Since 1978, as others closed their doors to those without private health insurance, we have continued to open ours. From a singleclinic we have now expanded into a network of health care facilities along the I-70 corridor from Parachute to Avon, and along the Roaring Fork Valley to Basalt.
As a nonprofit, community-led, Federally Qualified Health Center system, we are dedicated to providing comprehensive, integrated health care for all. We firmly believe health care is a basic human right, not a privilege.
Our system integrates medical, dental and behavioral care a necessary measure for the vulnerable, who often show up in our clinics with medical problems related to poor teeth, substance abuse, trauma, depression, anxiety and other complicated combinations.
We exceed national clinical outcome goals for childhood immunizations; hypertension control; tobacco screening and cessation; healthy weight screening, nutrition and activity counseling; depression screening and follow-up; early entry into prenatal care; and healthy birth weight babies. We were also recently rated Colorados top performing community health system for diabetes control.
Mountain Family Health Centers is a 501(c)(3) nonprofit Federally Qualified Health Center dedicated to providing high quality, integrated primary medical, behavioral, and dental health care in the communities we serve, with special consideration for the medically underserved, regardless of ability to pay. We proudly serve more than 21,000 patients in Garfield, Eagle, Pitkin, and Rio Blanco counties.
As an FQHC, Mountain Family Health Centers is eligible for Federal Torts Claim Act (FTCA) medical malpractice liability protection. As such, Mountain Family Health Centers is a Health Center Program grantee, under 42 U.S.C. 254b, and a deemed Public Health Service employee, under 42 U.S.C. 233(g)-(n).

Alice Kaniff, DDS, DABDSM owned a private practice, in the Suburbs of Chicago, where she was born and raised. Dr. Kaniff was recruited, from her practice, by the United States Air Force. She served active duty, as Lieutenant Colonel, at the USAFA Cadet Dental Clinic, in Colorado Springs, CO.
Dr. Kaniff moved to the Roaring Fork Valley in 2005, as a comprehensive dentist for Basalt, Aspen and Steamboat Springs. She brings her expertise in TMD/ oral facial pain, attention to periodontal disease and its prevention, removable and fixed solutions to tooth loss. In addition, Dr. Kaniff is a treatment specialist in oral appliances therapy for sleep apnea and snoring, as the life saving alternative to CPAP. As a past provider, in Long Term Care facilities, she enhanced and facilitated, the dental care/ preventive needs of the residents, in partnership with the medical staff, social workers and families.
Dr. Kaniff obtained her Doctor of Dental Surgery degree from Loyola University of Chicago Dental School. Her advanced education includes USAF, Dawson Center of Studies, and Northwestern University. She has specialty certifications, as a Diplomat with American Board of Dental Sleep Medicine.
Our Rifle Clinic Medical Director, Dr. Percyenjoysthe broad spectrum of challenges that family medicine offers, although he has particular interests in pediatrics and preventative medicine. Although he grew up in Cortez, Colorado and completed his medical residency in Salt Lake City, it was a rotation at Mountain Family that opened his eyes to the attractions of the Colorado and Roaring Fork Valleys, and hes been here ever since. He received his MD from the University of Colorado School of Medicine, after earning a Bachelor of Science in biology, with honors, from Stanford University. He speaks moderate Spanish and is working on getting better. When not at work, you can find him with his wife (a pediatrician herself) and three kids, exploring the outdoors on foot, bike or skis, or traveling the world.
Dr. Schreiner brings three decades of experience in private practice and institutional settings, including extensive experience with adolescents and the U.S. military, counseling servicemen and their families around the world. Previously he was also a school counselor and football and baseball coach. Dr. Schreiner holds a PhD in Marriage and Family Therapy from Texas Tech University. A native Coloradan from Boulder, he chose life on the Western Slope for its slower pace and outdoor opportunities. Dr. Schreiner and his wife enjoy skiing, golf, scuba and travel.
Dr. Glasss interest in dentistry for underserved areas began when he saw the value that a nonprofit clinic brought to remote populationsin his home state of Virginia. Ever since, he has been passionate about dentistryand community service. Dr. Glassand his wife came west after he completed dental school at Virginia Commonwealth University. Along with their dog Oso, they enjoy being in the mountains, climbing, skiing, and biking.
Dr. Kent joined Mountain Family in 2019. Previously he was an Assistant Professor of Family Medicine in the Air Force, where he taught for nine years. Upon completion of residency training in2009, Dr. Kent completed additional fellowship training in obstetrics and then began teaching at the University of Nebraska Offutt Air Force Base Family Medicine program. Four years later, Dr. Kent then joined the faculty of the Travis Air Force Base Family Medicine Residency program near Fairfield, California, where he completed the remaining five years of his Air Force service. Dr. Kent has interests in womens health and obstetrics, in-office procedures, as well as family and community health. He is board certified in Family Medicine by the American Academy of Family Physicians and is excited to be a part of the Mountain Family team.
