Screening means checking your body for cancer before you have symptoms. Getting screening tests regularly may find breast, cervical, and colorectal (colon) cancers early, when treatment is likely to work best. Lung cancer screening is recommended for some people who are at high risk.

Please request non-urgent, non-controlled medication refills by calling your pharmacy. If you need provider approval for a renewed prescription, you can send a message online via your Patient Portal or you can call the health center where your prescription was written. We process prescription refill requests on Monday through Friday, with 48 hours notice.

If your condition requires emergency care (chest pain, severe difficulty breathing, trauma, loss of consciousness, etc.), please seek treatment at the nearest hospital emergency room. You can also call 911 (or the local emergency number if you live in an area without 911 service). If you are unsure if your condition requires emergency care, call your local health center.

If your local center is closed, your call will be automatically forwarded to one of our urgent care facilities. After they close, calls will be forwarded to an on-call provider. Our telephone lines are open 24 hours a day, seven days a week.

Hudson Headwaters delivers test results several ways: through the Patient Portal, through the mail, and through our Results Call system an automated phone or email message.If you want your test results available viayourPatient Portal, please let your health provider know.

Most referrals will be handled during your visit. If you request a referral for additional or specialty services (not at the time of your visit), we will ask for the name of the specialist, the date and reason for your upcoming visit, and appropriate insurance information.

Yes.InDemand Interpreting services are utilized at all of our Hudson Headwaters Health Centers. This free service instantly connects patients who may have a language barrier or who are hearing impaired to a live, medically trained interpreter via a computer during their office visit to ensure a better patient experience. Simply notify our patient services staff if interpretation servicesare needed during your visit.

We welcome you to complete the required forms in your New Patient Registration packet, and bring themwith you. Or you may complete the forms when you arrive at the health center for your visit. Please come at least 15 minutes early to any health center visit and please bring your insurance card, a photo ID, and a current medication list.

Patients are responsible for co-payments, unmet deductibles, and non-covered services at the time of their visit. Payment in full is also expected at the time of your visit if Hudson Headwatersdoes not participate with your insurance plan.

Hudson Headwaters keeps a record of all financial transactions related to your health care. You are responsible for paying in full for any non-covered services, co-payments or unmet deductibles at the time of your visit. Please be aware that you may receive services at a Hudson Headwaters facility that may be from providers, practices, or others who are not employed by Hudson Headwaters. In these cases, you may receive a separate bill from them for the services they provided to you in our facility. For your convenience, we have listed most of the other providerswith contact information, in the event you need to address a billing concern.

At Hudson Headwaters, we believe that no one should delay seeking needed medical care because they lack insurance or have high medical costs. Thats why we offer discounts and payment plans to eligible patients.

We offer a Sliding Fee program that discounts your cost for medical and dental care at our health centers (including primary and specialty care, many dental services, prescriptions, and x-rays). You may be eligible for the program, based on your household/family size and how much you earn, even if you have insurance.

You may be eligible for brand name medications for free. Participating pharmaceutical companies make medications available to eligible Hudson Headwaters patients. You may qualify if you have no insurance or prescription coverage, high prescription co-pays or deductibles, low to moderate family income, and ongoing medication needs.
