Dr. Tom Surber is a lifelong resident of Northwest Indiana. He attended St. Mary of the Lake Elementary School in Miller and graduated from Andrean High School. He attended Indiana University and received a Bachelor of Arts degree majoring in biology and minoring in chemistry.
Dr. Surber matriculated to Indiana University School of Dentistry (IUSD) in Indianapolis for his Doctor of Dental Surgery (DDS) degree. With a keen interest in orthodontics and as a former patient, Dr. Surber completed a two-year postdoctoral program at IUSD in the specialty of orthodontics and received his certification. A long-time owner of Orthodontic Affiliates, Dr. Surber recently opened a new practice where he combines state-of-the-art technology and the latest treatment techniques with his years of experience in order to provide the highest possible quality of orthodontic care.
Outside of orthodontics, Dr. Surber has several other interests and hobbies. He is an amateur musician who plays the piano and pipe organ. He is a past member of the Northwest Indiana Symphony Chorus. Tom is a commercial, multi-engine instrument rated pilot. Other hobbies include fishing, boating, exotic cars, scuba diving and snow skiing.
Dr. Surber and his wife Deborah, who have been married for 35 years, reside in Valparaiso, Indiana. Their son, Dr. Timothy Surber, is an oral and maxillofacial surgeon practicing in Merrillville and Valparaiso, Indiana, with Stevenson and Fairchild OMS. Their daughter, Sarah, is a doctor of physical therapy with advanced certification in manual skills. She practices in Carmel, Indiana. Tom and Debbie are active members of Immanuel Lutheran Church Valparaiso, Indiana.

I just love my sons teeth! The dr and staff are always so informative and do a wonderful job. It's it such an inviting environment and the girls always know how to make my son laugh and have a good time while getting his teeth worked on. I definitely recommend Ismile to anyone and everyone of all ages.

Beyond sketchy business practices. Even letters from the office contain spelling errors on the dentist's name. I spent a few dozen hours trying to get them what they needed, essentially doing their job for them with the previous dentist, orthodontist, and insurance company. They still have my money (~700) they will not pay me and it's been 8 months. They also make you feel like you're the one doing something wrong by refusing to let you discuss your child's care with the doctor privately. In particular, the older lady in the office refuses to do her job making you do the work. When she does do basic work with insurance and office tasks they are often incomplete or wrong. I've heard from other local doctors that they don't hesitate to send people into collections quickly. I'm lucky I never had to deal with this part, but may need a lawyer anyway with them. My insurance filed a formal complaint on my behalf without me even needing to ask after their experience with them.

Horrible customer service. My son had an appointment for a free consultation for braces, however because I recently lost my job, my wife an son we're kicked out half way through the screening. I quote Dr. Surber, "if they can't pay, I'm not going to waste my time." Wasn't aware a free consultation actually cost money. When I called to speak with the Dr. as well, he was too busy to speak with me and discuss. Stay away.

The doctors and the friendly staff really care for their patients. They go the extra mile. The patients, staff, and doctors truly make having braces a fun experience.
