  • Sliding Fee Scale:We offer medical, dental, and behavioral health services on a sliding fee scale for our patients who earn up to 200% of the Federal Poverty Guidelines. We serve patients of all ages and income levels, both insured and uninsured.
  • FHLC does not discriminate in the provision of services to an individual (i) because the individual is unable to pay; (ii) because payment for those services would be made under Medicare, Medicaid, or the Childrens Health Insurance Program (CHIP); or (iii) based upon the individuals race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.

    In the 1960s, a group of health professionals and migrant advocates in cooperation with church leaders got together and began providing free health services to migrant farmworkers in some of the church halls. This ultimately resulted in La Clinica de los Campesinos being formally organized in 1972 as a non-profit Wisconsin corporation. That same year, La Clinica secured federal funding and began delivering primary health care services in Wild Rose, Wisconsin under the guidelines of the Migrant Health Center Program.

    In the mid-1980s, La Clinica initiated its outreach program providing migrant mobile health services and a voucher program. Under a formal agreement, the voucher program pays participating health care providers for services they provide to migrant and seasonal farmworkers who work and live too far from La Clinica's health center in Waushara County.

    Today, Family Health La Clinica continues to serve migrant and seasonal farmworkers across the state of Wisconsin via its state-of-the-art Mobile Health Center. The clinic's operations have expanded from its main, full-service site in Wautoma to additional dental centers in Mauston, Beaver Dam, and Stevens Point. Family Health also has a Behavioral Health & Substance Recovery Center in Friendship.

    Family Health La Clinica was one of two sites nationally that pioneered the process of retrospective equivalency into the National Health Disparities Collaborative. Due to hard work and continuous quality improvement with diabetes management programs, the health center was admitted into Phase 2 of the HDC.

    FHLC's CEO, Laura Waldvogel, has been appointed to the Wisconsin Minority Health Council which advises the Department of Health and Family Services and other state governmental units on minority health issues.

    In April 2020, FHLC received preliminary notice of funding throughAdvancing a Healthier Wisconsin Endowment of the Medical College of Wisconsin to be used over the next 6 months to support our efforts to develop a cohesive response to prevent and/or minimize the risk of transmission of COVID-19 among Migrant and Seasonal Agricultural Workers and their employers throughout Wisconsin. This grant is intended to protect our farmworkers, their employers and our communities by advocating for and assisting in the deployment of a coordinated statewide effort around COVID-19 prevention and response.

    March 2, 2020 - Rural communities often have distinct healthcare needs compared to urban counterparts, sometimes exacerbated by demographic and societal trends. This panel identified key issues in rural healthcare and discussed the promise of collaborations to improve the health of people in Wisconsins rural communities.

    April 6, 2020 - While the coronavirus is having a disastrous impact on cities like New York and New Orleans, the virus is also starting to hit rural America. Adriana Diaz visits a town in Wisconsin, where doctors are worried an already spread thin health care system could be stretched to the limit.

    Have a phone number to call but its not even the office phone number called for 3 days with no return thought it was suppose to be a medical office getting really irritating

    Do not take your kids there. They are not a pediatric dentist and the dentist was super rude to my 9 year old. She kept rolling her eyes and getting annoyed. I will never use this place again.

    Always a problem here. One time I went in for a scheduled appointment that "was not" scheduled that I had been waiting over a month for. At my most recent appointment, I had one tooth extracted & was left with my whole left side of my face swollen and bruised. When I called, because the pain has been unbearable for over a week, I kept getting transferred or their voicemail box. They treat clients as if they are an inconvenience & always rush the dental procedures. Do not recommend.

    Horrible place, 4 months and they haven't did a pre auth on getting my tooth fixed, tooth is infected and now the tooth just needs to be pulled. I wouldn't bring anyone to this place.

    Called to cancel my son's appointment. We rescheduled and when we got there said he didn't have one. I know I had the right day. I always put a reminder on my phone after I make a appointment

    I call ahead of time to confirm my son's appointment was told that they would still see him because he's got bronchitis and has a cough and then I get there after wasting all the gas driving up there to find out that all because he has a cough he's considered a covid patient and they cannot see him I guess medical documentation stating he has bronchitis is not good enough for them

    I didn't even get into the office yet. Called for an appointment and explained that I had 5 infections in the last year. As soon as she ( who ever answers the phone) found out that I have United Health thru Badger Care I was told I can just walk in and have to wait to when they can fit me in but it can take me going there and waiting for 1 day or more. Who does that? All I wanted was an appointment. I can't take days off to see if I get seen or not. Now I have 2 teeth broking off at the gumline and don't know where to go.

    Nicole, a dental hygienist at mauston dental, made me cry because she was so rude. Told me I'd be lucky if I don't lose my front teeth before I'm 30 because " I must've Forgotten how to floss." She told me to go home and watch a YouTube video to teach me how to floss correctly. She was so rough and caused me significant discomfort, even pain, just for a routine cleaning.

    0 if I could Numbed the wrong area 1st Then sprayed the nerve of my tooth with cold water and did nothing for me. But bravo you guys know how to check people's blood pressure. What a joke

    I developed an infection over the weekend that has spread into my face and jaw bone swelling it up like a grapefruit! Because all of the phone lines in Necedah were down, I walked in with a dental emergency, I was asking them for help... and was basically told to go away. Not in such a direct manner, But in a "they were just so busy, I could sit in a hot car all day with a pager hoping to get checked." manner. It was 86 degrees, and there is minimal shade. It was one of their dentists that originally messed up my mouth, and when I NEEDED help, I was denied. The receptionist was extremely RUDE!!!!!!!!!!! and she as unhelpful as a person could be. Honestly How long would it take to do a check up, so I could get some antibiotics so I do not die from an infection in my mouth. Why would there not be a buffer planned in a day for such emergencies?

    When working in my mouth always been comfortable and not to much pain. The dentist are gentle.The reason for leaving 4 stars is because of the receptionist can come off rude and if you have an emergency they never fit you in you have to call when they open to see if they can fit you in. So I've been waiting four in a half months with an infection and terrible pain.

    Had my first dentures made here and they were great. After getting my new set the person in charge of dentures was extremely rude and wouldnt let me change anything about how they would look. I just wanted a more natural look for myself and got dentures that now make me cry when I wear them.

    Throughout my life I have had extremely bad experiences with dentists. I panic at the thought of making an appointment. until I came here. everyone was so nice and the dentist was so gentle. I had 2 teeth pulled and never had any pain at all. I'm looking forward to having them take care of all my dental needs.

    I'm such a chicken when it comes to going to the dentist, but I broke a tooth and it needed to be pulled...I was in a lot of pain! From the time I called to ask about their office and insurance, they were very, very nice to me. COVID procedures were in place and I felt very safe ! The nurse I had was awesome! The Dr was great! I told him I'm a big ole chicken and he was so awesome! He had me numbed up and I could barely feel the shot or the procedure! It was out before I knew it! I will definitely be back!
