We accept most insurances and participate with many PPO plans. We proudly accept Forward Health patients of all ages and all counties. We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are cash, check, Visa, MaterCard, Discover, American Express and CareCredit.
Dr. Johnson has lived his entire life in Illinois. Growing up in Woodridge Illinois, a suburb of Chicago, he played high school basketball, wrestling, cross country and track for Downers Grove North. In addition to his school activities he was an active member of the Boy Scouts until going away to college. He attended Western Illinois University where he received a bachelor's degree in biology. While at Western he continued to be an avid athlete and was on the track team, intramural basketball, softball, tennis, and bowling league. He also served as President of the Canterbury Fellowship for three years. The campus ministry was supported by the local Episcopal Church. Upon completion of Western Dr. Johnson was accepted into the Dental School at Southern Illinois University. He graduated from SIU in 2000.
Attending continuing education courses annually, he is also a member of the Local, State and American Dental Associations. 'I am constantly striving to learn new and better ways to become proficient in general dentistry. There are always new methods and technologies being developed that will benefit those seeking dental care.'
When Dr. Johnson is not at work he enjoys spending time with his family, working on garden projects, golfing, reading, exploring nature and visiting his favorite childhood destination, Michigan. Dr. Johnson and his wife LaNae have two children Kennedy and Clayton. 'It is a special gift to have these two children, read to them, explore with them, and watch them develop and grow.'
Alicia enjoys working in the dental field and loves helping others. She and her family recently moved to Wisconsin from Illinois, she was born and raised in Indiana. She has two beautiful children Mya and Julien. Outside of work she enjoys working in her gardens, watching birds, and doing crafts. Most of all she loves spending time with her family and friends.
Wanda has been a hygienist for 16 years. 'I enjoy helping patients improve their oral health through education.' When not at work Wanda loves spending time with her husband Richard, daughter Sommer, son Lucas, and their 12 year old dog Jersey. Wanda enjoys spending her summers riding her motorcycle either around town or on long haul trips. She also enjoys smoking food and entertaining all her friends and family.
Katie enjoys fishing, hiking and doing crafts. She loves to knit but never finishes anything she knits. She enjoys spending time with her Husband Jax, their dog Skittle, and her many family and friends. Katie speaks Hmong and English.
Maikia was born and raised in Eau Claire, Wisconsin. She graduated with a sociology major from the University of Eau Claire-Wisconsin. She looks forward to working with a diverse community of all ages, her team, and learning more about the dental field through the daily challenges and responsibilities. She writes and speaks fluent Hmong allowing her translate and assist with the families who may be seeking an interpreter during their visit. In her free time she enjoys fishing, hiking, camping and traveling with her husband and daughter. She has a passion for creativity and photography.
