Dr. Ralph B Montgomery founded Salt Lake Donated Dental Services (SLDDS) in 1990 and is a member of the Board of Directors and Chair Emeritus.He founded the clinic on a firm belief that a healthy smile should not be limited to only those with financial means. Dr. Montgomery graduated with his Doctorate of Dental Surgery from Washington University School of Dentistry in 1954.He practiced dentistry for 36 years in Murray, Utah and served with the Clinical Research Associates of Provo, Utah where he participated for 21 years.
Dr. Montgomery has also served on the Utah Dental Board of Examiners and the Northwest Board of Examiners as well as lectured domestically and internationally to dental societies and dental schools on dental techniques and how to organize volunteer dental services. In the early nineties, Dr. Montgomery volunteered to go to a Viet Nam Refugee Camp on the island of Palawan in the South Pacific.Finding that the residents of Palawan were much worse off than the refugees, Dr. Montgomery returned home and raised money to build a mobile dental clinic complete with an X-ray machine and generator.With the help of Utah Senator Orrin Hatch, he was able to get the U.S. Air Force to fly the mobile clinic to Palawan.Through Rotary Clubs, in many states, volunteer dentists were recruited to go to Palawan for 30 days at a time to treat the Palawan people.
Today, this mobile dental clinic is still in use. Prior to taking the mobile dental clinic to Palawan, Dr. Montgomery brought it to 400 South and 400 West in downtown Salt Lake City, where meals were provided for the homeless. At one of those meals he announced that free dental care was available for the next week.There were so many individuals seeking care that Dr. Montgomery created Salt Lake Donated Dental Services (SLDDS), an organization that provides free quality, comprehensive dental care for the homeless and poor.The clinic has had several homes since its inception and is now located in the heart of the Glendale community in the beautiful Sorenson Unity Center, at 900 West and 1300 South.

The staff is pleasant, and very professional, the clinic is nicely well maintained, and the Doctor who I saw was most affable. All of which is why Im giving 3 stars instead of 1. But this is a business after all and I as a customer can't understand why dentists always wanna extract a tooth?? Im pretty sure my tooth is still viable. For crying out loud check out the discovery channel any given day, and all these ancients that are unearthed from like 55 bc have full set of teeth still??! Surely a modern dentist should be able to fill a tricky cavity in 2022!! Along side that, if I want a filling and don't want a tooth extracted shouldn't that be what I recieve?,. I am paying for the procedure after all. liability shouldn't be a concern, because I'm quite happy to assume responsibility if it fails. (Assuming of course Mr dentist does his best work to prep and clean the tooth); Im confident that the repair would work out fine!! Ultimately for me, its frustrating to learn that donated dental services, or discounted services means extract not repair. Btw, This is not a charity. it is a fully staffed and functioning dental clinic just with fair and reasonable pricing.

Received excellent service everyone was courteous polite and helpful and knowledgeable. I was my first visit. Alexis- called me after a cancellation. She knew I had pain issues. She has a such great personality and knows her stuff. She helped the new front desk person with questions in such a polite way. Dr Michel Haynie- was so caring and explained to me in detail the procedure he was about to perform I will certainly go back for his services. Laurissa - Dr Haynie's assistant was very knowledgeable and explained in detail my aftercare for my procedure. I like that Dr Haynie and Laurissa had that close connection working together. Thank You I hope the public appreciate the services you all perform so well.

After they told me I qualified for free dentures they told me I would have to wait a total of about a month to get them. I asked if I had to pay anything and they told me "No". After a month and 1 appointment I was told it would be a 2 month wait, 1 months later and 2 appointments, I had half my teeth extracted and was told it would be another month til I was done. The next month I got the last of my teeth extracted and then was told it could (and most likely) be 5 months til I could be fitted and get my dentures made. 3 months later they call and tell me they have a appointment open for me, BUT!! I have to pay $750 for lab fees to get the dentures.!!! AFTER THEY EXTRACTED ALL MY TEETH. Then they tell me. Now I have no teeth and can't afford the $750. I have a hard time eating, the extractions make me look horrible. I've fallen into a depression. Thanks Donated Dental, you helped make my situation worse, not better.

My experience with donate a dental has been very pleasant I was told I need to get a few documents and I did brought them in and and I seen the dentist. They extracted my teeth at no charge. Cause I proved that I had no money coming in. I waited quite some time and they called me and said I could come get fitted for my dentures and then it would cost $450 for Lab fees. And for a smile that's not very much sing I haven't smiled in five years yes I had to save up some money. And the dentist recommended I get a bottom partial so the teeth would lineup and I couldn't afford that so we went with just the upper denture however to my surprise when I arrived to that appointment they informed me that they had found some funding for my lower partial. I was so happy and the dentist that did my work was upfront with me didn't pull no punches and I appreciate that so thank you very very much I would recommend them anywhere else would've been double or triple the price that I paid!!

I had my daughter go to Donated Dental after she had been beaten and abducted. Her Captors knocked out all her front teeth .We almost lost her to Human Trafficking she awoke in Atlanta GA Where she managed to escape. The person who I spoke to named Luke, told me by phone that they only take applicants that have pay stubs and if we are self-employed our documents are not "RELIABLE" really????? so just like that we do not have the opportunity. I understand there are policies but, saying that my proof of income is not reliable. What the heck???

Donated dental is an extraordinary facility of absolutely amazing people for which I am very grateful for having found this awesome place of kind souls for saving my teeth when I had no where else to turn nor the money to pay out of pocket. If it wasn't for them it'd cost thousands in which I could not afford and would have been dealing with a mouth full of pain and hurt to this day, the gratitude I have for everyone here I can't even express in these texted words. Miracles happen here so if your in need of a trust worthy honest place of folks that truly care about you, your health, your teeth and your well being this certainly is the place to come to if you haven't anyone else to turn to. I went there for remove the wisdom tooth. All the procedures and treatments were ok and also all staff members were really courteous, specifically Lucas. I definitely have no complaints and I recommend it.
