Advance care planning is an important step to ensure you get the medical care you want, even if family members are making the decisions for you. Advance care planning involves identifying a health care agent who can help with medical decisions, and developing a written health care directive a plan for your future medical treatments.
Our informational meetings are designed to educate you on advance care planning, and offer you the option to schedule a free individual session with a facilitator to develop a personalized advance care plan. If you have a health care directive that you would like to review at the session, please bring a copy with you.

Through our Pregnancy and Infant Loss Support Group, you can find support from those who understand your intense grief other parents who have lost a baby. Come to a confidential, safe place to share your feelings and learn helpful coping strategies. This group is free and no registration is required.
