Born in the Philippines and immigrated to the Bay Area of California. Left California for New York City where he was educated in the City Public School system. Attended William Howard Taft High School and later to The City College of New York , where he was elected Class president. Dr. Levi was also a Varsity Fencer. He received the Student Service Award and was a member of Caduceus Society, the Honorary Pre-Medical and Pre-Dental Society where he served as President. He attended New York University College of Dentistry where he received his Doctor of Dental Surgery Degree. He interned at the Bronx Lebanon Medical Center in the Bronx, and was later accepted to Columbia University College of Dental Medicine . He received his Post Doctoral Degree in Endodontics in 1974., and was appointed to the position of Assistant Clinical Professor in Endodontics.He has served at Columbia University CDM since 1974 to the present a total of 44 years of service.He is currently serving as an Associate Clinical Professor in the Department of Endodontics. He received the Oustanding Faculty Award in 2007, in recognition for 32 years of dedicated service to the Division of Endodontics. Dr. Levi was appointed an Adjunct Instructor at Nova Dental School NSU, and an attending at the West Palm Beach VA Hospital. Dr Levi practices in Teaneck NJ, Dumont NJ and in Union City NJ.

NEVER I REPEAT NEVER CONTACT THIS DENTIST ! They had a STUDENT preform and 3 hour root canal on my tooth & when I called them to tell them how much pain I was in the doctor screamed at me and my mother st the top of their lungs UNPROFESSIONAL! Then I went in for a second time where the doctor grounded my tooth down to nothing so I didn't bite on it only to still be in pain, go in it a third time & WITHOUT NOVICAIN he went in & when I screamed it hurt stop stop he held me down by my shoulder in the chair, spitting and screaming into my open mouth STAY STILL STAY STILL as he proceeded to drill into my tooth with shaky hands pericing the inside of my mouth three times (WHICH STILL DID NOT COMPLETE THE ROOT CANAL & THE OFFICE WENT AHEAD AND SUBMITTED THE TOOTH WAS COMPETE) I cried my eyes out. Their office is something of the 1980's, they use note pads & planners instead of computers to hold their records, the dentists wife is the secretary who uses track flip phones as their office answering system & WHENEVER you call for an appointment the wife of Dr. Levi Jack tells you to call back and remind her of her schedule. Worst experience of my entire life and the treatment to have what he's done resolved has taken over 5 months and still counting because of insurance fruad. Also, they refuse to give me my medical records and hang up on me every time I call and request them. Now another endodontist has confirmed Dr. Levi's work is incomplete and I have to pay out of pocket to fix their incompete work. I REPEAT IF YOURE LOOKING FOR AN ENODONTIST DO NOT CONTACT DR. LEVI JACK, THE PROBLEM WILL ONLY BECOME WORSE.

My 13 years old daughter had an accident that caused her to chip both her front teeth as a mom it was the hardest thing to go through she was told she needed 2 root canals now my main concern was finding a Dr to do it as soon as possible and someone that took her insurance she has Medicaid w/ Amerigroup which a lot of Doctors don't take...Dr. Levy was on of the Dr's on the list that took it the same day I called his office his wife who was beyond the nicest lady I spoke too was able to put her in that same day. Dr. Levy is a professor at Columbia University in NY so his hours are not normal office hours I took her in on a Wednesday night at 9pm and he stared the process of the root canal. Dr Levy was the most caring Dr I have ever seen he was intrigued with the case my daughter had. He specializes in trauma root canals you can tell the passion he has for such cases he was beyond amazing and beyond gentle with my daughter. I was a total wreck during the procesure but she was in the best hands she could have been in. She went back a week later on a Sunday to his Dumont office it was a little bit of a wait but he's worth every minute you wait. He finished the root canal that same day. I've read some reviews abt him being rude or critizing his equipment but Dr. levy is one of a kind doctor which there isn't a lot of. A lot of doctors are in the business for the money which is understandable they worked hard for it. But Dr. Levy is in it for the passion he has for it. I HIGHLY recommend him best Dr I have ever met. We were truly blessed
