Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Dr. Blauzvern & his team has taken care of my dad,daughter & myself for many years-always done in a friendly,comfortable manner-last year Dr.Fuchs did a beautiful,skilled job of replacing 2 teeth I had broken. I so highly recommend these dentists & all of their team.

Very professional, honest and friendly staff! Made me feel very comfortable considering I'm not a fan of someone working in my mouth. Took his time and made sure the work was done right. The office was clean with convenient parking. Overall a very good experience.

Historically, I have dreaded visiting the dentist, but now that I see Dr. Peter Blauzvern I am actually excited! His calming demeanor immediately relaxes me and he always gives me instructions on how I can care for my teeth better. I have also been fortunate to see Dr. Fuchs who is a huge bonus to the team!

My husband and I have been going to Dr. Blauzvern's office for years and we could not be happier with them. The doctors and staff are all very friendly. The office is clean. I know many people don't like visiting the dentist, but Dr. Blauzvern and his team make going to the dentist a safe, comfortable, and actually enjoyable experience. I highly recommend them.
