We provide a variety of services based on the needs of the communities that we serve, including primary medical, dental, and behavioral health. All people are welcome to receive care regardless of their insurance status and ability to pay. For those that qualify we have a sliding fee scale that is based on income.

We are deeply involved in our communities providing dental and behavioral health in schools, taking dental care to nursing homes, and working with WIC Women, Infants, and Children. Since our opening in 2008, we aim to fill the gaps in healthcare and to fulfill our vision of healthy, prosperous, engaged communities where everyone thrives.

Federally Qualified Health Centers have advocates across all political parties and halls of government. Its an easy thing to get behind. One of our jobs as a health center is to stay in touch with those who represent us in government. Policies and plans can and do effect peoples health so when we need to share information that can influence and advocate for our communities and our patients we have mechanisms in place to do just that.

The Board of Directors is a governing body of a non-profit organization and an FQHC. Individuals who sit on a Board of Directors oversee our activities, and they meet periodically throughout the year to discuss and vote on the affairs of our organization, the mission, strategy, and goals.

We are always seeking applications for patients who are interested in becoming a board member. If youre interested in becoming a board member and are a patient, please fill out the Board Self-Nomination Form and email it to marketing@nlccwi.org.

It all started when a group of citizens in northern Wisconsin got together because they saw gaps in the healthcare resources in their community. They dug in and did the work to design a solution to address those needs. They submitted a grant application for federal funding, and they got it.

In 2008, a building that had previously been a home, a winery, a coffee shop, and who knows what else became The Lakes Community Health Center in Iron River, Wisconsin. They also worked with the city of Ashland to open up a dental clinic, but that was only the beginning.

In 2013, The Lakes had the opportunity to grow and help to serve more communities. They merged with North Woods Community Health Center. North Woods, which was created in 1976 by a similar community group, had clinic locations in Hayward and Minong. The new organization changed its name to NorthLakes Community Clinic.

NorthLakes patient-majority board believes that there are many healthcare facilities in our service area that could benefit from joining an FQHC. The following organizations have also joined NorthLakes:

In addition to our clinical services, we also have community outreach programs that are active in 20 counties. Our community outreach programs include: School-Based Behavioral Health Active in 22 school districts across 11 counties.

Absolutely horrible experience. After calling for 3 weeks to get an initial appt I wait 3 months for 1st appt where they take images only. Then next appt 2 months later, was canceled by the provider to be rescheduled. I then go to a rescheduled appt a month later for a cavity fill and they grind away jumping the tool around in my mouth, ouch!. Supposed to have another appt the next week to finish what they started. Instead I get a call they need to reschedule. How can providers cancel so much on their patients? Oh wait the regional manager called, gave excuses and staff shortages. If there was shortage why was appt scheduled for that date to begin with? I would avoid this clinic like covid or the plague.

never answer phone calls or return them and it's taken them weeks to send a referral to an orthodontist so i can get my wisdom teeth removed. will not ever be returning here

70 minute round trip drive for me keep in mind. In August we found that I needed a root canal. Got the debridement and temporary filling (almost didn't happen due to some confusing instructions given by Dr Malone). Scheduled six months out for rc. When I arrived I was sitting in the chair for 20 mins before Dr Malone comes in and says that they don't have the equipment. Told they'd call when they do. Ended up scheduling for 3 weeks later. The day prior I get a message that they will have to reschedule it for earlier in the day. End up sleeping in due to the plan changes. Told by Amy that I can't schedule and I'll just be on cancellation list. Called later in the week and suddenly policy changed and I was able to schedule an appt and was told that I'd be waiting a long time on the list for a 75 minute appt on the cancellation list because 2 consecutive people would have to cancel on the same day and lunch breaks etc. Told late June is the earliest appt. At this point I start bluntly stating the facts and how horribly this whole thing is being managed and that I need to get in sooner than that so that my tooth doesn't shatter. She accuses me of being very rude to her and hangs up(wife was present on speaker and has assured me that while I was being blunt what I was saying was absolutely professional, I did not threaten, did not swear once). Later the manager calls me and lectures me about how incredibly rude I was and that I need to act more professionally while I'm there, basically I was being a total jerk. Manager also implies that a provider was also witnessing the conversation and attested to my rudeness. The only plausible explanations for this would be if I happened to be on speaker phone which is a violation of privacy laws, or the call was recorded without any prior indication. So this place is either committing HIPAA violations or this was a falsehood and what likely occurred is that Amy misrepresented my bluntness as yelling to a doctor. 1 star for screwing up so many times that I'm waiting 11 months for one root canal. Also because a business shouldn't hire people who are so sensitive that they basically slander clients to the rest of the staff.

i am giving a negative review- - 1 star. infection for 3 weeks had to go to emergenct care to get a prescription for anitbotics- ( they give you less then a week)- went here, they should have renewd my prescription knowing the infection was not gone yet- instead, they pulled the tooth and sent me on my way. out of antibotics and they will do nothing about it. more bs from overpaid heatlh care workers.

I called for an emergency visit and got in the next day. The dentist pulled two teeth and was awesome with everything. He made sure I was comfortable and made sure I was okay. Recommend going here!

I have had some issues with my teeth and just recently had to go and have a tooth pulled the dentist i had (Dr.Koch) was amazing I didn't feel a thing and he was fast and efficient and explained everything thoroughly. I recommended my mom and my daughter for their issues and they got them in immediately. I would recommend this dentist to anyone. I was very impressed!!!

I love this place. They are a honest and hard working dentist group who do excellent work and only what is needed at a very fair price. I am highly satisfied with this Dentist Office and friendly staff. I have been to dentists in Green Bay, but it is worth the drive to go here instead.

I like the dental assistant's, but dentists themselves could stand to get a personality. They are ok office staff but if you have a problem, don't expect much help from the front desk all the way up to the billing department