Deb joined Mountain Family in 2020. She practiced for nearly a decade with the Poudre Valley Family Medicine Residency program in Fort Collins, Colorado. Since 2007, she has been engaged in primary care practice transformation activities and subsequent population health and value-based payment innovation efforts at the state and national levels. Shes held a variety of clinical, quality, and administrative leadership roles with both commercial and government payers and managed care organizations. She also serves as an adjunct faculty member with Colorado Christian Universitys Health Care Administration program and participates in sub-committee work of Colorado HCPFs Accountable Care Collaborative (ACC) Program Improvement Advisory Committee (PIAC). As the Director of Interdisciplinary Care, she oversees many steps and systems Mountain Family has in place to ensure we provide excellent, high quality medical, dental, and behavioral health care for our patients. Deb earned her Bachelors in Nursing from Concordia University, River Forest, IL, in 1990. She holds a Masters in Nursing (Family Nurse Practitioner-Rural Health track) from the University of Wyoming in Laramie, WY, and an MBA with a focus on Health Care Administration, from Colorado Christian University, Lakewood, CO. She enjoys reading, puttering in the kitchen, entertaining with her spouse and son, and anything outdoors in the Colorado mountains when shes not at work.
Emily Borkovec, M.M.S, PA-C started with Mountain Family Health Centers in 2017. She was born and raised in Iowa and received her undergraduate degree from Minnesota State University. Emily attended the Physician Assistant program at Red Rocks in Denver, earning her Masters degree with Saint Francis University in Pennsylvania. She is also a graduate of the Roaring Fork Leadership Academy, Class of 2021. Emily has devoted her entire career thus far to community health; she previously worked at two other Federally Qualified Health Centers in Colorado. She stepped up to be the Site Medical Director of the Basalt clinic in 2021. Emily is passionate about the underserved having access to quality,affordable health care. In her free time, she enjoys hiking, backpacking, mountain biking, cooking, and spending time with family and friends.
SteveLeazerjoined Mountain Family in 2019 after 6 years with HCA Healthcares Continental Division where he oversaw financial decision support functions. Prior to HCA HealthcareSteveworked for University Physicians, Inc. and Huron Consulting Group. As Director of Finance,Steveleads financial record keeping, reporting and budgeting for Mountain Family.Steveearned his Bachelor of Science degree in Industrial and Systems Engineering from the University of Wisconsin Madison and is currently working on a Master of Finance degree. Outside of workSteveenjoys spending time with his wife and two sons as they hike, camp, fish and ski their way through Colorado.
Marija joined Mountain Family in 2019 after 5.5 years with the Colorado Community Health Network (CCHN), where she led the organizations work on value-based payment and new FQHC payment models. Prior to CCHN, Marija worked at the National Network for Oral Health Access and United Way of Larimer County. Marija earned her Bachelor of Arts in Theology from Franciscan University of Steubenville in 2003 and her Master of Social Work from the University of Denver in 2008. When shes not at work, Marija can be found knitting, biking and baking.
Working for Mountain Family over the years, Wendy has found that the Revenue Cycle world has ongoing challenges Front desk collections and customer service; Billing teams that are efficient and stream-lined; mentoring young men and women and staying positive through any and all changes.
Jan Jennings joined the Executive Team at Mountain Family in 2020. As Development Director, Jan is responsible for all aspects of development, fundraising, public communications, and public relations. After graduating with a BA in Modern Thought and Literature from Stanford University, she spent 10 years working in leadership planned giving positions for national environmental groups in Washington, DC such as National Audubon Society, The Ocean Conservancy, and World Wildlife Fund. Afterwards she worked in Boston for the Harvard-affiliated Joslin Diabetes Center before starting her own consulting company, Viking Consulting Services. Her clients included the American Air Museum in Britain in Duxford, Ecole Centrale in Paris, and the Friends of the Louvre when she was based in Paris. Returning to her roots in Aspen and the Roaring Fork Valley where she was raised, she worked for The Aspen Institute and consulted for the Aspen Center for Environmental Studies. Jan enjoys hiking and skiing with her husband and daughter, writing sonnets, and copper enameling.
Dana joined Mountain Family in late 2021 after two years serving as the Director of Human Resources for the City of Glenwood Springs. She was also the Benefits Administrator/Business Partner for Garfield County for three years. Dana began her HR career at Mountain Valley Developmental Services where she served as HR Director for seven years. She is passionate about serving organizations and their people and is delighted to return to the non-profit, healthcare arena. Dana earned a Bachelor of Arts from Wake Forest University in Religious Studies and Communication and a Master of Divinity from Vanderbilt University with a focus on Ethics and Feminist Studies. Drawn to people and organizations, Dana received a Master Certificate in Human Resources and Organizational Leadership from Villanova University in 2015. When shes not working, you can usually find Dana outside running, skiing or biking with her young son and husband, or teaching ballet to adults.
