Born in the Philippines and immigrated to the Bay Area of California. Left California for New York City where he was educated in the City Public School system. Attended William Howard Taft High School and later to The City College of New York , where he was elected Class president. Dr. Levi was also a Varsity Fencer. He received the Student Service Award and was a member of Caduceus Society, the Honorary Pre-Medical and Pre-Dental Society where he served as President. He attended New York University College of Dentistry where he received his Doctor of Dental Surgery Degree. He interned at the Bronx Lebanon Medical Center in the Bronx, and was later accepted to Columbia University College of Dental Medicine . He received his Post Doctoral Degree in Endodontics in 1974., and was appointed to the position of Assistant Clinical Professor in Endodontics.He has served at Columbia University CDM since 1974 to the present a total of 44 years of service.He is currently serving as an Associate Clinical Professor in the Department of Endodontics. He received the Oustanding Faculty Award in 2007, in recognition for 32 years of dedicated service to the Division of Endodontics. Dr. Levi was appointed an Adjunct Instructor at Nova Dental School NSU, and an attending at the West Palm Beach VA Hospital. Dr Levi practices in Teaneck NJ, Dumont NJ and in Union City NJ.

The tale of my time in Dr. Jack Levi's office is a joyful one that I honestly take pleasure in recounting. The atmosphere in the lobby immediately told me that this was a unique place. You never see that kind of personality in doctor's offices anymore--think retro. I immediately felt both intrigued and welcomed. The receptionist made pleasant conversation with me that distracted me from my nerves. The general mood was less corporate and more human, a welcome change. Everyone in this office took me seriously. They understood and believed that my pain was severe. They talked to me like I was a real person on their own level. As a young-looking girl in a wheelchair, I often struggle to be taken seriously by medical professionals. I am so relieved and grateful to have found an exception in Dr. Jack Levi and his associates. Dr. Levi was a miracle for me. He did an excellent job working on my teeth and gums. The procedure was not painful, even though it was one that required invasive manuevers. The operation completely removed the pain in my gum. Dr. Levi's efficiency told me that he is very experienced and talented as an endodontist. He was gentle and deft. His personality was golden. It was comforting to know that such a kind person was the one operating on me. Dr. Levi's assistant was clearly well-trained. Moreover, she was an extremely reassuring presence during the procedure. I knew that if I had any issues, she would be sure to help me right away. It made me feel secure. When I entered Dr. Levi's practice, I was in a near-constant state of pain so persistent and heavy that I couldn't think straight. When I got home that night, I was alleviated. I am beyond grateful for that alone, but it doesn't stop there. The people in that office made it clear that they truly cared for me, which is (in my opinion) rare and invaluable in the modern medical field. I cannot recommend Dr. Jack Levi enough, and I thank God that there are doctors like him.

Jack Levi was very rude to my girlfriend. No sort of compassion for another human being who has been through a lot in her life. He was very rude and violent to his nurse assisting on the root canal. At one point he shook her wrist violently because she held an instrument a little incorrect. I would not recommend this establishment at all.
