Clayton Cook was born and raised in Cache Valley, Utah, and comes from a large family he is the 5th of 6 children. Growing up, Dr. Cook spent many hours in his fathers podiatry office cleaning rooms, stocking cupboards, and when his dad wasnt looking playing with equipment. Consequently, Dr. Cook discovered his fascination with the medical world at a very young age.
Dr. Cook met his best friend, Kallee, now his wife, in 9th grade. After graduating from high school, he worked in the Dallas area for a short time and fell in love with Texas. He left Texas to serve a Christian mission in Hiroshima, Japan. Shortly after returning, he married his wife, and theyve been married for 8 years and have 4 beautiful children.
In his spare time, Dr. Cook enjoys hiking, playing sports of all kinds, reading, challenging his wife to a game of scrabble, and giving his kids Wild Buckin Bronco rides in the familys front room. Hes always wanted to make it back to Texas and is so excited to be in Fort Worth.

I have always been very picky about my dentist, and have been to several trying to find the right one; I have found the place that truly fits me. From Dr's, Hygienists, Assistants and Front Office you can tell they honestly care about helping their patients find the best solutions. I can't say enough wonderful things about this place!

Today was my first visit and if it is indicative of the future, I am extremely pleased. From the moment I entered the lobby, to the x-rays, to meeting the dentist, the whole team was extremely friendly and very professional. They thoroughly explained everything and even showed me what they were looking at. I could not recommend Polished Family Dental enough. I am typically very leary of the dentist but I think they have changed that for me. I look forward to my future with them. Amazing team.

Absolutely love this place. Front office is always nice and I never have to wait long. I have had great experiences with all the Hygienist and providers. I also really appreciate being able to watch HGTV on the ceiling while someone is rooting around in my mouth.

Came in today to get a basic cleaning and routine dental care and was blown away with the level of customer service I was provided with. Melanie was such an amazing person. Not only did she help me understand everything that was going on but she also helped me feel like I was part of the family. I will definitely be coming back and recommending Melanie's services to my friends and family!!!!
