Salud is a Federally Qualified Health Center (FQHC), 5013 nonprofit, operating 13 clinic locations, administrativeand training site, and a mobile unit. We provide medical, dental, pharmacy and behavioral health care services to keep you and your family healthy.
As an FQHC Salud is governed by a patient-consumer Board of Directors, which ensures patients direct delivery of healthcare services.Salud receives operational funding from Health Resources & Services Administration (HRSA) to provide health care services in the communities we serve.
We focus on low-income, medically underserved populations as well as the migrant and seasonal farmworker population. Salud Family Health Centers accepts Medicaid, Medicare, CHP+, and most private insurance plans. We offer a sliding fee scale based on family size and income. Insurance enrollment assistance is available.
Salud serves as your Patient-Centered Medical Home (PCMH). The foundation of aMedical Home is the relationship between the patient, family and the Care Team. Services are designedto respond to your personal goals and to support self-managementof your health.
