Over a period of time, the jawbone associated with missing teeth atrophies is reabsorbed. This often leaves a condition in which there is poor quality and quantity of bone suitable for placement of dental implants. In these situations, most patients are not candidates for placement of dental implants.

With bone grafting, we now have the opportunity to not only replace bone where it is missing, but also the ability to promote new bone growth in that location! This not only gives us the opportunity to place implants of proper length and width, it also gives us a chance to restore functionality and esthetic appearance.

Autogenous bone grafts, also known as autografts, are made from your own bone, taken from somewhere else in the body. The bone is typically harvested from the chin, jaw, lower leg bone, hip, or the skull. Autogenous bone grafts are advantageous in that the graft material is live bone, meaning it contains living cellular elements that enhance bone growth.

However, one downside to the autograft is that it requires a second procedure to harvest bone from elsewhere in the body. Depending on your condition, a second procedure may not be in your best interest.

Allogenic bone, or allograft, is dead bone harvested from a cadaver, then processed using a freeze-dry method to extract the water via a vacuum. Unlike autogenous bone, allogenic bone cannot produce new bone on its own. Rather, it serves as a framework or scaffold over which bone from the surrounding bony walls can grow to fill the defect or void.

Xenogenic bone is derived from non-living bone of another species, usually a cow. The bone is processed at very high temperatures to avoid the potential for immune rejection and contamination. Like allogenic grafts, xenogenic grafts serve as a framework for bone from the surrounding area to grow and fill the void.

Both allogenic and xenogenic bone grafting are advantageous in that they do not require a second procedure to harvest your own bone, as with autografts. However, because these options lack autografts bone-forming properties, bone regeneration may take longer than with autografts, with a less predictable outcome.

This product is processed allograft bone, containing collagen, proteins, and growth factors that are extracted from the allograft bone. It is available in the form of powder, putty, chips, or as a gel that can be injected through a syringe.

Graft composites consist of other bone graft materials and growth factors to achieve the benefits of a variety of substances. Some combinations may include: collagen/ceramic composite, which closely resembles the composition of natural bone, DBM combined with bone marrow cells, which aid in the growth of new bone, or a collagen/ceramic/autograft composite.

Synthetic materials also have the advantage of not requiring a second procedure to harvest bone, reducing risk and pain. Each bone grafting option has its own risks and benefits. Drs. Rothschild, Bevin or Smagalski will determine which type of bone graft material is right for you.

David Rothschild has been practicing oral surgery in the Berkshires and Southern Vermont since 1980. Prior to that time he was on the teaching staff of Montefiore Hospital and the Albert Einstein College of Medicine in New York. He is a Diplomat of the American Board of Oral-Maxillofacial Surgery and a member of the AAOMS.

Dr. Rothschild has published numerous articles and delivered many lectures nationally and internationally on the subject of dental/facial surgery. In 1978 he served as visiting professor at the Tri-Service General Hospital in Taiwan Republic of China where he taught residents new techniques of oral maxillofacial surgery.

When not serving the community in all aspects of oral surgery Dr. Rothschild enjoys Platform Tennis in the winter, Tennis and Golf in the warmer months. He also has a avid interest in politics and current events. Dr Rothschild lives with his family in Williamstown.

Christopher Bevin grew up in Storrs, Connecticut. He attended Middlebury College in Vermont graduating Phi Beta Kappa and Magna Cum Laude earning a B.A. in Physics with a minor in Spanish. He was an all New England Small College Athletic Conference (NESCAC) high jumper for the Middlebury Track and Field Team his senior year. After college, he worked as an acoustics engineer at Electric Boat in Groton, Connecticut ensuring that U.S. Navy submarines were appropriately quiet to elude detection by foreign countries.

Dr. Bevins desire to work with his hands and interest in health-care led to a dental degree from the University of Connecticut School of Dental Medicine in 2001 and ultimately to complete a residency program in Oral and Maxillofacial Surgery at the Mayo Clinic Graduate School of Medicine in Rochester, Minnesota in 2007. During this time, he received a medical degree from the Mayo Clinic College of Medicine in 2005. He was also blessed with a wonderful wife and three children.

The medical and surgical training at the Mayo Clinic provides him with the necessary expertise to treat a wide range of head and neck diseases and conditions in addition to traditional dentoalveolar pathology. His special areas of interest include dental implant rehabilitation, bone graft reconstruction, and orthognathic surgery. He is on the surgical staff at the Southwestern Vermont Medical Center, Berkshire Medical Center and Rutland Regional Hospital providing emergency services to these communities along with other partners in the practice.

