This Notice describes how your health information may be used and disclosed and how you can get access to this information. Please review it carefully. The privacy of your health information is importantto us.

Federal and state laws require us to maintain the privacy of your health information. We are also required to provide this Notice about our offices privacy practices, our legal duties, and your rights regarding your health information. We are required to follow the practices that are outlined in this Notice while it is in effect. This Notice takes effect10-10-16, and will remain in effect until we replace it.

We disclose medical information to our employees and others who are involved in providing the care you need. We may use or disclose your health information to another dentist or other healthcare providers providing treatment that we do not provide. We may also share your health information with a pharmacist in order to provide you with a prescription, or with a laboratory that performs tests or fabricates dental prostheses or orthodontic appliances.

We may use and disclose your health information to obtain payment for services we provide you, unless you request that we restrict such disclosure to your health plan when you have paid out-of-pocket and in full for services rendered.

We may use and disclose your health information in connection with our healthcare operations. Healthcare operations include, but are not limited to, quality assessment and improvement activities, reviewing the competence or qualifications of healthcare professionals, evaluating practitioner and provider performance, conducting training programs, accreditation, certification, licensing or credentialing activities.

In addition to our use of your health information for treatment, payment or healthcare operations, you may give us written authorization to use your health information or to disclose it to anyone for any purpose. If you give us an authorization, you may revoke it in writing at any time. Your revocation will not affect any use or disclosures permitted by your authorization while it is in effect. Unless you give us a written authorization, we cannot use or disclose your health information for any reason except those described in this Notice.

We must disclose your health information to you, as described in the Patient Rights section of this Notice. You have the right to request restrictions on disclosure to family members, other relatives, close personal friends, or any other person identified by you.

We will not send you unsecured emails pertaining to your health information without your prior authorization. If you do authorize communications via unsecured email, you have the right to revoke the authorization at any time.

We may use or disclose health information to notify, or assist in the notification of (including identifying or locating) a family member, your personal representative or another person responsible for your care, of your location, your general condition, or death. If you are present, then prior to use or disclosure of your health information, we will provide you with an opportunity to object to such uses or disclosures. In the event of your incapacity or emergency circumstances, we will disclose health information based on a determination using our professional judgment disclosing only health information that is directly relevant to the persons involvement in your healthcare. We will also use our professional judgment and our experience with common practice to make reasonable inferences of your best interest in allowing a person to pick up filled prescriptions, medical supplies, X-rays, or other similar forms of health information.

If this dental practice is sold or merged with another practice or organization, your health records will become the property of the new owner. However, you may request that copies of your health information be transferred to another dental practice.

We may, and are sometimes legally obligated, to disclose your health information to public health agencies for purposes related to preventing or controlling disease, injury or disability; reporting abuse or neglect; reporting domestic violence; reporting to the Food and Drug Administration problems with products and reactions to medications; and reporting disease or infection exposure. Upon reporting suspected elder or dependent adult abuse or domestic violence, we will promptly inform you or your personal representative unless we believe the notification would place you at risk of harm or would require informing a personal representative we believe is responsible for the abuse or harm.

We may disclose your health information to appropriate authorities if we reasonably believe that you are a possible victim of abuse, neglect, or domestic violence or the possible victim of other crimes. We may disclose your health information to the extent necessary to avert a serious threat to your health or safety or the health or safety of others.

We may disclose to military authorities the health information of Armed Forces personnel under certain circumstances. We may disclose to authorized federal officials health information required for lawful intelligence, counterintelligence, and other national security activities. We may disclose to correctional institutions or law enforcement officials having lawful custody of protected health information of inmates or patients under certain circumstances.

Upon arriving at our office, we may use and disclose medical information about you by asking that you sign an intake sheet at our front desk. We may also announce your name when we are ready to see you.

You have the right to look at or get copies of your health information, with limited exceptions. You may request that we provide copies in a format other than photocopies. We will use the format you request unless we cannot practicably do so. You must make a request in writing to obtain access to your health information. You may obtain a form to request access by contacting our office. We will charge you a reasonable cost-based fee for expenses such as copies and staff time. You may also request access by sending us a letter. If you request copies, there may be a charge for time spent. If you request an alternate format, we will charge a cost-based fee for providing your health information in that format. If you prefer, we will prepare a summary or an explanation of your health information for a fee. Contact us for a full explanation of our fee structure.

You have a right to receive a list of instances in which we disclosed your health information for purposes other than treatment, payment, healthcare operations and certain other activities for the last six years. If you request this accounting more than once in a 12-month period, we may charge you a reasonable cost-based fee for responding to these additional requests.

You have the right to request that we place additional restrictions on our use or disclosure of your health information. We are not required to agree to these additional restrictions, but if we do, we will abide by our agreement (except in emergency). In the event you pay out-of-pocket and in full for services rendered, you may request that we not share your health information with your health plan. We must agree to this request.

You have the right to request that we communicate with you about your health information by alternative means or to alternative locations. You must make your request in writing. Your request must specify the alternative means or location, and provide satisfactory explanation of how payments will be handled under the alternative means or location you request.

