I am a lifelong resident of south Louisiana. Originally from New Iberia, I went to high school at St. Thomas More in Lafayette and then entered the University of Louisiana. I graduated with a bachelor of science degree in Biology and Chemistry and entered the dental field as a clinical assistant in private practice. I returned to school and completed my bachelor's degree in Dental Hygiene through a joint program with UL and the LSU School of Dentistry.I worked for years as a dental hygienist before pursuing DDS degree at the LSU School of Dentistry in New Orleans. After finishing my training in New Orleans, I moved home and practiced dentistry in Lafayette until finding a practice of my own in Crowley, LA in the summer of 2018.
I am a member of the American Dental Association, Louisiana Dental Association, Acadiana Dental Association and other local study clubs. I am also a 2017 member of the Louisiana Dental House of delegates.
When I am not working, I love spending time with my husband and 3 young children. I enjoy cooking, riding bikes, going to the beach and reading, although I'll admit I don't get to as much as I'd like. I am also enjoying getting to know Acadia parish and the wonderful people who live here
I have worked at this office ever since with my dad, Dr. Anzalone, who was the previous dentist and now with Dr. Fontenot. The best part of my job is creating relationships with my patients and helping them have a beautiful, healthy smile!

I was so impressed with Dr.Jade and her staff!! Her assistants, who each work with her and explains what they are doing and ask several times if you are doing o.k. ! Her receptionist greets you and is always professional and helpful!

Hands down the best dentist for children. Dr. Fontentot and the entire staff go above and beyond to make your child and you feel comfortable. Never see anyone not smiling always very nice!!!!

HIGHLY RECOMMEND DR JADE FONTENOT' to anyone looking for dentist for their child. She explained everything she did what tools she used why she used it, Made sure my son was comfortable, she let me go on the back with him, she also offers the nitrous oxide aka laughing gas if it's necessary Amazing dentist, ,

I absolutely advise anyone looking for a dentist to try out Dr. Fontenot. I just had the best and most thorough exam ever with her. I was very pleased with her and her staff. Very friendly and very personable. So thankful I found these guys after my dentist passed away last Nov. Very happy with Dr. Fontenot.

I absolutely love Dr. Fontenot and her staff!!! They are the absolute best!! I need/needed a lot of work done to my teeth and gums and she came up with a whole plan and when I go to my visits she always asks how much am I willing to do and I always tell her as much as she wants!! Her tech's are amazing as well always asking me if I'm okay and if I'm hurting!! My fiance and I both go here and if anyone every asks me where to go I'll give her name in a heart beat!!! They also treat you like family and know you by name after yur first initial visit!!!

My two daughters and I just went there for the first time and everyone was amazing. Every person I met was awesome, I seriously have Zero complaints. They were very gentle and kind to my girls and I'm very thankful that I found this place. Anyone looking for a great dentist office, this is the place to go!

Was looking for a good dentist that was local. This place is amazing from the staff to Dr. Fontenot. My daughter had a root canal and a few days later they even called to check to see how she was doing. I've never had such a great experience with any dentist like this. If you choose to schedule an appointment with these great people you will not be disappointed.

Such a friendly group of people! I had seen the same dentist for many years and when I moved back to this area I really dreaded finding a new dentist I liked as well as my previous. Then I met Dr Jade Fontenot and her team! I highly recommend them. I love the view from the patient chairs! If you are cold, they will get you a blank. Need a neck pillow, they've got you covered. I have found my new dentist and super pleased! Thank y'all for taking good care of your patients!

I can not say enough about this dentist.. She has gone above an beyond to help me.. I had an emergency 1,day before my daughter wedding an trying to find a dentist on a Friday is already difficult especially if you dont already have one but they didnt hesitate an took me in an fixed me up to make,it threw the weekend.. Went back the next week to have a tooth pulled an was,still having alot of pain a week later so I called the emergency number an she,called me back within 10 mins with,helpful things to try.. Me an my entire family will be seeing her from now on... I,can't speak highly enough about dr fontenot an,her staff katie( I,think i spelled it right) is also amazing...
