Hearts for Health's mission is to connect people to services that address the social determinants of health - no matter what door they walk in. You could walk in for a routine physical and share how you are struggling to find a job with your doctor and immediately see a Supported Employment Specialist at that same visit who begins helping you find a job. No matter why you come to visit the Hearts for Health Integrated Care Center we want to help you heal your mind, body, and spirit.
The Hearts for Health Integrated Care Center brings together medical, dental, behavioral, and mental health care under one roof, combined with a wide range of educational opportunities. We believe this will reduce stigma and dramatically improve people's ability to get the care they need, when they need it and with a very high level of confidentiality.
The Hearts for Health Integrated Care Center is a partnership between Winding Waters Clinic and Wallowa Valley Center for Wellness to provide a multitude of healthcare services under one roof. Winding Waters Clinic offers dental, medical, and behavioral health services in the building. Wallowa Valley Center for Wellness provides integrated mental and behavioral health services as well as developmental disability services and is the County Mental Health program. Both organizations offer healthy living educational classes as needed in the building. This allows our healthcare providers to work together in treating the whole person and to work across silos.
Holistic Patient Care Under One Roof -the integrated care center has medical, dental, and mental, and behavioral health care all in one facility and is located near the hospital, physical therapy, and senior living. There are 5 dentaloperatories, 5 medical exam rooms, 14 therapy spaces as well as educational rooms like the teaching kitchen and the Oregom Community Room.
A Healing Place for All - we see this building as a community space and for everyone to learn, grow and heal. This building has a large meeting space (called the Oregon Community Room), a teaching kitchen, and a family resource center in the lobby for our community to use.
