Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.

Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Lori joined North Gwinnett Dental Care in 2009 as one of our patient care coordinators. Lori previously worked at a dermatology office, an orthopedic office and was the director of a preschool. She was born in Illinois, moved and married her husband, Steve, in 1987. They have two daughters. Lindsay, who is a Radiologic Technician and Abby, who is a Prosthetics and Orthodic Assistant. Lori has been apart of North Gwinnett Dental Care for over 10 years and strives to make patients feel right at home.

Damaris joined North Gwinnett Dental Care in 2015. She has been a hygienist since 2009 and works at North Gwinnett Dental Care on Mondays, Tuesdays and Thursdays. Damaris was raised in Washington state and moved to Georgia in 2005. She was a Dental Assistant before going to school for Dental Hygiene. Damaris graduated from Central Georgia Tech with an Associates in Dental Hygiene. She loves dentistry and hopes to one day further her education in periodontics. She and her husband, Nathan have five children; Joseph, Jace, Noelle, Sephora and Titus.

Anita has been an expanded function dental assistant for 11 yrs, and is a Atlanta, Georgia native. She is married to Dexter and has a son (Cameron). She has additional experience as a Endodontic assistant and has a certification in oral sedation treatment. Her outside interest are traveling, cooking and volunteering at a local food bank. On her days off she enjoys cheering on her favorite sport teams ( The Atlanta Falcons & The Hawks) and spending time with family and friends.

Loretta joined our wonderful North Gwinnett Dental Care team in June 2018 as an Expanded function dental assistant. Loretta has been a dental assistant since 1996 and also received her radiography certification. Loretta has a 4 children and 5 grand-children and loves spending time with her twin grand-babies who she absolutely adores. In her spare time she enjoys cooking, keeping a clean house and travelling.

Lejla Joined our NGDC team in September 2018 as an Expanded Function Dental Assistant. She is a graduate of Gwinnett Technical College and has been assisting for 11 years. Lejla is married to her high school sweetheart, Ermin and they have two children together; Eldar and Emma. In her spare time she enjoys traveling, gardening and spending time with her family and friends.

Carola joined North Gwinnett Dental Care in September 2020 as our childcare supervisor. She previously worked in the medical field for over 30 years. Carola has 3 sons of her own; Adrien, Antony and Andre' and 3 grandchildren, Ansley, Ashton and Atlas. She is well-rounded when it comes to caring for her patients children of all ages. In her spare time she loves to garden, travel and spend time with family.

We are pleased to offer a dental wellness care plan for patients that are not fortunate enough to have dental insurance through their employer. We have noticed that patients without dental insurance come in less frequently. This usually means when they do come in for a visit, we tend to find bigger problems that are more difficult to manage than if we had been able to catch them sooner. Knowing that traditional dental insurance premiums are expensive, and that those insurance benefits are extremely limited, we have created a comprehensive and customized membership program that will cover 100% of all of your preventive care in our practice: including cleanings, x-rays and exams. Two dental cleanings for a healthy mouth (usually about six months apart) a deep cleaning or scaling and root planning is different and will be charged at 85% of normal fee (with the 15% discount). Since periodontal patients are usually seen for checkups every 3 or 4 months they will still get the two free cleanings per year, but will need to do the deep cleaning prior.
