Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

My co-worker Maria L Gutierrez doesn't doesn't stop talking about Dr Cohen. She says: Dr Cohen is absolutely the best. Not just as a dentist, but as a person. He has a heart of gold. His staff Denisee, Annmarie, Joana, Stacey and Maggie make you feel like family. Don't waste your time with other dentists go to the best! I love him.

In life there are many essential trades people that you should know...doctors, a sheriff deputy, car mechanics, ac techs and especially a trustworthy Dentist. I have been going to Dr. Cohen for quite sometime and his whole office always goes above and beyond to ensure I am properly taken care of, which I truly appreciate. This is an A++ Dental Firm!

Been going to Dr. Cohen for 2 years. Have had a relaxing pleasant experience. Very patient and professional. Great staff too! Very accommodating. When you need any dental work, call them, you won't regret it.

My biggest phobia is the dentist, I've been scared since I was a child. After speaking to Joanna she helped me build the courage to book an appointment and come in for my first visit. Everyone who worked there was incredibly nice, they talked me through everything. And helped out my nerves at ease. Dr Cohen was amazing also. I will be returning and recommending to friends. Today was Big day facing my fears, and I couldn't have done it without the kind and compassionate staff at this office. Thankyou so much

Hi Folks... Introducing the most professionally agreeable AND pleasant dental practice that I've ever experienced! After living 60+ years, with well maintained teeth, and having had at least six different dentists, I think my experience counts. All truth be told, I was only ever tormented by my childhood dentist, so that doesn't count. :-) Though all my other dentists have been professional and useful, Dr Cohen (JC) and his staff are a perfect balance of services, cost, comfort and people-pleasers. And NO, nobody there asked me to do this, nor am I personally connected to any of them. Keep it up. Yaayyy !!!

My dentist almost 25 years. Dr.Cohen is dedicated to doing what's best for his patients and treating you as a whole person! Some staff have been with him almost just as long and are like family. Dr. Cohen has been my dentist for 17 years. He's not only an amazing dentist, he's a great man. He genuinely cares about his patients. You're treated more like family than a client. The front-office staff is wonderful as well. Maryanne is my hygienist and she's awesome. Absolutely love this place!

Dr. Cohen has been my dentist for almost 20 years. I have never had a negative experience under his care. He is extremely knowledgeable, kind, always willing to explain, generous, patient, and understanding. He has always taken such great care of me that I refuse to let anyone else anywhere near my mouth. I don't foresee ever changing dentists -- his prices are more than fair, the quality of work that he does is outstanding, and he is a great man to top it all off. If you need any work done on your teeth, go see Dr. Cohen and his team! You won't be disappointed.

Dr. Cohen is an extraordinary dentist. He is very knowledgeable about the latest dentistry and takes his time when treatment planning. He even has the latest technology, a scanner. He was able to scan my tooth instead of putting the gross impression material in my mouth. It was so much easier. Thanks DR. Cohen!

I have had my teeth done for 21 years at Dr. Cohen's office, and my teeth are in fantastic shape. Dr. Jeffrey Cohen is knowledgable and consistent in his dentistry, as he has been practicing for 30 years and counting. His office was just remodeled into a beautiful and unique dentistry office, unlike any office I have seen in South Florida. In addition to his upgraded practice, his staff are friendly and reliable. To anyone in South Florida that is new to the area or just looking for a great dentist I highly recommend Dr. Jeffrey M Cohen. You will actually look forward to the dentist. You are not just a patient, you are part of the family. If you have not been for a cleaning in many years don't feel bad or embarrassed...Dr. Cohen's staff is very down to earth and will make you feel right at home. I cannot say enough about Dr. Cohen and his entire staff . I have been a patient for 15 years and I actually recenly moved 250 miles from Dr. Cohen but I travel to his office for all of my dental needs, he is the only one I trust. Dr Cohen is very conservative and will never suggest any un needed treatment. He is a very fair dentist and I have always felt at home. A great office with a friendly and caring staff!