For the past several years he has served as the President of the Vermont Society of Oral and Maxillofacial Surgeons and has been a fellow of the International Team for Implantology earning recognition as one of the most successful clinicians placing dental implants.

Dr. Bevin, his wife, and three children live in Bennington close to their extended family and friends. He is an avid outdoor enthusiast and loves to hike, ski, play tennis and enjoys raising his family in this picturesque New England setting.

A huge thank you to Dr. Rothschild and your staff. My husband, Dennis Golinsky, had his oral surgery yesterday, March 1st. We were treated with nothing but kindness and respect from the time we first walked in for his initial visit to yesterday when we left. My husband surgery was quicker than I expected. He's doing great. Thank you Dr Rothschild, Nora and Kerri.

I would like to specifically give a shout out to Kiara who provided compassionate, thoughtful, and thorough care both before and after the procedure. My first experience in North Adams for my bottom wisdom teeth was terrible BUT this second time at the Pittsfield office to get my uppers removed with Dr. Holmgren was amazing. The staff was so kind and Dr. Holmgren sensed how anxious I was and made sure I was as comfortable as possible. The office kept calling to ask how I was doing and I am having a swift recovery so far. Sad that Dr. Holmgren is leaving the practice :(

Waited over 2 months for an appointment. Gave them all my insurance information when I scheduled. Made sure it was all set. The day before my appointment they call and inform me my insurance doesn't cover them and I have to pay up to $3000 to have a tooth removed.

Dr Smagalski was amazing hands down and his nurses were really nice and kind. He explains everything really. When you go for your pre op visit he explains everything really well and he is so kind and patient with you. When I went for my wisdom teeth surgery yesterday his nurses were really nice and kind and the were really good at answering my questions and explaining what was going to happen each step of the way and why Dr Smagalski came in he said good morning how are you and do you have any questions. I didn't have any questions and then he explained everything to me as he was putting me under for surgery and then after words when I got woken up by one of the nurses it was a very smooth transition to the recovery area and then he came and talked to my mom and I but I don't really remember that. The best part of it all was I got to take a picture with him because I wanted to. He is a very good surgeon and doctor and I would recommend him to anyone who needs oral surgery.

Dr Smagalski was amazing hands down and his nurses were really nice and kind. He explains everything really. My surgeon DR.Bevin was amazing, I was very scared to get oral surgery for that I had a bad experience many years ago with oral surgery . I've had a lot of skeletal surgeries and never had a problem but my mouth is a different story. If your nervous about pain, he's your guy. I only needed Tylenol after surgery. Thanks Again to Dr Bevin and his wonderful assistants.

it is an extraction of a tooth by referral only. How big did you honestly expect the scope to be for a procedure that takes less than a minute. itty bitty scope, right? You know what I am saying? Be realistic patients. God!

Oral and Dental Implant surgery are UTTER MONSTERS. They ripped out 9 teeth in under 45 minutes, making no effort to minimize damage, and brutalized my gums and jaw in doing so. And when I FULLY AWOKE from anesthesia halfway through, they didn't redo sedation. Instead, they had two people HOLD ME DOWN AND YELL AT ME TO STOP SCREAMING while they finished ripping out the remaining four. I'm home now, still shaking and crying.

I had a work-related accident that resulted in a traumatic head and face injury a year ago. I had to get a tooth removed, bone graft, implant and multiple check-ins throughout the whole process. Dr. Smagalski is incredibly professional, respectful, and thorough in his work. I knew I was in great hands the second I entered the building. I healed beautifully thanks to him, and he made what could have been an absolutely terrible experience so much better. ****ALSO, it should be noted that the amazing staff working there are some of the most patient and amazing people you will find. I struggled through worker's compensation denying every bill I had and they worked with me and went above and beyond for me to get everything figured out. Incredibly grateful for their work and services.I would recommend Pittsfield ODI to anyone and everyone.

The staff is wonderful. Both of my experiences here I didn't have to wait too long and Dr. Rothschild had explained everything right out and was straight forward with what could go wrong during the surgery. Not only was Dr. Rothschild right to the point but he did an excellent job with my surgery along with the assistant(s) that helped him out. Thank you all for working with me to make the next step even better of being able to have my smile back again that I haven't had for years.
