Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Implant & Cosmetic Dental Care goes above and beyond for their patients. There are so many good things to say about Dr. Demirdjan's Office and his staff. I always feel welcomed and taken care of. They are all kind, respectful and patient. Dr. Demirdjan is one of the most honest and diligent dentists I've ever been to. His work is great. The office truly cares about you and your dental health. Dr. Demirdjan and his hygienist are extremely gentle and I am never in discomfort. You would truly be missing out on quality dental care if you didn't come here!

I found Dr.Demirdjan before pandemic at December when I have emergency teeth issue. He took care of me right away very professional. He explained to you everything and plan out things very well. The appointment is easy to make and no need to wait more than 10 mins. I went to 6 dentists and most of them make me feel they want to add more charges than it should be. I think I will not go anywhere else for my teeth. I don't mind to spend more money but it need to be a reason. Recommend Dr. Demirdjan !

I called to make an appointment and the woman was extremely professional and friendly. Clearly, I will never go anywhere else! From the moment you enter the office you are in a stress free environment. Very comfortable waiting room with calming music playing. Dr. Demirdjian is the best Dentist I have ever had in NY. He is extremely knowledgeable, professional and thorough from start to finish. I needed a front crown and he created a new tooth that looks amazing!!! I I never felt any pain, judgement, pressure or stress. His assistant was friendly and professional. Beyond any Dental office I have ever been in. I cannot wait for my next visit!

Dr. Edmond Demirdjan is the best dentist I've ever been to!!! He is very honest, kind, and extremely professional. My teeth have never been better since I started seeing him. His staff is great too. I visited Dr Demirdjan in October 2017 on a Saturday when an aching tooth became unbearable. Dr Edmond did the paper work briskly (travel insurance as I was in New York just for a visit) and then carefully diagnosed the problem. Treatment stopped the pain and enabled our visit to proceed with no further incident. On return to Melbourne a dentist associated with my own dentist has full treatment underway. I was referred to Dr Edmond by Dr Raffi Barsoumian with the assistance of the hotel where we were staying. Although the cost of US dental and medical services is mind blowing to an Australian the quality of clinical care provided by Dr Edmond was better than pretty good.

My family and I have been Dr. Demirdjan's patients for over 10 years. Dr. D provides friendly and professional service for both adults and children. We wouldn't go anywhere else.
