The Cincinnati Health Network, Inc. (CHN) serves individuals experiencing homelessness or living with HIV/AIDS in the Tri-State area, as it has since 1986. We offer quality medical, behavioral and case management services. As a Federally Qualified Health Center (FQHC), CHN has earned the distinguished Patient Centered Medical Home (PCMH) Level 3 seal, the highest level possible, proving we go the extra mile to ensure patients receive outstanding care. CHN runs several integrated medical and behavioral care clinics in the area, making it one of the few local organizations that provide this type of integrated care.

The CHN remains the only Federally Qualified Health Center in greater Cincinnati dedicated exclusively to meeting the complex health care needs of individuals and families experiencing homelessness. It has evolved from limited services on a makeshift bread truck in 1988, to a well-coordinated, fully integrated patient centered system of care serving 10,000 individuals experiencing homelessness annually.

In addition, CHN also manages the Greater Cincinnati HIV Collaborative (GCHC), an existing network of health providers serving a patient base of over 2,100. The GCHC provides a continuum of outpatient HIV early intervention, linkage to quality primary medical and mental health care, and supportive services to individuals living with HIV throughout our tristate area.

CHN partners with other area organizations to respond to the ever-changing needs of our patient population. This improves access to comprehensive, coordinated quality care while maximizing existing resources.
