With your consent, Nebraska Oral & Facial Surgery is permitted by federal privacy laws to make uses and disclosures of your health information for purposes of treatment, payment, and health care operations. Protected health information is the information we create and obtain in providing our services to you. Such information may include documenting your symptoms, examination and test results, diagnoses, treatment, and applying for future care and treatment. A nurse obtains treatment information about you and records it in a health record. During the course of your treatment, the doctor determines a need to consult with another specialist in the area. The doctor will share the information with such specialist and obtain input.

We submit a request for payment to your health insurance company. The health insurance company requests information from us regarding medical care given. We will provide information to them about you and the care given.

We obtain services from our insurers or other business associates such as quality assessment, quality improvement, outcome evaluation, protocol and clinical guideline development, training programs, credentialing, medical review, legal services, and insurance. We will share information about you with such insurers or other business associates as necessary to obtain these services.

Request a restriction on certain uses and disclosures of your health information by delivering the request in writing to our office. We are not required to grant the request, but we will try to reasonably comply with any request;

Obtain an accounting of disclosures of your health information as required to be maintained by law by delivering a written request to our office. An accounting will not include internal uses of information for treatment, payment, or operations, disclosures made to you or made at your request, or disclosures made to family members or friends in the course of providing care;

A true husker at heart, Dr. Rallis attended the University of Nebraska Medical Center College of Dentistry, graduating at the top of his class. He founded Rallis Oral & Facial Surgery in 2010, after completing medical school and a six-year residency at the world-renownedMayo Clinic. He understands that as a patient, you may be nervous to visit the oral surgeon. To ease those nerves, Dr. Cleverly believes its important to take time with each patient. He will answer all of your questions, explain what to expect during your visit, and reassure you on any worries that you may be experiencing.

Lincoln, NE became home to Dr. Cleverly while attending the University of Nebraska Medical Center College of Dentistry. After dental school, he moved to Wisconsin to complete a comprehensive surgical residency in Oral & Maxillofacial Surgery at Gundersen Lutheran, joining the staff there after. When the opportunity to practice with Nebraska Oral & Facial Surgery presented itself, Dr. Cleverly knew it was time to move his family of eight back home.

With over 10 years of experience in the field and six children of his own, he recognizes the importance of empowering patients of all ages, especially children, with the knowledge they need to feel confident and comfortable during their visit.

Dr. Kevin Rieck is a highly-regarded surgeon, driven by Nebraska Oral & Facial Surgerys dedication and team-approach to patient care. To ensure the team performs at a high and safe ability, you will regularly see him performing extra training for staff members. Dr. Rieck is truly dedicated to the needs of each and every patient. Whether he is helping patients by removing a wisdom tooth or replacing a missing tooth with a dental implant to restore a smile, he finds it extremely gratifying when patients leave telling friends and family about their positive experience.

With over 20 years of practice, Dr. Rieck is a well-versed oral surgeon. He called Chicago home while attending the University of Illinois at Chicago College of Dentistry. After dental school, he moved to Minnesota where he completed medical school, as well as, a six-year residency at the acclaimed Mayo Clinic. Before joining the team at Nebraska Oral & Facial Surgery, he spent 12 years as an attending surgeon at the Mayo Clinic, where he pioneered the technique of computerized virtual surgical planning for orthognathic surgery.
