For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are check, cash, or any major credit card.
Dr. Jerome Forman returned to his hometown of Kenilworth N.J. in 1984 following his years of graduate education in dentistry. He joined his childhood dentist Dr. Lloyd Nimaroff, whose practice was founded in 1948 in Kenilworth. Together they formed Kenilworth Dental Associates. Chosen by his peers as a Top General Dentist in N.J. Monthly magazine for the past 10 consecutive years as well as Top Dentist by the National Consumer's Research Council in Washington D.C. and USA Top Dentists, Dr. Forman is a member of numerous dental associations, societies, and study clubs and has held professional leadership positions throughout his career.
A graduate of Kenilworths Harding School and David Brearley High School, Dr. Forman attended Rutgers College and completed graduate research in Human and Neuroanatomy at Farleigh Dickinson Dental School. In 1984, he graduated top of his class with a D.M.D. degree from Washington University School of Dental Medicine in St. Louis, Missouri. To further his training, he completed a highly coveted general practice residency at St. Joseph's Hospital in Paterson, New Jersey in 1985.
A Fellow in the Academy of General Dentistry, which requires 500 hours of continuing education and passing a rigorous examination, Dr. Forman pursues monthly continuing education courses to remain at the forefront of state-of-the-art dentistry, applying innovative technologies to specific dental applications. In 2009, he was nominated by his peers and accepted as a Fellow in the American College of Dentists (ACD), the oldest major honorary dentistry organization comprised of only 3.5 percent of practicing dentists in the United States. Its members have exemplified excellence through outstanding leadership and contributions to dentistry and society.
Dr. Forman has lectured extensively on dental practice management, interdisciplinary dental diagnosing and comprehensive treatment planning. One of the first practitioners to introduce computers into treatment rooms and reap the value of digital radiography, which greatly reduces patient radiation and enhances patients understanding of their dental care, Dr. Forman is proficient in all phases of adult and children's dentistry. Practicing conservative cosmetic functional dentistry, he is skilled in the application of crowns, bridges, fillings and the restoration of implants, having handled vast numbers of these procedures during his career. Dr. Sorn Yaibuathes has joined Dr. Forman at Kenilworth Dental Associates and together they continue the mission of Kenilworth Dental Associates.
A member of the Kenilworth Board of Health, Dr. Forman serves as its finance chair and also leads the annual Kenilworth Health Fair. Selected to the Kenilworth Hall of Fame, he is a past member of Kenilworth Rotary Club and received its prestigious Paul Harris Fellow Award. A past president of the JCC in Scotch Plains, he has also received numerous awards from the town of Kenilworth and neighboring town councils, police, fire, rescue, and educational organizations for his local and national contributions. Dr. Jerome Forman and his wife Diane have three children. Stefanie is an educator; Daniel is a financial analyst; Michael is a MD/DMD Oral Surgery resident. In his spare time, Dr. Forman enjoys his family, golf, travel, reading, and theater.
In 2014, Dr. Sorn Yaibuathes joined the practice and continues to provide excellent care alongside her mentor, Dr. Forman, and their dedicated team. A native New Yorker, she earned her Doctorate of Dental Surgery from the prestigious University of Michigan School of Dentistry and was admitted to the Scholars Program in Dental Leadership. Upon graduation, she was recognized as the top ten outstanding clinicians of her class for excellence in comprehensive patient management and total patient care.
Following dental school, Dr. Yaibuathes completed a one-year General Practice Residency program at Morristown Medical Center, New Jerseys leading hospital in the nation. Her extensive training and knowledge made her well-equipped to manage dental emergencies and medically complex patients. In addition, she is able to execute difficult dental treatments to achieve the most aesthetic and functional results.
Dr. Yaibuathes is a Fellow of the Academy of General Dentistry, one of the most rigorous continuing dental education programs today. She was selected as TopDentists in 2018 and 2019. She is always striving towards honing her dental skills and has participated in numerous advanced training including Spear Education, Aesthetic Advantage, and the Pankey Institute. She is committed to staying abreast of the very latest proven materials, research and techniques to provide the highest quality of dental care to her patients. As a strong advocate of continuing education, she has adopted the lifelong learning philosophy as it pertains to enhancement of patient care, and her own personal and career growth. She has managed a great work-life balance where she enjoys international traveling, cooking and staying active.

