Our highly-trained and dedicated team of professionals work to create personalized treatment plans aimed at enriching the health and well-being of all patients and their families. Our care team includes board certified and board eligible Physicians, Nurse Practitioners and Physician Assistants complemented by a wide variety of support staff that provide comprehensive, high-quality, cost- efficient medical care regardless of the ability to pay.

Services include Behavioral Health, Dental, Family Medicine, Pediatrics, Podiatry, Womens Health, and Community Services, which includes ACCESS (Assisted Service Site), Diabetes Self-Management Support, Medication Assistance, Outreach and Enrollment Assistance, Patient Care Coordination, Smoking Cessation, and other wellness programs.

In 1978, a group of citizens from the Trilby Civic Group met concerned for people in need in the area. This group of volunteers secured donations to open East Pasco Health Center in January of 1979 and became the inaugural Board of Directors. The first patients were seen in a modular facility that served as a health center with one primary care physician providing services to medically underserved patients. Later that year, the center was fully incorporated as a 501 (c)(3) non-profit organization. It was designated as a Federally Qualified Health Center and a Migrant Health Center, led by a consumer majority community board.

From those humble beginnings, the center went through names changes, location changes, renovations, expansion, and significant growth. Its current name, Premier Community HealthCare Group, Inc., was established in 2004 to better brand itself as a primary care provider for the entire county. Premier Community HealthCare has grown from serving a few hundred patients in a modular unit with one provider and less than 10 employees to serving more than 36,000 patients at multiple Pasco and Hernando County locations with an average of 250 care team members. Since its inception, the organization has provided more than one million patient care services.

Premier Community HealthCare board members either reside or work in our service area. Many are involved community leaders and 51% utilize our services as required by the Bureau of Primary Health Care. This provides our board members first-hand knowledge and experience in regards to the level of care provided.

Aaron Brandt joined Premier Community HealthCare Group, Inc.as the Chief Financial Officer (CFO) is fall of 2018. Prior to joining the organization, Brandt served as the Finance Director for the Community Health Center of the New River Valley in Blacksburg, Virginia. He earned his double major undergraduate degree in accounting and management from Saint Leo University. Brandt is a proven leader in the fields of healthcare management, financial and strategic planning.

Dr. Heron joined Premier in August 2015. Dr. Heron has practiced medicine as a primary care physician for twelve years. After earning a medical degree at Ross University School of Medicine, Dr. Heron completed her residency at the Albert Einstein College of Medicine, Jamaica Hospital Medical Center in New York. As the Chief Medical Director, Dr. Heron continues to see patients at the Zephyrhills Family Health Center and also leads the team of Premier providers. She is responsible for all clinical outcomes of the health center.

Joined Premier Community HealthCare in late 2020. Prior to joining Premier, Jennifer worked at Raymond James in St. Petersburg, FL where she served as a Human Resources Director.She is responsible for all aspects of recruitment, orientation, annual evaluations, conflict resolution, employee relations, credentialing, benefits, and separations. Lineburg is dedicated to supporting all employees to reach their career goals and objectives and upholding the Premier mission of reducing barriers to healthcare services.

Dr. Jose Peralta joined Premier Community HealthCare as the Dental Director in 2015 and was promoted to Associate Medical Director in 2016. Dr. Peralta has practiced dentistry for twenty years. Read Full Bio.

Cheryl Ann Pollock joined Premier Community HealthCare Group, Inc. as the Director of Business Development in 2012 and currently serves on the senior leadership team as the Chief Advancement & Community Engagement Officer. She is a veteran project manager, grant writer, community organizer and trainer/group facilitator with over 25 years of experience in the public health & human service fields. Her strengths include strategic project development, public relations, community engagement, project management, annual/capital campaign development, grant-writing and management. Cheryl also has a passion for implementing results-based programs that help people and communities thrive.

Ibrahim (Abe) Farah joined Premier in June 2021 as the Chief Operations Officer (COO). Farah joined Premier from Santa Cruz Valley Regional Hospital in Green Valley, Arizona. As Chief Operating Officer, Abe will work closely with CEO, Joey Resnick, and clinical leadership, focusing on continued operational improvement, integration, and other strategic initiatives that impact the overall quality, goals, and patient experience. Responsible for day-to-day operations, he will ensure that the quality of care for patients mirrors the mission, vision, and core values of Premier Community HealthCare.

Tina Beehler, RN joined Premier Community HealthCare in 2014 as a nurse, served as the site manager for Dade City Family, and was promoted to Nursing Director in 2021. She is responsible for all aspects of the clinical support team, assisting the Chief Medical Officer, and establishing and maintaining all clinical policies.

Rhonda Cameron, Ph.D. joined Premier Community HealthCare Group, Inc. as a Behavioral Health Counselor in 2001. Since joining the Premier Community HealthCare team, Dr. Cameron has evolved as a vital team member in multiple roles. As the Director of Behavioral Health Services and Executive Leader, Dr. Cameron is responsible for the strategic direction of behavioral health services, ensuring clinical performance measures are met.

TravisCox joined Premier Community HealthCare in 2020. He previously worked as the Chief Information Officer for True Health and Nevada Health Centers. He brings more than 30 years of experience in Information Technology, Voice and Data Networking, and Business Operations, as well as 15 years of experience working with Federally Qualified Health Centers.Travisoversees a team of three IT specialists, Facilities, and Call Center. He is responsible for all aspects of Information Technology, including Electronic Medical Records, networking, telephone, and data security.

Jesse Dang joined Premier in August 2021 as an Operations Director. Dang completed his undergraduate studies in Biomedical Sciences and graduate studies in Health Administration and Business Administration at the University of South Florida. He is a Certified Project Management Professional, a Certified Lean Six Sigma Black Belt, and a Certified Professional in Healthcare Management Systems. Jesse oversees Zephyrhills Family, Behavioral Health, Womens Health, and future growth as Premier expands service lines.

Mrs. Marcy Hammond joined Premier in 2019 as the Director of Risk Management & Compliance. Marcy brings 19 years of experience in healthcare and certifications in Risk Management, Healthcare Quality, and Health Information Management. In this role, Marcy oversees risk management and ensures Premier meets or exceeds standards for quality, ethics, safety, and privacy.

Mrs. Gabriela Herrera joined Premier Community HealthCare in 2004 as a referral specialist. Over the years, she has worked in various positions ultimately rising to the role of Office Manager for the Zephyrhills Family Health Center. After leading the site for nearly four years, Mrs. Herrera was promoted to Director of Operations. Herrera leads the team of office managers and is responsible for all aspects of health center operations to ensure consistency, efficiency, and the optimal patient experience.

Suzanne McEachron, MS joined Premier Community HealthCare in February 2021, as the Director of Dental Operations. Prior to joining Premier, Suzanne worked at Community Health Centers of the Rutland Region in Rutland, VT where she also served as Operations Director. McEachron has a Master of Science degree in Healthcare Transformation from the University of Texas in Austin, TX. As the Director of Dental Operations, McEachron augments our leadership team with over 14 years in management serving the underinsured population.
