Separate efforts in Manitowoc County and Sheboygan County to increase access to primary health and dental care for underserved residents joined to form Lakeshore Community Health Care in 2014. Today, residents of both counties can access primary and preventive medical, behavioral and oral health care at two convenient locations in Sheboygan and Manitowoc.
United Way of Manitowoc County provided a venture grant to expand Healthy Teeth Healthy Community dental services to adults. A new non-profit, Healthy Teeth Healthy Communities of Manitowoc County Inc. (HTHCMC), was created to support the expanded services.
In order to make the best use of federal and local grant opportunities, the Manitowoc and Sheboygan organizations merged. Lakeshore Community Health Care, Inc. became the newly combined non-profit's official name.
Both LCHC clinics received Patient Centered Medical Home designation and quality awards for our vaccination efforts and Health Information Technology from the Health Resources and Services Administration.
