Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental and behavioral health services through a team-based model of care. It is the only clinic in its 5-county service area that offers a comprehensive sliding fee scale, primary care to Medicaid and low-income, uninsured patients, and is the only safety net dental provider.

Services include diagnosis, treatment and management of acute and chronic illnesses, well-child and adult care, family planning, gynecological and obstetrical care, prenatal and perinatal services. Our clinic is here to meet the needs of all individuals in our community.

The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scalebased on Federal Poverty Guidelinesif deemed income eligible. No patient will be denied service for their inability to pay.

We believe that optimum well-being includes the treatment of the mind, body, and soul. That is why we use an integrated care approach, offering treatment services for emotional health concerns in addition to physical and oral health problems that our patients experience. In recognizing the close relationship between physical and emotional health, our care enables you to recover more fully and quickly.

As a Level 3 Patient Centered Medical Home (PCMH), our priority is making sure that your care is provided in the right place, at the right time, and in the manner that best suits your needs. We recognize the unique culture, values and preferences of each patient and will work with you to develop a treatment plan that best suits your individual needs.

Our rigorous Quality Assurance and Improvement program focuses on evaluating the quality of services that we provide to make sure each is effecctive. Using the extensive reporting capabilities of our Electronic Medical Records, we are able to review the health outcome data for each patient to make sure their health improves.

Summit Community Care Clinic was established in 1993 as a weekly walk-in clinic staffed by volunteers and managed by Summit County Public Health to address the need for primary and preventative health care for people who were uninsured and underinsured in Summit County. In the first year of operation, the Care Clinic provided 294 patient visits. The mission then was to provide care for all, regardless of barriers to care and the ability to pay. A lot has changed since 1993, but the mission of the Care Clinic remains the same. We are committed to providing exceptional, integrated, patient-centered health services designed to meet the health needs of all patients, particularly those who experience barriers to care, regardless of ability to pay.

Today that mission is more important than ever. With high deductibles and costs in the central mountain region that far exceed national and state averages, health care has become a challenge for almost everyone. Although approximately half of our patients have health insurance and half dont, all face similar challenges in navigating an increasingly complex, overly costly health care environment.

Helen joined the Care Clinic in 2006 as aGrant Coordinatorandshe implementedthe Integrated Care program.She subsequently served as the Behavioral Health Director, Quality Assurance Director and Chief Operating Officer before becoming Chief Executive Officer in 2016.Helen has a bachelors degree in Organizational Psychology from the University of California and a masters degree in Agency Counseling from the University of Northern Colorado.She has lived in Summit County since 1988.

Dr. Kathleen Cowie is a family physician with a special interest in women and childrens health. She is fluent in Spanish and has a passion for providing compassionate medical care at home and abroad, having volunteered on outreach trips in Honduras, Peru, Bolivia, and Guatemala. Dr. Cowie graduated from Loyola Chicago Stritch School of Medicine in 2006 but fell in love with the Colorado Rockies while completing her family medicine residency at St. Anthony Central/North Hospitals in Denver. Prior to joining the Care Clinic in 2012, she provided full-spectrum family medicine including obstetrics and newborn care at Salud Family Health Centers in Commerce City and Platte Valley Hospital in Brighton. Dr. Cowie became Chief Medical Officer in 2017. In her free time, she loves doing all things outdoors and spending time with her family.

Erin joined the Care Clinic as the Dental Coordinator when the Care Clinic first implemented dental services in 2007. She subsequently spent 10 years as the School Based Health Director before returning to the Dental Department as the Chief Dental Officer in 2018. In her 35+ years in Summit County, Erin has served in various leadership positions including on the boards of Summit School District, Early Childhood Options, and the Colorado Association of School Based Health Centers. Erin has a Bachelor of Science degree in Dental Hygiene from the University of Iowa. She loves all that Summit County has to offer for recreation and spends her weekends skiing at Arapahoe Basin.

Eleanor Bruin joined the SCCC in 2020 after working as an Integrated Behavioral Health Provider with High Country Healthcare in Frisco, CO. Eleanor has worked as a behavioral health provider in an integrated medical setting for more than 10 years with previous work experience at the University of Colorado Cancer Center, Sky Ridge Medical Center and Swedish Hospital. She has held previous leadership positions with the Association of Oncology Social Workers and has supported facilitation of national conferences. Eleanor graduated with her masters in Social Work from the University of Denver Graduate School of Social Work. Eleanor is a Colorado native and when she is not at the SCCC she is busy spending time with her husband and her three young children enjoying the outdoors.

