T he Primary Health Network (PHN) began with one small community health center site in the Shenango Valley, in 1984, with a central focus on providing the highest quality of care to the people we serve. Since our inception in 1984, The Primary Health Network, a private, non-profit organization, has worked with local communities to develop and implement a new model of health care: the Community Health Center.
After over 38 years of service, PHN has grown to become the largest Federally Qualified Health Center (FQHC) in Pennsylvania and one of the largest in the nation. Currently, we staff over 150 physicians, dentists, physician assistants, certified nurse practitioners, and other health professionals. Our support staff includes over 450 employees, as well. Last year, we reached over 75,000 patients through health, educational, and enabling services.
Our country's future healthcare system will present new challenges and opportunities for growth. The Primary Health Network stands ready to meet the healthcare needs of all residents in the communities we serve, and we will continue to look to the future, as we continue to provide quality healthcare.

Doctor was extremely unprofessional and kinda rude speaking to his assistant and gets 1 star but changing it to 4 stars as his assistant gets 5 stars and customer service was nice about handling it and rescheduling with a different doctor

Can't ever reach anyone on the phone. Been trying for months now to get in to get my tooth pulled or looked at considering there's a giant hole in it. Call during normal business hours & get the answering machine of leave a message or call back during normal business hours.

Fantastic! I have been in pain with my teeth that it's affecting my health. I went in there to just ask if they could help me out and the lovely lady in the dental reception was so helpful. She gave me all the sliding scale details and I was in there within 20 mins having one of the teeth taken care of. I was clearly nervous but the dental assistant and the male dentist were fantastic. Next appointment in a few weeks, finally somewhere that can help me and a dental practice that actually cares rather than be about money ! Fantastic service. Thank you so much

Employees very unprofessional when calling to make an appointment and employee says hello can I help you she refused to speak. I hang up after a few minutes called back multiple times and it went to answer during normal business hours. It appears these people are playing games with people that have State Health insurance.

Just had a tooth pulled there today and although I was extremely nervous I didn't feel a thing!!! Excellent service and I would highly recommend them to anyone!!! Job well done

If I could rate this at a zero I would. I brought a lady in as her aide, the assistant called me back to make me answer questions that my lady was clearly trying to answer. Just for the dentist to tell me I had no business back there and I needed to leave immediately. Very rude and disrespectful.

Not a fan, but nowhere else that I can afford. The doctors are rather rough and it takes forever to see your doctor even when you show up on time.

Not a fan but they are the only place that would take my insurance. Another downfall..they never answer the phone and want you to leave a message but they never call back.
