In the past, replacing upper back teeth involved placing implants in the grafted maxillary sinus area. This used 8 to 10 implants plus a sinus lift and graft in the upper jaw. The novel All on 4 technique (when appropriate) places implants at an angle tilted to the back of the mouth. This avoids the sinus area entirely and may eliminate the need to use grafting or a sinus lift to replace missing back teeth. Not All patients can use this technique because (for example) sometimes the sinus can extend so far into the front of the jaw, or sometimes there is too much bone missing, to use an All on 4 approach.

What has evolved from the fruits of technology and science is a long-lasting, comfortable, esthetic replacement of natural teeth. The patients who I have treated with this technique no longer have the sad half-closed eyes in their preliminary photographs. The new pictures show them smiling and happy, with their eyes fully opened. They are enjoying the benefits of improved self-esteem when they eat, talk and laugh with the world again. These are some of the benefits that the All on 4 technique or similar techniques have brought these patients.
For the nuts and bolts of All on 4 planning, when designing an implant-supported bridge, it is important to not only replace the teeth but also to make sure that there is an improvement in appearance as well as function. It is important to find out why the teeth will be or have been lost in the first place. The final bridge should correct the reasons for the lost teeth in the mouth as well as improving function and esthetics.
That requires some detective work and a preliminary evaluation to make sure that the final bridge will not fail. Bridge failures have been known to occur when a thorough investigation and a corrective plan is not made. I prefer photographs and model copies of the preexisting teeth so I can detect the problems that caused tooth loss in the first place. Was it a tooth crowding problem, was it lacking home care, was it parafunctional habits such as unconscious tooth grinding, day or night?

Just visited Dr. Korwin to have a crown recemented. I was one of the first post COVID reopening patients. As I expected Dr. K's and his team had very professionally implemented a set of protocols to protect patients and the team. My family and I have been treated by Dr. Korwin for about 20 years and I have always been impressed with his professionalism: 1. I've never waited more than a few minutes. My time is respected. 2. Dr. Korwin explains options clearly and manages risk of complicated procedures wisely. 3. He has modern equipment. 4. The front desk staff, Carol, Nancy, Peggy and Stephanie are unfailingly cheerful and friendly. They greet me like a member of the family. 5. On the occasions when I have needed an emergency appointment I have been able to get one within 24 hours in every case. Thank you Dr. and the team.

Dr. Robert Korwin is an excellent dentist! He's kind and compassionate and very concerned about his patients being comfortable and not anxious regarding dental procedures. My, husband, Bob, has been seeing Dr. Korwin for many years. When my dentist retired, Bob recommended Dr. Korwin. I am so happy he did! I have complete confidence in him and always feel like I'm in good hands. His staff is terrific too! They are all very friendly and really good at what they do! I am sooo blessed and grateful that I found Dr. Korwin and his team. Everyone at his practice is always extremely professional and kind. Peggy and Jessica at the front desk are a breath of fresh air... they are always genuinely friendly and so helpful. All of the hygienists are great... they are very thorough with awesome bed-side manner, especially, Denise and Sally. I have always looked forward to going to the dentist, however for anyone who is fearful, everyone's hands truly do have a compassionate and gentle touch! They go above and beyond to protect everyone with their covid protocols, so there is absolutely no need for any patient to worry about that. Also, they are the first and only dentist practice I have ever visited that takes vital signs prior to going in. I'm a nurse and I am very aware of how valuable this simple measure is as it could prevent a major problem from occurring in a patient who is unstable... I'm extremely impressed with this extra precaution they take. I underwent Invisalign treatment with Dr. Korwin and I was super grateful he informed me of the important of replacing my retainers. I had underwent Invisalign in the past, however that provider never informed me that I would need to replace the retainers and as a result, all of my teeth shifted, making the entire process a complete waste. I was so impressed with all of the education Dr. Korwin and his team provided throughout the entire Invisalign process. Dr. Korwin is a remarkably dedicated professional and is extremely meticulous at his craft... he has shaped my teeth so beautifully, they have never looked better... all thanks to his naturally talented hands and artistic eye! The directions were easy to follow and I found the staff to be very friendly and attentive, plus they took the time to read my rather long and complicated medical history.
