Dr. Cieslak began his dental career in the United States Air Force, serving both stateside and abroad for 6 years. He attended The Ohio State University and Medical University of South Carolina, where he also completed his Oral & Maxillofacial Surgery Residency. Dr. Cieslak moved to and fell in love with Louisville, where he founded Louisville Oral Surgery & Dental Implants in 1996. Dr. Cieslak specializes in dental implants, tooth extractions,BOTOX, jaw pain relief and wisdom teeth treatment. In addition to his duties at Louisville Oral Surgery & Dental Implants, Dr. Cieslak also serves as the President of the Louisville Dental Society. Dr. Cieslak enjoys the outdoors and prefers to spend his free time with his children, hunting, or fishing.
Our Office Manager, Christy has been with Louisville Oral Surgery since 2014, and working in dentistry since 1996. Christy loves spending time with her family and is very involved with her church community. Christys favorite part of working at Louisville Oral Surgery? Helping patients figure out what their best options may be and assisting everyone who walks through our door!
Tiffanys sparkling personality makes every patient feel right at ease the moment they become part of the Louisville Oral Surgery practice. She is a wealth of knowledge and enjoys answer any and all questions that new or current members have. In her free time, Tiffany can be found at a soccer game or a dance class with either of her two beautiful little girls.
Lauren has been with Louisville Oral Surgery since 2014 and is passionate about being up to date on all aspects of dental healthcare. Her dedication to her work is evident the moment you meet with her. Lauren leads a very active lifestyle playing softball, bowling and disc golf but nothing keeps her on her toes as much as her sweet two-year-old!
As a key member of our surgical team, Miriam works closely with our Doctors to ensure that all patients receive the best care in Louisville. She loves working in dentistry and helping patients navigate the implant and surgical processes with the rest of our Louisville Oral Surgery Staff. When shes not working hard for our patients, you can find Miriam spending time with her children and family.

The surgical staff at Louisville Oral Surgery are experienced oral and maxillofacial surgical assistants, and have taken the anesthesia course provided by (AAOMS). Patients are continuously monitored during and after surgery. Dr. Cieslaks office meets the high standards required by the Kentucky Society of Oral and Maxillofacial Surgery (KSOMS) for providing in office anesthesia for patients safety and comfort. All staff are CPR certified.

I went here to get 19 extractions and was very pleased with my outcome. I have passport and was made to feel welcome and treated with the upmost respect. I would recommend them to anyone. Thanks for the good care guys, great job.

I have had my family's Oral surgical needs and my wife's cosmetic procedures (Botox, lip injections, etc) done at this office over the last year and a half and I couldn't begin to relate how professional and awesome this office is for anything related to oral surgery or maxillofacial surgical needs. My wife just had three permanent implants placed and their aftercare phone calls and check ups make her feel like she really belongs in the planning, care, and aftercare process as a teammate instead of just as a patient. I DEFINITELY RECOMMEND THIS TEAM OF DOCTORS AND THEIR STAFF! I review a lot of places and -- as one could see by going through my past reviews -- I will openly state when a company fails to meet my expectations, but this office performs above and beyond my expectations.

My son Adam had his wisdom teeth extracted by Dr. Babcock who performed the job quickly and expertly. The staff were very kind, patiently outlining the schedule of medication.

Dont go there! I walked in this place to get a tooth looked at. He looked at the xray I brought with me. Looks good..ok..walking out after 1 minute..do I owe anything, no your good, I said I need to pay right now while I got the money. Nope your good..Now I'm out of work because of cancer. Which they didn't listen . Got a 45 dollar bill. I'm paying 12 dollars every month . Sometimes I didn't have 12 to send. Now all I owe is 12 dollars left..getting ready to send it in..they sent it to the collection..r 1 step away..I called and explained my healt . ..who cares..

I have been in pain since December over a wisdom tooth that needed to be pulled. I walked into their office and not even a hour later they were helping me by taking it out. Not only was the rate affordable the staff and Dr. Joseph were friendly, patient and kind. I have anxiety and they were still able to make me laugh and make pulling the tooth a easy experience. Such welcoming people! Thank you guys so much.

Zero stars if I could. I made an appointment to get a wisdom tooth pulled. My mother took an unpaid day off, drove over an hour to be my ride and I paid a sitter to watch my children. We got there 30 minutes early and waited for their opening to call and check in. They CANCELLED my appointment, telling me they called to confirm and got no answer. I have NO record of their call and was not told I would get one! So the appointment I ALREADY COMMITTED TO BY MAKING, was cancelled! The lady on the call with me was kind to explain how the dentist's 2 hours of money was more important than the satisfaction of of his customers. I was also belittled because of my income and insurance. I then tried to reschedule and was HUNG UP ON. As I'm talking to my dental office to make an appointment elsewhere, guess who calls back? Yep, their call came through fine that time! I will NEVER be back there again. They have greatly inconvenienced my family and blamed ME for it!

Best visit yesterday. Doctor thought it best to wait a few more weeks on final implant uncovering and I appreciate his honesty. Walked me to front desk saw me out. These people have become my family.

Not professional....no confidentially...loud and rude staff...didnt even make it to the doctor they so damn unprofessional....yelling info about patients throughout office...can hear doctors convo; clearly through wall in waiting room (some woman was receiving botox)...dont go here...call me to back just to tell me they wasnt ready for me....

I just got my wisdom teeth out this morning here. They did a fantastic job, and with the pain meds they gave me I am feeling great except for the swelling. There was one nurse I had in there who wasn't really interested in being friendly, but I understand it might have just been a bad day for her. She also told the second nurse I had, that people coming out of anesthesia were so annoying and I didn't appreciate that but please don't hold that against her. The second nurse I had was the one who gave me my IV and she was fantastic. She was super nice and funny, and she really helped calmed me down. She even helped me walk out to my dad's truck to get in which I appreciate immensely. Also, when I got home I was a bit uncomfortable and distressed but my dad called the office for me and they explained to me and him how to ease my pain and symptoms. I really feel a lot better after hearing that advice. And I am surprisingly pain free right now. All in all, I had a great experience here and I'd just like to thank both the nurses and the doctor for their help in my wisdom teeth removal. By the way in case someone from the office happens to see this review, my name is Meagan Hindman. Cheers!
