The original mission at FLCH was to serve our regions agricultural workers. In 2009 we became a Federally Qualified Health Center (FQHC) and expanded our services to include everyone in the Finger Lakes region. Today, patients of all incomes, ethnicities, and walks of life come to our health center locations. As an FQHC, Finger Lakes Community Health offers discounts for services, regardless of a patients insurance status, based on income and household size.

Advocates for a healthy community. We strive to bring a new standard of healthcare to our region through education, technology, and preventive care. Finger Lakes Community Health has been an active participant in keeping our community healthy during the pandemic. We conduct COVID-19 testing and hold vaccine events. From December 2020 to December 18, 2021, we have given 11,650 vaccine doses, 6,582 were fully vaccinated and of those, 2,700 are farmworkers.

They are experts in the fields of medical and dental care, reproductive health services, and behavioral health counseling. We also have multilingual staff that helps patients with non-medical issues such as referrals, follow-up, transportation, financial advocacy, and insurance enrollment.

Experts in coordinated care. Coordinated care ensures that all aspects of a patients treatment plan are organized through a primary care provider and delivered in accordance with the patients preferences. Our Geneva, Penn Yan, Ovid, Newark, Port Byron, and Sodus Community Health Centers* have been designated as Level 3 Certified Patient-Centered Medical Homes (PCMH) by the National Committee on Quality Assurance. This is the highest level of recognition for excellence in coordinated primary care.

Our multilingual staff works hard to make every visit a good experience for everyone. Their goal is to help our patients feel comfortable throughout every step of the healthcare process. Our staff consists of:

Medical and dental providers including primary care physicians, physician assistants, nurse practitioners, nurses, behavioral health counselors, dentists, and dental hygienist.We also have access to specialists, hospitals, and other medical centers.

The services requested will provide connectivity to each site as specified in the RFP and network diagram attached. The configuration must support QoS, specifically DSCP prioritization end to end for telehealth and secure reliable connections for electronic HIE

The NY Community Broadband Partnership (NYCBP) was formed as a part of the Federal Communications Commission (FCC) Healthcare Connect Program (HCP). The HCP promotes the creation of rural consortiums of healthcare organizations for the development of broadband infrastructure for healthcare providers in rural underserved communities. The consortium can be comprised of rural and urban healthcare partners and is designated rural if there is a 51% or greater representation of rural partners.

NYCBP welcomes rural and urban partners from the communities of New York State to the consortium. All eligible partners will get a 65% discount in their monthly cost for broadband services offered by an Internet Services Provider (ISP) chosen by the consortium. The partners will pay a small subscription fee to NYCBP for the administration and management of the consortium activities.

I had never been there before.They took care of my problem,I had an extraction,one of my molers.Great staff,the dentist is truly one of the best around,I am glad I found this office.

Never, ever answer the phone. I like the dentist and the office staff is great. Unfortunately if you call to make,cancel or reschedule, an appt,you cannot get through. There is a prompt to have them call you back. If you are cancelling,and can't get through or a call back,you are marked as missing an appt.Last time took two weeks to get a call back for a front tooth cracked in half. I was told I could get squeezed in on Dec 23rd as they are too busy. Especially for people that "miss" appointments. I did end up 'only 'waiting a total of three weeks for a front cracked tooth after much deliberation with Karen, who was rude and condescending at best. I was told that the Dentist "crams 20 patients a day into a 5 patient day schedule." Every time I do manage to get an appointment(called a "squeeze in" by Karen) I am told I will be waiting up to a few hours. When asked if I can get a call that day to let me know around what time to come instead of waiting, she said NO. So, it is a call center. You can't call to reschedule, cancel, make an appt...or see how the dentist is running..as not to wait a few hours. Then, being marked as a no show/missed appt, (which they have you sign a document stating you will be excused from practice for) they scold you and refuse to make you an appt when they finally call back.And if it was a "squeeze in" (which is every time)on an over booked day, then how is it a problem? This is unacceptable and clearly unprofessional...not to add,unethical. Because it is a clinic that accepts "state insurance" you are at their mercy. Looking into how to report this neglect/abuse on low income families and people who receive SSI /SSDI. I had work done last week on front tooth(one that wasn't cracked) which did not need fixing. It was drilled so thin, it cracked/broke the next day.Hopefully I can find another practice, but it seems anywhere accepting medicaid have similar reviews.

Dr.Phippen is above and beyond professional and compassionate .The assistant was fantastic as well . definitely highly recommend this office

When I moved to Penn Yan I had lots and lots of work needed to my teeth. I called and made an appointment and when they saw how much work needed to be done they got me in fast and started the process of fixing all my teeth. They are amazing people with great personality's. I would recommend them to others. Thanks for all the hard work on my teeth. Great job everyone.

When I moved to Penn Yan I had lots and lots of work needed to my teeth. I called and made an appointment and when they saw how much work needed to be done they got me in fast and started the process of fixing all my teeth. They are amazing people with great personality's. I would recommend them to others. Thanks for all the hard work on my teeth. Great job everyone.

My daughter is autistic. She had three teeth we knew were bad. They got us an appointment quickly, the dentist and hygienist were great with her! While most dentists would give a diagnosis and make one wait until the next appointment, they pulled them immediately and explained everything they were going to do and what they were doing along the way to help keep her at ease.

First time having a dentist in new york just moved in july from tenessee. when i first came in it was crampt and weird then i fonally got called back i was laying there waiting while they were playing around and not necassarily taking things seriously my first check up and they drilled my tooth stuck me 6 times with a needle which my tooth had a cavity i guess point is after my first appointment with them my tooth hurt even worse then when i came in in the first place wouldnt recommend it even if i had no money. They just made my tooth hurt even worse then it did

Closed before hand. Had a temporary cap fall off. Can't eat or drink anything and they haven't called me back. Some dentist.