You have the right to request that we amend your health information. (Your request must be in writing, and it must explain why the information should be amended). We may deny your request under certain circumstances.

If you are concerned that we may have violated your privacy rights, or you disagree with a decision we made about access to your health information or in response to a request you made to amend or restrict the use or disclosure of your health information or to have us communicate with you by alternative means or at alternative locations, you may send a written complaint to our office or to the U.S. Department of Health and Human Services, Office of Civil Rights. We will not retaliate against you for filing a complaint.

Welllllll my husband and I both tried this place for the first time and both felt very expedited. The dentist told us what was wrong without first inspecting our teeth and for me there was a huge lacking in polite and professional communication. I had said I needed extra time since I hadn't been since pre-covid and was 10 minutes late due to traffic stops. I called at one minute late to notify them but when I arrived the dentist just rushed me in and mentioned to his employees FIVE times that there next person would arrive soon and another person would arrive soon and they're already getting behind...no communication directly with me about timing. I understand I was late and that can throw off a day but all they said to me was near the end of my VERY fast appointment when I asked "so am I not getting my teeth cleaned today?" "No we're not going to have time today." And that was all. As a fellow service business owner who encounters this very problem I'd like to suggest saying something like "Hi Jane I'm Doctor Joe it's so nice to meet you. I heard you were stuck in traffic I'm so sorry to hear that. We were really looking forward to cleaning your teeth today Unfortunately we are on such a tight schedule in here that ten minutes late is too much for our schedule to accommodate you today. We really like to be thorough and want to give you all the time you need for a thorough service. Can we reschedule you so we can still take care of that for you?" Since we both left feeling that we need an alternate opinion on our diagnosis and that we were surprisingly rushed through we're going to have to try another dentist but we hope this suggestion helps and wish you luck!

$350.00 for new patient appointment is on the highest side for services. But nothing like the $375.00 fee for a 10 minute max death cleaning and missed appointment. Get a fee cost first.

Totally unnecessarily drilled the hell out of my teeth, which have hurt ever since. Maxed out my insurance, filled my didn't-need-to-be-filled teeth very shoddily (large, painful gaps that had to be redone elsewhere), did unnecessary procedures (over the course of what felt like an insane number of visits), and all in all created persistent tooth pain (still life interrupting 6+ months later). Had perfectly fine teeth before I came here. Very sad to have had such a poor experience and feel so scammed -- office seemed caring and competent initially, and disappointed to see such blatant profiteering underneath the veneer of friendliness. Not sure how this continues to fly in such a small community -- everyone I know who has gone here has had some version of the same experience.

Dr Henderson billed my insurance for a cleaning that never happened I only know, because I wanted my cleaning done somewhere else. Insurance says: "Sorry, dentist's medical records are legal documents and indicate that the cleaning was performed".

Unfortunately, it seems a lot of dental practices nowadays are just there to make a buck off of people and don't really care about patients or providing good dental care. In contrast, Dr. Henderson really seems to care about and do right by his patients. He took time to recommend at-home procedures to prevent problems. He resells Rx toothpaste and Rx mouthwash at cost; I've never even heard of this before. He is knowledgeable about insurance (rare!) and even told me which insurance will cost me the least instead of pay him the most. He is a valuable find and well worth the 45 minute or more drive.

Something is very off about this practice. They offer discounts to new patients who review them online, so I assume that is where many of the good reviews come from as I did this also when I was a new patient to save money on my next appointment. I went in after having no signs of new cavities or fillings needing to be replaced and have never missed any cleanings/exams. I was told that I needed to have a cavity filled and a filling replaced, what happened after was multiple days of work that I now believe was all to max out my insurance. Their billing is never correct and I played a game of back and forth after each bill as I always "owed them" and then had somehow overpaid and was sent a check covering the difference. It became difficult to follow, so I had to do my own accounting to make sure that everything matched up with what was being billed to insurance and pointed out discrepancies to them. I decided to go back to my old dentist after the experience as it was very violating and stressful and I feel like the practice does have patients best interest at heart.

I'm simply not getting the service i asked for when i went in a for a standard cleaning... this is basics 101... clean my teeth, don't sign me up for more expensive treatments that i don't need... Henderson, i checked with another dentist who told me that deep cleaning is completely unnecessary and only something you prescribe when you have an infection in the gums. Not cool man.

We have started going to Kings Beach Dental 2019. They really care about their patients and gives the flexibility as much as possible to accommodate their patients. They take time to know their patients! Dr. Jason and his staff treats you like their own family! My teeth has been taken care of, clean and very detailed! Today was my daughter's first dental visit! They are very welcoming and treat her very special. She is a bit nervous but Dr. Jason was amazing with kids. She is happy now with more teeth coming out for tooth fairy Thank you! Really happy and satisfied with our dentist within the north lake basin that is professional and personable!

We are a family of three and having been going to Dr. Henderson for the last 2 years and very satisfied with his work. He is professional, timely and very friendly. His work is the best on the North Shore of Tahoe. His staff is also some of best quality of professionals in the area. We highly recommend Kings Beach Dental!! 5 stars!
