Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Our dental office uses the latest state-of-the-art equipment and cutting edge technology and we uphold the strictest sterilization techniques. We view it as our mission to educate our patients about all of their oral health care options and to help guide them to choose a treatment plan that is most suitable and appropriate for their needs.
We accept Mastercard and Visa credit cards as well as cash and personal checks. We accept many insurance plans including all PPO plans. We are contracted in network with Cigna Radius, Assurant, DELTA Premiere, GEHA, BCBS of Florida, and Guardian. In addition, we accept financing options through CareCredit. I have recently relocated to Jax from NYC and I consider myself lucky because I found this Dental office !Highly recommend Ponte Vedra Dental. I would give them ten stars , but i am limited to five. I admire their service and quality of dental care. Everyone in the office is extremely professional , personable and attentive. Doctor Newaz does great job fixing even most difficult and problematic teeth. And the hygienist Runa that works in the office is superb. I most definitely can state that that was the best dental cleaning experience I have ever had. On top of all- their pricing is very reasonable and the quality of Ponte Vedra Dental office work definitely is worth every penny!!!

Dr. Bolton came in to see me and it felt very rushed, he did not even stand in front of me when he was talking. Front desk was very rude and rushed over the phone. Hygienists we're the only plus.

I just moved here from NC and would highly recommend this practice. The whole team was knowledgeable, professional and very nice! The doctor also caught a lot that I had brought up to my previous dentist but he had brushed off. I finally feel like I have a great dentist!

This dentistry is awesome! Dr. Chaffair and his daughter take great care of their patients. I've had my wisdom teeth removed, several cleanings, and recently my crown replaced. I cracked my crown on a Saturday and called them. They came on short notice and replaced it that day! HIGHLY RECOMMEND!

This dentistry is awesome! Dr. Chaffair and his daughter take great care of their patients. I've had my wisdom teeth removed, several cleanings, and recently my crown replaced. I cracked my crown on a Saturday and called them. They came on short notice and replaced it that day! HIGHLY RECOMMEND!

Doctor Chiafair was excellent. During and after my implant I did not experience any pain or discomfort. There were no swellings or pain after the process. The whole experience was less invasive than a regular cleaning. I have no reservations to recommend him for any dental work. He will definitely continue to be my dentist. He is very confident and assuring. The staff is great.

Everyone here is very helpful, friendly and professional. Excellent treatment from thorough cleaning to a painless/easy first root canal. Appreciated that they took the time to fully explain everything beforehand, including the insurance.

The whole staff were completely helpful with anything I asked, and the staff had very good customer service through my whole time there. I had the impression from the first few sentences while talking to Dr. Chiafair that he was very knowledgeable and professional, and he did not disappoint! He was very accurate and precise with my mouth. I'd gladly recommend this office to anyone!