Cassandra Anton has been with the Care Clinic since 2015 with experience as a quality professional in multiple industries. Cassandra holds a bachelors degree in Biology from the University of Colorado at Colorado Springs. She is a Colorado native who loves hiking, camping, skiing, and exploring the Colorado backcountry.

Susan earned her Master of Public Health Degree from the University of North Carolina at Chapel Hill and has dedicated her career to non-profit management and development, working with multiple organizations in Summit County and statewide to support health promotion programs. She is a former Peace Corps Volunteer (Central African Republic), has a close connection to Senegal, and dreams of her next trip to Africa.

Misty joined the Care Clinic in 2020 after prior work managing dermatology clinics across Western Colorado. She also has experience in home health and hospice care with a collective 20 years experience in healthcare. Misty holds a Bachelors degree in Clinical Health Services/ Healthcare Management from Missouri State University and has been licensed in Practical Nursing since February 2000. She is passionate about serving the Colorado mountain communities and creating avenues for patients to access healthcare. In her free time, Misty loves spending time with her husband, 3 human children and 3 fur babies. She enjoys all that living in Colorado has to offer and wont turn down a chance to travel.

Megan joined the Care Clinic in 2015 as a Dental Hygienist and she helped to implement the SMILES and COMDI programs in the School Based Health Centers. Megan has a Bachelors Degree in Integrative Physiology from the University of Colorado and an Associates Degree in Dental Hygiene from Pueblo Community College. She is a Colorado native.

Liz joined the Care Clinic in 2020. She graduated from Colorado State University with a Bachelor of Science in Business Administration. Liz pursued her career in nonprofit management and has worked in programming and development for arts, cultural, and educational organizations in Winter Park, Boulder and Vail, Colorado. A third generation native of Colorado, she has lived more than half her life on the Western Slope and spends her free time exploring the surrounding mountains on skis, bikes and in a great pair of hiking boots.

Karen Augustyn joined the Care Clinic in a joint position with Summit School District as their Health Paraprofessional in 2020. Prior to working at the Care Clinic, she worked at UCHealth as Manager of Operations for Dermatology, Infectious Disease, and Hospitalist services lines. Karen holds a Bachelor of Science degree in Human Physiology from University of Iowa. In her free time, Karen loves to bike, snowboard, and hike.

Karen started her career in 2012 as a Medical Assistant. Her experience workingdirectlywith patients gave her a deep understanding of what works and what doesn't work on the clinical floor and how leadership interaction affects all patient-facing staff members and patient outcomes. Karen's role as Clinical Manager has given her the opportunity to support all clinical staff members and to give the best quality of care to patients.

Amanda joined the Care Clinic in 2019 as a Dental Patient Access Representative. Before becoming the Dental Clinic Manager in 2021, she had worked as the School Based Health Dental Coordinator and Dental Schedule and Referral Coordinator. Amanda has a Bachelor of Arts degree in Sustainability Studies from Colorado Mountain College. She is a fifth generation Colorado native and when not at the clinic she is busy spending time with family, caring for her chickens, gardening, fishing, camping, foraging, and baking.

Araceli joined the Care Clinic in 2007 as a Dental Assistant. With almost twenty years in the dentistry field, her passion for taking care of patients has been her main focus. She enjoys taking care of people of all ages and loves seeing patients walking out with a smile on their faceafter a great dental experience. As the Lead Dental Assistant, she takes great pride in training the staff to be great dental assistants. Before Aracelis career in the dentistry field she was a professional pastry chef making high end desserts and wedding cakes. When she isnt working, she enjoys spending time with her family going on hikes with her dog, camping, cooking, and watching movies.

Myrna joined the Care Clinic in 2021 as the Revenue Cycle Manager, with almost 15 years of experience in the billing field at FQHCs. She is a Certified Professional Coder and Certified Professional biller. Originally from Mexico, she moved to the United States 18 years ago were she met her now husband in Colorado Springs. Myrna has 2 sons; the oldest is currently serving in the Navy, the youngest a Senior in high school. When shes not at work, Myrna likes to spend time outsideand does volunteer work in the community.
