Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our team when you arrive for your initial visit to our office, or you can fill out the forms online.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring team. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Our dental office uses the latest state-of-the-art equipment and cutting edge technology and we uphold the strictest sterilization techniques. I have been "Dental-Phobic" my whole life, but Dr. Lewis changed all that for me. She is so kind, compassionate, and funny that I felt at ease the whole time. She's changed my relationship with dental hygiene and I am eternally grateful to her and her whole staff. I never thought I'd enjoy a visit to the dentist.

I had a crown come off on my second day of vacation. After a few calls I found a next day appointment at Lahaina Dental Group. A new Dentist is kinda scary, but the whole staff welcoming and Dr Brown was quick and gentile. He answered my questions and got me fixed up so I could enjoy the rest of our vacation. Highly recommend.

I had a cleaning with Lani, best cleaning ever! She was very gentle, and didn't miss a spot. During my cleaning it was discovered that i needed a filling. Helen at the front desk was able to fit me in with Dr. Lewis who did an amazing pain-free job.

I was on vacation in Maui and developed a excruciating toothache. I called around and finally got onto Lahaina Dental group who were amazing and were able to squeeze me in at short notice. Thanks to Dr Brown and his team for their expert and compassionate work they did to fix my problem enabling me to continue on with my vacation on Maui.

I would give zero stars if possible. Don't bother making an appointment with this group, they will book you over a month out and then call you a few days before your appointment to reschedule you for a month later. Completely unprofessional.

We had a very pleasant experience with Dr. Brown and his staff. I was very anxious about the appointment and any procedure, but Dr. Brown was very patient with me. The staff was very helpful and kind and always available to help. Dr. Brown is very professional, he takes the time to explain what needs to be done and provides valuable advice. I highly recommend Dr. Brown and his staff.

My husband's tooth starting hurting the first day on vacation and was killing him on Haleakala. West Maui dental allowed him to walk in and be seen quickly for a very fair price. The dentist was knowledgeable, helpful, and kind. This is the kind of place I'd be lucky to find back home. Thank you so so much!!
