Dr. Niloofar Zarkesh DDS, MS is a Board Certified Periodontist specialist and the Founder of Perio4U Periodontics and Dental Implant Center where she diagnose, prevent and treat periodontal diseases and replace missing teeth with dental implants. She is the leader of the Perio4U Periodontal Practice and manages the practice and its employees, so they can provide the best possible quality service in a caring environment.

Dr. Niloofar Zarkesh attended University of Southern California (USC) Periodontics Specialty Program from 1995 to 1998. She obtained her Certificate in Periodontics and Masters in Craniofacial Biology at completion of this program. She attended a Periodontics Preceptorship Program at the University of California Los Angeles (UCLA) in 1993-1994. She got her dentistry (D.D.S.) degree from National University of Iran in 1992. She is licensed to practice dentistry in the State of California since 1996 and has been practicing as a periodontist in Southern and Northern California since her graduation.

Dr. Zarkesh started her Perio4U Practice in 2003 after moving from Los Angeles to San Jose. She relocated her practice in 2013 to a brand new building close to Santa Row and Valley Fair Mall. She has more than 20 years of experience in the field of Periodontology and has helped thousands of patients achieve optimum health anda beautiful smile through surgical and non surgical periodontal treatments as well as dental implant placement.

My passion in the field Periodontology is to be a leader in my profession and my practice and to diagnose, prevent and treat periodontal diseases providing the latest in non-surgical and surgical treatment options. Dr. Christine Hayashi has been practicing Periodontics for almost 25 years. She was raised in Palo Alto and enjoys living in the Bay Area with her husband and French bulldog, Dizzy. Dr. Hayashi received her Bachelors of Science degree in Pharmacology at the University of California, Santa Barbara. Prior to dental school, she worked for 4 years in pharmaceutical research at Syntex, Inc. in Palo Alto.

Dr. Hayashi continued her studies at the prestigious University of California at San Francisco UCSF where she earned a Degree in Oral Biology and Doctor of Dental Surgery degree. A passion for preventative care and surgery led her to continuing post-graduate training in Periodontology at UCSF where she earned a Masters Degree in Oral Biology and certificate for specializing in Periodontology. She has been licensed to practice dentistry in the State of California since 1993 and she established her own periodontal practice, Forest Periodontics, in San Jose in 2000. Dr. Hayashi enjoyed a thriving periodontal practice at Forest Periodontics for almost 20 years. She and her entire team from Forest Periodontics moved to join Dr. Niloofar Zarkesh at the Perio4U Periodontics and Dental implant Center in July 2019

Dr. Hayashi has taught clinical periodontics at UCSFs School of Dentistry since 1996 and she currently holds the position of Assistant Clinical Professor and teaches in the Post-Graduate Periodontal clinic. She is active on the UCSF Dental Alumni Association (DAA) Board, is Past President and is about to begin a new term on the UCSF DAA board as the current President-Elect. Dr. Hayashi has also held positions as a Board Member at the San Mateo County Dental Society and the Santa Clara County Dental Society and is a current member of the following organizations:

American Academy of Periodontology, American Dental Association, California Dental Association, Santa Clara County Dental Society, Omicron Kappa Upsilon Dental Honor Society, Spear Education Study Club and Spear Education Faculty Club.

Dr. Hayashi enjoys downhill skiing, playing golf and travelling with her husband. She enjoys entertaining and spending time with friends and family. She also loves contributing to her community whenever possible.

Dr. Gunstream was born and raised in Northwest Indiana and, after graduating from Northwestern University, moved to San Francisco, California to attend dental school. While a dental student at University of the Pacific, Dr. Gunstream developed an interest in the specialty of Periodontics. She received her specialty training at the University of California, San Francisco where she also conducted a research project on biofilm eradication and completed a graduate degree in Oral and Craniofacial Sciences. She is licensed to practice dentistry in the State of California since 2004. As a board certified periodontist, she is a Diplomate of the American Board of Periodontology.

Dr. Gunstream first joined Forest Periodontics in 2007 after completing her periodontal training at UCSF. But, in 2009 she relocated to Iowa, where her husband had accepted a new position. Dr. Gunstream founded Eastern Iowa Periodontics in 2009 and practiced there until 2016. Her husbands job has now brought her back to the Bay Area and she re-joined Forest Periodontics. She joined Perio4U Periodontal practice in July 2019.

In addition to clinical practice, Dr. Gunstream was a faculty member at University of the Pacific School of Dentistry from 2007-2009 and at University of Iowa College of Dentistry from 2009-2016. Dr. Gunstream has also been a guest lecturer at Kirkwood Community College. She was a continuing education provider for the state of Iowa and frequently lectured at state and local dental meetings. She is the past president of the Iowa Society of Periodontology and past president of the Johnson County Dental Society.

