now that we've actually communicated. As I'm filling out the paper work, all I can think is it would have been nice to be warned beforehand that you would have several pages of paperwork to fill out - OR, better yet, I would suggest a link to do so online PRIOR to the appointment. We arrived for the appointments on time and still waited an hour and 15 minutes for my children to be taken back. The first impression from walking in is cold & unfriendly. Once taken back, the hygienist and the dentist were more friendly but overall, it was less than satisfactory.

They don't answer their phone, have a confirmed appointment at 2pm show up at 155pm doors are locked, made repeated calls, knocked, attempt to get apt completed. 215 pm still sitting g in parking lot. They didn't answer the phone or door. When I tried making an appointment couldn't get ahold of anyone either.

Took my 5 year old son to have a cap put on his tooth. This was his first time having anything like that done to him. The female dentist and her assistant do NOT need to work with kids. You can't throw a kid in a chair get them high on gas then get upset with them when they start getting upset because they have no clue why they are feeling different or what in the world is going on. They gave no explanation to kind words no encouragement no nothing. She will never touch my child again!

I took my daughter here a few months ago. The female dentist and dental assistant just seemed rude and unhappy to be there. They didn't explain anything to my daughter. The dentist put a face mask on her for nitrous oxide, which freaked my daughter out and the dentist just kept saying sniff, sniff ,sniff. Ignoring my daughter when she asked what it's for. So of course my child freaks out and gets up. They then gave us a referral to a different dentist. They didn't try to calm her or explain anything. I tried but they didn't. They just sent us out the door. I think you need more kid friendly staff!

We recently moved to the area and my daughter had a tooth hurting her. We called several offices and no one was seeing new patients. Thankfully Sarrell in Boaz was able to get us in same day and were so nice. They even called to follow up on us. We will definitely be back to use them as our dentist.

I was a potential new patient at Sarrell Dental in Boaz but after being in the waiting room with a friend and her child I have decided that I will avoid going there at all costs. I was sitting very close to the front desk area and I could overhear the girls who work there talking bad about patients and laughing at them. I would not recommend this place to anyone unless you want to be made fun of by the staff. It is very unprofessional to not only speak so terrible about their patients, but to go to the level that these front desk staff do is very disheartening.
