Our highly-trained and dedicated team of professionals work to create personalized treatment plans aimed at enriching the health and well-being of all patients and their families. Our care team includes board certified and board eligible Physicians, Nurse Practitioners and Physician Assistants complemented by a wide variety of support staff that provide comprehensive, high-quality, cost- efficient medical care regardless of the ability to pay.

Services include Behavioral Health, Dental, Family Medicine, Pediatrics, Podiatry, Womens Health, and Community Services, which includes ACCESS (Assisted Service Site), Diabetes Self-Management Support, Medication Assistance, Outreach and Enrollment Assistance, Patient Care Coordination, Smoking Cessation, and other wellness programs.

In 1978, a group of citizens from the Trilby Civic Group met concerned for people in need in the area. This group of volunteers secured donations to open East Pasco Health Center in January of 1979 and became the inaugural Board of Directors. The first patients were seen in a modular facility that served as a health center with one primary care physician providing services to medically underserved patients. Later that year, the center was fully incorporated as a 501 (c)(3) non-profit organization. It was designated as a Federally Qualified Health Center and a Migrant Health Center, led by a consumer majority community board.

From those humble beginnings, the center went through names changes, location changes, renovations, expansion, and significant growth. Its current name, Premier Community HealthCare Group, Inc., was established in 2004 to better brand itself as a primary care provider for the entire county. Premier Community HealthCare has grown from serving a few hundred patients in a modular unit with one provider and less than 10 employees to serving more than 36,000 patients at multiple Pasco and Hernando County locations with an average of 250 care team members. Since its inception, the organization has provided more than one million patient care services.

Today, Premier Community HealthCare employs a highly-skilled, compassionate, and culturally diverse workforce committed to improving residents lives and removing healthcare barriers for the uninsured and medically underserved communities through high-quality, affordable, accessible primary and preventive care.

After earning a medical degree at Ross University School of Medicine, Dr. Heron completed her residency at the Albert Einstein College of Medicine, Jamaica Hospital Medical Center in New York. As the Chief Medical Director, Dr. Heron continues to see patients at the Zephyrhills Family Health Center and also leads the team of Premier providers. She is responsible for all clinical outcomes of the health center.

    Go anywhere else. Went here with my 5 year old son. The questionnaire they give asks what gender he identified as. Strike one. Then when you go there they act like it's April of 2020 with covid measures. They still have seats blocked off. Strike two. Then, even though my insurance told us to go here, they send me a bill for $231. If I knew I was going to pay for this anyway, I'd go to a dentist that is not run by people who clearly care more about woke virtue signaling than kids dental care. I will never go back, and you should consider literally anyone else.

    The dentist didn't tell me anything of what he was doing. Came in, did his thing and literally went home. On top of that I was unaware that for a filing you have to come INDIVIDUALLY to the office so they can fill them. I have three that need to be filled so I have to pay $120. Are you serious?? What dentist doesn't fill them all at the same time? I'm confused because this is supposed to be low cost but nothing about that is low cost.

    How is it , for an emergency dental you don't have a scheduled visit till the following year? So one is suppose to wait 1 complete year with a tooth infection with the operator not giving any options at all.

    Here for a hospital f/u due to heart problems and refused to be seen due to covid test not being in!?!?! New policy! Really. Wish I could give zero stars!!!

    The receptionist Anita's is a very nasty unprofessional individual. Like just do your job reception. She hung up the phone and refused to let me speak to a supervisor because I engaged in a conversation that did not have anything to do with her when she was suppose to be getting the supervisor or director on the phone once I gave her my name and date of birth. Now I'm not getting an answer lol like she can denied my services. The reason for the phone call is because they are telling you they take your Medicaid or insurance and really don't and don't wait for approval on if your insurance will cover services and bill you instead.

    This office put fliers all around town for a day of free dental services with the only requirement being that you must have an ID. But, when I called about it, the representative rudely told me that I misunderstood the flier even after I read it to her word for word. She claims that it is only for people who work in a certain career field. This is false advertising and I definitely would not recommend this office. Unfortunately, this is the only option for those on Medicaid. If you have other options, it's better to pursue those.
