Dr. Pham is proud to have provided excellent dental care for patients in Santa Clara and surrounding areas since year 2000. The minute you walk through the door, you know youve come to the right office. Everything from soothing water fountain to digital X-ray and intra-oral camera, is designed specifically with patients comfort, health, and convenience in mind.
Dr. Pham holds a B.S. in Biological Sciences and a Doctorate of Dental Surgery (D.D.S) from the University of the Pacific, School of Dentistry. She continually dedicated to study advanced, post doctoral dentistry with some of the most progressive clinical dentists in the nation.
Dr. Le, originally from San Jose, CA, received his DMD from the University of Nevada, Las Vegas, School of Dental Medicine. After practicing for a number of years in Las Vegas, he decided to move back to California to be closer to family. After meeting Dr. Annie Pham and her husband, Tim Dang, Dr. Le chose to practice here at Perfect Smile Dental because of a shared belief in providing state of the art, relationship-based, comprehensive care that is tailored to fit into the lives and budgets of his patients.
Dr. Le is currently a member of the American Dental Association, the Academy of General Dentistry, the California Dental Association, and the Santa Clara Dental Society. Committed to providing the best care possible for his patients, Dr. Le completes over thirty hours of continuing education per year, and has taken postdoctoral courses in prosthodontics, endodontics, oral surgery, and is a member of the Mid Peninsula Progressive Dental Study Club. He is in the office Monday through Friday, including every other Saturday.
Dr. Chou was born in Taiwan but grew up in Southern California. After graduating from the University of California, San Diego with a degree in Biochemistry and Cell Biology, and working in microbiology research, Dr. Chou went on to earn her Doctorate of Dental Surgery from the University of California, San Francisco. Dr. Chous passion is to provide high-quality dental care to her patients. She, in particular, enjoys working with children with whom she will quickly develop rapport and trust. Dr. Chou strives to create a comfortable, friendly and relaxing dental experience and environment for patients through understanding and meeting patients individualized needs.
During her spare time, Dr. Chou is also a passionate baker, always testing new recipes to perfect her baked goods. She loves to do crafts, go biking, hiking with her rescue dog Bonus, traveling around the world, and watching movies.
Dr. Leslie Toy sincerely welcomes you to the Perfect Smile Dental Care family! She is experienced in restorative and cosmetic dentistry and is dedicated to providing you with the highest level of personal and comprehensive care to meet all your dental needs and goals. With her gentle, thorough, and caring approach, she will make it more comfortable for you to receive the dental care you need. She patiently listens and discusses your concerns to make certain you have the information you need to make a well-informed decision about your dental health. She graduated from the highly regarded University of the Pacific Dugoni School of Dentistry in San Francisco with a Doctorate of Dental Surgery and went on to complete residency in Advanced General Dentistry with the Lutheran Medical Center.
Dr. Toy is a Bay Area native and loves the richly diverse community, culinary offerings, and natural landscape that characterizes this magnificent region. In her free time, Dr. Toy enjoys spending time with family and friends, playing tennis, hiking, and exploring the outdoors and all that the Bay Area offers.

I am so grateful to have discovered Perfect Smile Dental! The office on First Street is beautiful, everything is incredibly clean and the reception staff is always friendly and efficient. The office staff is knowledgeable and is great about following up with appointments and other details. The Hygienists and other staff have been very professional in all my experiences. I have received great care from Dr.Chou who is wonderful at explaining and providing expert dental care. I have also been seen by Dr.Pham who is so knowledgeable and helpful. She explains everything so well using photos, X-rays and models so that I feel fully informed. And finally, Brianna of the office staff has been particularly helpful; I have appreciated all the attention she provides me as a client. I highly recommend Perfect Smile Dental!

Had a crown installed and lived with pain for 2 years thinking it was sensitivity , finally went back, doc opted for root canal, however before he can finish I still can't chew on that teeth. Now I have to go to specialist to check if there is a root fracture!! If I had never opted for the crown maybe the root wouldn't have cracked! Living with pain, may have to extract a molar !!!!

Must Read TIP : this clinic has online emergency appointment option available, Do not book emergency appointment unless it's really an emergency, if you can hold minor issues, go for comprehensive exam instead of emergency exam.....otherwise you will end up claiming it on insurance or pay out of pocket which is not worth it. I met Dr. Allen,he is kind great, expert and explained my situation. I have another appointment scheduled will update my review soon.

