Rooted Dental Care started with the idea of creating a modern neighborhood dental office that is accessible to patients of all ages, in the heart of the Lincoln Park neighborhood in Chicago. We offer an uncompromising approach to dental care, utilizing advanced technologies, and foster long-lasting relationships rooted in our community. By providing education on prevention and general wellbeing, we hope to reduce the stigma around and alleviate the anxiety associated with dentistry.

After graduating fromCase Western School of Dental Medicinein 2011, Dr. Priyanka Patel, DMD, completed a one-year residency program at the Cleveland VA Hospital. This advanced training solidified her deep conviction and commitment to providing state-of-the-art dental care needs.
Being from Chicago, she could not think of a better city to come back to and establish herself. She enjoys taking the time with my patients to understand their needs and help achieve the final result they are looking for, whether it be maintaining or enhancing their smile. Dr. Patel focuses on helping reduce patients anxiety from the moment they step in the door to the moment they leave. Dr. Patel enjoys spending time with her husband, daughter Maya, and two dogs exploring the city in her free time. She also takes time to unwind both at home, painting or cooking, or abroad, exploring other countries and cultures.
Dr. Nisha Chauhan completed her undergraduate studies at the University of Illinois at Champaign-Urbana where she received a Bachelor of Science in biology and psychology. She then graduated from the New York University College of Dentistry where she obtained her Doctorate of Dental Surgery. Dr. Chauhan is committed to life-long continuing education and believes a good clinician must keep up with the latest dental knowledge and advancements in technology. She regularly attends continuing education classes, ensuring that her patients receive the most comprehensive and advanced dental care. She strives to treat her patients with compassion, understanding and strongly believes in the importance of communication and an excellent patient-doctor relationship. She is a member of the American Dental Association and Chicago Dental Society.In her spare time, she likes to explore the city with friends, try new recipes, draw and travel.
Regan graduated from Ferris State University in 2021 and is currently finishing up her Bachelors Degree of Science in Dental Hygiene. She grew up in Kankakee, Illinois, and recently moved to the city. In her free time, Regan enjoys spending time with her kittens, friends, and family as well as going shopping and finding new coffee shops to check out!
Erica graduated from Western Kentucky University in 2018 with a Bachelor of Science in Dental Hygiene. She grew up in the Chicagoland suburbs but now loves living in the city. In her free time, she enjoys walking along the lakefront, traveling, and spending time with friends and family.
Sayde has been in dentistry for over 10 years as an assistant, insurance coordinator, and office manager. Her versatile dental background shows her dedication to the field and to her patients. She has a daughter and a grand puppy; and when shes not working, she enjoys traveling and running on her days off.
Originally from Chicago, Stephanie has been in the dental field for 5 years now. Primarily working as the face of the office, her bright personality shines through when talking on the phone and greeting you in person. On her off time, she enjoys spending her days with her two-year-old son, Logan. She also loves to cook, read, and watch plenty of reality television shows.
Maria has been in the dental field for 2 years, as an administrative assistant and insurance coordinator. She is passionate about her work because and loves what she does. Her steady source of motivation drivers her to do her best. She graduated from cosmetology and spa academy in 2015 as an esthetician, which became her passion and in her free time she works from home doing facials. She enjoys walking with her dog, cooking and listening to music

We provide estimates for the dental work you might need based on the information we receive about your policy. Rooted accepts all PPO insurances and those that do not have any insurance coverage.
