Northwest Colorado Health supports community wellness through a range of health programs and services helping residents of all ages, income levels and cultures liveas healthy aspossible. Northwest Colorado Health (formerly Northwest Colorado Visiting Nurse Association) was established in 1964 as a Home Health and Public Health agency in Routt and Moffat counties. We have grown significantly over the decades, expanding our services to include primary care at Community Health Centers in Steamboat Springs and Craig, a wide range ofPrevention Servicesin Routt, Moffat, Rio Blanco and Jackson counties and Home Health and Hospice services in Routt andMoffatcounties. In addition to our Community Health Centers, we operate Dental Clinics in Craig and Oak Creek,The Haven Assisted Living Facility and Community Centerin Hayden and the Rollingstone Respite House in Steamboat Springs.

Regardless of their income, race, age, gender, health status, or insurance coverage, everyone deserves the chance to be healthy and well. No one should be denied the opportunity to see a doctor, receive healthcare, or participate in preventive health programs.

While we do treat illness and injury, our goal is for every member of our community to be empowered with the information, tools, and resources they need to live a healthy life.

The Rollingstone Respite Houseprovides comfortable rooms and a warm setting for Hospice patients who are not able to live their final days in their homes. A kitchen, dining room and living room area are available to families visiting their loved ones. The Rollingstone Respite House also hosts bereavement support activities and Hospice volunteer events and training.