Apart from the hygienists and doctors who are extremely knowledgeable and helpful at Perfect Smile, I am a big fan of Shannon! She goes above and beyond to schedule the appointments. And by that I mean, she would take special care to make sure your appointment is made as soon as something comes available. I had one recent incident where my appointment was 2 weeks later but because of the issue I was experiencing, I expressed over a call to her and asked if something can be done sooner. An hour later, I got a call from Shannon and she was able to get me in the very next day.

Great experience, every time working with Dr. Toy. I had tooth fillings that needed to be done, she was very supportive and reassuring during the whole process, frequently checking up on me to make sure I was doing okay. The staff in the office was also very friendly and easy to work with to get through the appointment.

Dr. Lei has been my dentist for about 6 years, and he is always so patient and caring, also brings good sense of humor during the procedure. As the dental assistant, Vivian is also very gentle and considerate every time we go through the cleaning process. Thank you both!

Visited twice during 2022. The environment is clean. Visited dr. Chou and dr. Le both are good to worth for 4-star reviewing here. However, the missing 1-star is for their assistants... Both have been force me to speak Chinese and making awkwardness . The more uncomfortable ones is the assistant has been yawning (I can see it from her mask) three times during her assistant work for dr Le when he was working in the mid-afternoon...I think she needs a double expresso before onboarding to show her professionalism. Receptionists overall are nice and helpful.

I've been coming here for a year now, but will no longer be coming back. The good: - Nice location. I live about 5 minutes away, so it was really convenient to drive there. - Allen Le is great! Really nice and knowledgable, and he was great to talk to besides cleaning and treating my teeth. The bad: - Getting treatment here costs quite a bit of money. Obviously, this is a high cost of living area, but the issue here is that they always try to force me and my wife into signing up for more services than we really need. When I came in for cleaning, Allen said my teeth were fine and I didn't need deep cleaning. So, we scheduled an appointment 6 months later and did another basic cleaning in Nov 2021. The issue though is that, when I came in for my appointment, they told me it was for cleaning, examination, and xray. This didn't make sense to me since I didn't recall ever talking about (or needing) another examination and xray, so I said no, did the cleaning, and scheduled another appointment for Feb 15, 2022. I then called the office today (Feb 4, 2022) and spoke to Trang, who said this appointment was for DEEP CLEANING, exam and xray. I told her this was a mistake; Allen told me my teeth were fine and didn't need deep cleaning. Instead, she yelled at me, said I needed deep cleaning, which requires additional cleaning every 3 months, and I had no option but to either come in for the appointment or reschedule for a different date. So I told her to cancel the appointment, and she hung up on me without saying thank you or sorry. I will be looking for a new dentist now and will never be coming back here.

I was seen by the Dentist at the Perfect Smile recently and from the beginning it was very pleasant experience. They are well organized, all reception staff are very friendly and helpful. What I really liked was transparency with what they do and how I'll be charged. Both dentists Dr. Le and Dr. Pham were professional and explained everything to me. Hygienists were very gentle and polite. Watching Netflix while you're having treatment was a nice bonus and distraction. I really recommend this dental office if you want team who cares about your dental experience.

Because going to the dentist is a must, let it be a good experience. Try Perfect Smile Dental Care in San Jose! I have.....hmmm....troublesome teeth, however, never fear walking in there. Not only are the front desk staff and assistants always pleasant, professional, knowledgeable, and great with explaining things, the same goes for those who actually work on your teeth as well! Specifically, Dr. Allen Le and Dental Hygienist, Ms. Diana, never fail to ensure I understand and am satisfied with all aspects of my care (and they're gentle too!). Moreover, the facility even has monitors at each chair with access to Netflix for patients while services are being performed. HUGE perk for me for a few of reasons: #1: Multitasking. I don't have much free time in my profession so this "tv head" is afforded the opportunity to catch up on her binge watching while simultaneously receiving quality dental care. #2: I don't care how cool one is with going to the dentist, NO ONE wants to hear/focus on the sound of scraping, drills, etc while sitting in that chair. I repeat, NO ONE. #3: It's just super awkward staring at your dentist's forehead while he/she is fixing all the damage you've done with cake. Hope you try Perfect Smile Dental Care and love them as much as I do!