My practice philosophy is to provide patient-centered, predictable care. Every patient has a unique set of needs and goals, which should be considered when developing a treatment plan. By utilizing the latest technologies in dental imaging, digital treatment planning, regeneration and minimally invasive surgical techniques, I feel confident in the predictability of the treatments I am able to provide.

I really enjoy providing esthetic periodontal treatments, including gingival grafting and implant placement. At the same time, diagnosing and treating periodontal disease and allowing patients to keep and maintain their natural teeth is a very rewarding experience.

Han Duong, RDH is our office registered dental hygienist. Han has graduated from Chabot College Dental Hygiene Program. She has a license from State of California to practice as a hygienist, which allows her to anesthetize patients, perform cleaning, deep cleaning and apply chemicals to reduce inflammation and sensitivity of the teeth. She is CPR trained and adheres to HIPPA regulations.She is an excellent hygienist with a great skill. She loves her job and her patients. She is detailed oriented and delivers quality treatment for each patient. She educates our patients and helps them maintain their oral health through professional cleaning and our recare program. We are thrilled to have her as a great team member to deliver professional care to our patients.

Vida Jabbari, RDA is our financial coordinator. She has obtained a certificate in Dental Assisting Program from San Jose City College and is a registered dental assistant. She has x-ray and coronal polishing license and a certificate in radiology stately and infection control. She is CPR trained and adherers to HIPPA regulations.

Vida is always ready to answer your financial questions. She provides you different financial options to help you receive the dental care you need. She helps you maximize your dental benefits through your insurance company for the prescribed treatment. She submits your insurance claims with necessary documentation to ensure a timely and thorough reimbursement. We are proud to have her in our team and happy that she is there to help you with your financial needs and questions.

Teresa Pintois our front office assistant and the face and voice of our practice. She answers the phones and schedules patients and coordinates the treatment with other offices. She can provideyou withyour insurance benefit information. She speak English and Spanish. Teresa had got her Baclor's degree in socieilogy and minor in Political science from Cal State East Bay in Hayward in 2018.Teresa volunteered in Oakland International High Scholl in the city of Oakland helping migrants during school in classroom and in the afterschool in college.Teresa bagan working in private radiology clinic in San Matero as a patient Coordinator and then startedat Perio 4U as a treamtnet coorinator in 2021. She a always smiling and is happy to help our patients and doctors.

Lyn Paas, RDAis a registered dental assistant with x-ray and polishing license. She graduated from Everest College Dental Assisting program in 2012. She has a certificate in Radiology safety, Certificate in infection control. She is CPR trained and adheres to HIPPA regulations. She speaks English and Tagalog. She is our front office assistant at this point and answers the phones and schedules patients and coordinates the treatment with other offices. She coordinates our hygiene schedule as well and get insurance beenfits. She can assistthe doctor during different periodontal surgical and non-surgical procedures and makes sure the patients are comfortable and well taken care of during procedures.

Mahtab Baghai, RDAis a registered dental assistant with x-ray and polishing license. She has a certificate in Radiology safety, Certificate in infection control. She is CPR trained and adheres to HIPPA regulations. She also obtained a Bachelor of Biology from University of California in 2006. She is bilingual in English and Farsi. She assists the doctor during different periodontal surgical and non-surgical procedures and makes sure the patients are comfortable and well taken care of during procedures. She sterilizes the instruments with the best possible equipment and makes sure that the rooms get completely cleaned in between each patient. Your safety is her number one priority. She checks the efficiency of each one of our sterilizing equipment, through a regular weekly spore testing check by an outside company. She is our equipment maintenance coordinator and takes care of the instruments and equipment safety and efficiency. She is a nice, caring and a people person and we are proud to her in our group.

Fariba Yousefi, DA is a dedicated and caring dental assistant. She coordinates continuing education programs for the referring dentist offices and develops our marketing and educational plans. She has got a certificate in dental assisting program and x-ray license from San Jose City College in 2004. She has AA degree. She is CPR trained and adheres to HIPPA regulations. She speaks English and Farsi. She has been working with our office since 2004. She knows most of our referring doctors and their staff very well and has been the face of our practice during all these years. Fariba has worked with our practice for years in different capacities and has been a dedicated employee and we are happy to have her in our team.

Merry Oun, DA is dental assistant with x-ray and polishing license. She has a certificate in Radiology safety, Certificate in infection control. She is CPR trained and adheres to HIPPA regulations. She graduated from Yerba Buena High school in San Jose and is currently attending San Jose City College. She speaks English and Cambodian.