Dr. Forman and his staff are awesome! Dr. Forman really cares about his patients and what is best for them. He is very dedicated to being the best dentist possible. He puts a lot of time and dedication in to his work. His staff is amazing, they are so professional and caring. I highly recommend this practice.

Nina M is the office manager and really nice person to work with. I was a patient there about a decade ago and now called the office for my dental record. Although they don't keep any archive over 7 years, Nina still helped me print out my visit record and sent out via postal mail without any charge. She made my day! Thank you so much, Nina!

I visited Dr. Yaibuathes for a check up and cleaning. I hadn't been to the dentist in four years (I know I know, very irresponsible) and I was nervous at what she would find. She informed me I had several cavities developing but we caught them in time before they got bad. She preceded to then put a little camera on a stick in my mouth and show me tooth by tooth exactly what she was seeing and explained to me what needed to be done. I came back for two more appointments to get the fillings in. Both times it was like watching an artist with their paintbrushes, only my mouth was the canvas, and the paintbrushes were drills and whatever they use to make fillings. She numbed the areas so I didn't feel a thing and before I knew it I was in and out. Swiftness aside, her craftsmanship in conforming the filling to my tooth and making sure my bite was still in alignment was impressive. Dr. Yaibuathes is very good at what she does and I trust her and this office with all my future dental needs. Speaking of the rest of the office. From the second you walk through the door you are welcomed with bright, friendly faces and attitudes from everyone behind the desk. It was a warm inviting environment that helps ease any concerns you may have about being at the dentist. If you are looking for a friendly and skilled dental staff, look no further than Kenilworth Dental Associates.

I first went to this dentist for a regular cleaning nothing else. After the cleaning Dr. Yaibuathes informed me that some of my old fillings should be replaced. A couple of days later she replaced the fillings on four of my teeth but I left the office in a lot pain that lasted several days. After a couple of unsuccessful attempts to rectify the problem and stop the pain which included drilling and refilling one of the teeth a second time, I had to seek treatment elsewhere. Little did I know that going for a tooth cleaning would set me back several hundreds of dollars, not to mention what my dental insurance had to pay for procedures that didn't work. I guess I should have gone for a second opinion on filling replacement and leaving well enough alone. Dr. Forman should've hired an experienced dentist someone that knows how to do fillings. If he keeps this so called dentist he will eventually loose a lot of his patients. I am very dissatisfied with their services. Dr. Yaibuathes was very unprofessional and unknowledgeable.

If I could give these folks 6 stars I would. I could write a whole term paper on these guys but I'll keep it short and sweet. I've been a patient of Dr. Forman for 20 years. He is by far one of the best dentists in NJ. His bedside manner is outstanding, he treats his patients as if they were the only patient he has. He is extraordinarily efficient in proactive care as well as reactive. He does everything in his power to ensure you are comfortable while doing the work that needs to be done. If you have even the crappiest insurance, he and his team are very proficient in quickly getting every possible cent from them to take the edge off your financial obligation. His office is run like clockwork and the attention to detail by him, his very friendly and talented partner Dr. Sorn, his assistants, and hygienists are spot on. He also has top notch and friendly office staff and stay on top of patients (which is a rarity). While very friendly, he will also be very forthcoming on your dental condition as it is, even if it's not what you want to hear; he is more interested in being proactive to minimize reactive visits of his patients, specifically with the annual and bi-annual cleanings. My teeth are the best they can be. If it wasn't for him I'd probably have several root canals by now, or even less a tooth or 2. Virtually my whole family are patients of Dr. Forman as well as they absolutely love and trust him like I do. Also, he is very responsive to afterhour/weekend emergencies. It is very difficult to find a good dentist, so please look no further than Dr. Forman as he is the exception, he truly cares about his patients and strives for perfection. But don't take my word for it, set an appointment with his friendly staff and see for yourselves!

I have had dental problems ever since I can remember and it all started getting better only after I met Dr. Sorn and Dr Forman. Both doctors are patient-centered and took the time to explain my treatment and procedures. The entire office is extremely friendly (and very clean and organized) and makes going to the dentist a great experience. Highly recommmended!
