Dr. Cieslak began his dental career in the United States Air Force, serving both stateside and abroad for 6 years. He attended The Ohio State University and Medical University of South Carolina, where he also completed his Oral & Maxillofacial Surgery Residency. Dr. Cieslak moved to and fell in love with Louisville, where he founded Louisville Oral Surgery & Dental Implants in 1996. Dr. Cieslak specializes in dental implants, tooth extractions,BOTOX, jaw pain relief and wisdom teeth treatment. In addition to his duties at Louisville Oral Surgery & Dental Implants, Dr. Cieslak also serves as the President of the Louisville Dental Society. Dr. Cieslak enjoys the outdoors and prefers to spend his free time with his children, hunting, or fishing.
Our Office Manager, Christy has been with Louisville Oral Surgery since 2014, and working in dentistry since 1996. Christy loves spending time with her family and is very involved with her church community. Christys favorite part of working at Louisville Oral Surgery? Helping patients figure out what their best options may be and assisting everyone who walks through our door!
Tiffanys sparkling personality makes every patient feel right at ease the moment they become part of the Louisville Oral Surgery practice. She is a wealth of knowledge and enjoys answer any and all questions that new or current members have. In her free time, Tiffany can be found at a soccer game or a dance class with either of her two beautiful little girls.
Lauren has been with Louisville Oral Surgery since 2014 and is passionate about being up to date on all aspects of dental healthcare. Her dedication to her work is evident the moment you meet with her. Lauren leads a very active lifestyle playing softball, bowling and disc golf but nothing keeps her on her toes as much as her sweet two-year-old!
As a key member of our surgical team, Miriam works closely with our Doctors to ensure that all patients receive the best care in Louisville. She loves working in dentistry and helping patients navigate the implant and surgical processes with the rest of our Louisville Oral Surgery Staff. When shes not working hard for our patients, you can find Miriam spending time with her children and family.

The surgical staff at Louisville Oral Surgery are experienced oral and maxillofacial surgical assistants, and have taken the anesthesia course provided by (AAOMS). Patients are continuously monitored during and after surgery. Dr. Cieslaks office meets the high standards required by the Kentucky Society of Oral and Maxillofacial Surgery (KSOMS) for providing in office anesthesia for patients safety and comfort. All staff are CPR certified.
